Psychological problems of kidney transplantation were examined in 275 hemodialysis patients. Desire for transplantation was significantly lower in aged patients than in nonaged cases. The prevalence of
alexithymia
which is characterized by difficulty in expressing one's feelings and a lack of fantasy and which may be a clinical feature associated with psychological defense mechanisms was significantly higher in aged than in nonaged patients. The results suggest that aged patients tend to suppress the desire for transplantation by means of psychological defense mechanisms such as 'denial' directed toward transplantation and an unconscious pretence of
indifference
. From the psychological aspect, reconsideration of the quality of life may be required for hemodialysis patients.

The present study assessed the relationship between numbing and three associated conditions of
alexithymia
,
apathy
, and depression, utilizing data collected on 353 Vietnam combat veterans diagnosed with Posttraumatic Stress Disorder from in- and out-patient settings and an outreach center at various Department of Veterans Affairs Medical centers. All subjects completed four self-report measures: the Glover Numbing Scale, the Beck Depression Inventory, the
Apathy
Evaluation Scale, and the Toronto
Alexithymia
Scale-20. The correlation matrix indicated that scores on the four measures were moderately to highly correlated. Principal components analysis with a varimax rotation indicated a five-factor solution that provided evidence for the factorial validity of each of the constructs assessed. Results of the factor analysis of items from the four measures were consistent with numbing being a separate and distinct construct from
alexithymia
,
apathy
, and depression. In general, results indicated that all constructs measured were separate and distinct from one another.

In a cross-sectional study, the authors investigated 40 female patients with fibromyalgia syndrome (FS) for the degree of
alexithymia
and parental bonding style.
Alexithymia
was assessed by the Toronto
Alexithymia
Scale-26; parental style by the FDEB (a German version of the Measure of Parental Style). In 15% of patients with FS, clinically significant
alexithymia
was found. Also, there was a positive association between the
alexithymia
scores (TAS total score) and "maternal abuse" and higher values in "paternal
indifference
," which predicted higher scores on "Difficulties identifying feelings" (TAS). The results of this study suggest that parental styles are associated with higher
alexithymia
scores.

Alexithymia
, an impairment of affective and cognitive emotional processing, is often associated with human immunodeficiency virus (HIV) and may reflect effects of the virus on brain areas that are also important for multiple cognitive functions, such as the prefrontal and anterior cingulate cortices. We hypothesized that there would be a correlation between extent of
alexithymia
and cognitive performance associated with these brain areas, including attention, executive function, and visuospatial processing. Thirty-four asymptomatic HIV+ participants and 34 matched healthy HIV- volunteers were administered the Toronto
Alexithymia
Scale, a series of neuropsychological tests, and measures of
apathy
, depression, and quality of life (QoL). The HIV+ participants had significantly higher levels of
alexithymia
, depression and
apathy
than the HIV- group. The extent of
alexithymia
and two of its processing components (Difficulty Describing Feelings [DDF] and Externally Oriented Thinking), but not depression, correlated with performance on measures of executive and visuospatial abilities, consistent with dysfunction of the frontostriatal circuits and their cortical projections.
Apathy
was related to
alexithymia
and two processing components (Difficulty Identifying Feelings and DDF) but to only one cognitive measure. The higher rate of
alexithymia
, as well as cognitive dysfunction, in HIV may be a consequence of the infection on the frontostriatal system and its cortical connections. Our findings also demonstrated a dissociation of
apathy
and
alexithymia
in HIV, pointing to overlapping but distinct neural substrates within frontostriatal circuits.
Alexithymia
correlated strongly with QoL ratings, underscoring the importance of assessment and treatment of HIV-associated emotional and cognitive processing deficits.

This study aimed at investigating the association between motor subtypes and alexithymic features in patients with newly diagnosed untreated (de novo) Parkinson's disease. This objective derived from empirical findings about an association between the postural instability/gait difficulty motor subtype of Parkinson's disease and more marked symptoms of depression, an affective disorder strongly related to
alexithymia
. A total of 42 patients with de novo Parkinson's disease underwent neuropsychiatric assessment, including the toronto
alexithymia
scale (TAS-20) and the geriatric depression scale (GDS-15). On the basis of scores reported at the unified Parkinson's disease rating scale III section, patients were classified within postural instability/gait difficulty motor subtype tremor dominant motor subtype and mixed motor subtype. Patients of the postural instability/gait difficulty motor subtype reported significantly higher alexithymic features compared to patients of the other motor subtypes. Considering the strong association between
alexithymia
and depression, this finding is in line with previous findings reporting that the postural instability/gait difficulty subtype of Parkinson's disease is associated to more marked psychopathological features, especially affective features (depression and
apathy
). In conclusion this brief report suggests the usefulness of an early neuropsychiatric assessment of affect regulation difficulties in PD patients, especially in those with a prevalence of akinetic/rigid symptoms.

We investigated
alexithymia
, a deficit in the ability to identify and describe one's emotions, in a sample that included patients with neurodegenerative disease and healthy controls. In addition, we investigated the relationship that
alexithymia
has with behavioral disturbance and with regional gray matter volumes.
Alexithymia
was examined with the Toronto
Alexithymia
Scale-20, behavioral disturbance was assessed with the Neuropsychiatric Inventory, and regional gray matter volumes were obtained from structural magnetic resonance images. Group analyses revealed higher levels of
alexithymia
in patients than controls.
Alexithymia
scores were positively correlated with behavioral disturbance (
apathy
and informant distress, in particular) and negatively correlated with the gray matter volume of the right pregenual anterior cingulate cortex, a region of the brain that is thought to play an important role in self and emotion processing.

Patients with Parkinson's disease (PD) typically present with motor symptoms, but several non-motor symptoms, such as cognitive impairment, autonomic dysfunction and neuropsychiatric symptoms, are usually also present, when adequately looked for. The objective of this paper is to provide an up-to-date, comprehensive review of the influence of affective disorders, mainly depression and
apathy
, on cognitive functioning of PD patients. Reviewed empirical findings suggest that, although depression and
apathy
have differential neurobiological bases in PD, both are associated to an increased risk of cognitive impairment, especially of executive functions, in this clinical population. The potential influence of other affective disorders, as anxiety and
alexithymia
, on cognitive functioning of PD patients is actually almost unknown and needs further empirical investigation. The clinical implication of these findings is that the best assessment and management of PD patients should include both neuropsychological and neuropsychiatric evaluations and the presence of non-motor symptoms as cognitive disturbances and affective features should be investigated with patients and caregivers.

Non-motor symptoms such as neuropsychiatric and cognitive dysfunction have been found to be common in Parkinson's disease (PD) but the relation between such symptoms is poorly understood. We focused on
alexithymia
, an impairment of affective and cognitive emotional processing, as there is evidence for its interaction with cognition in other disorders. Twenty-two non-demented PD patients and 22 matched normal control adults (NC) were administered rating scales assessing neuropsychiatric status, including
alexithymia
,
apathy
, and depression, and a series of neuropsychological tests. As expected, PD patients showed more
alexithymia
than NC, and there was a significant association between
alexithymia
and disease stage.
Alexithymia
was associated with performance on non-verbally mediated measures of executive and visuospatial function, but not on verbally mediated tasks. By contrast, there was no correlation between cognition and ratings of either depression or
apathy
. Our findings demonstrate a distinct association of
alexithymia
with non-verbal cognition in PD, implicating right hemisphere processes, and differentiate between
alexithymia
and other neuropsychiatric symptoms in regard to PD cognition.

Pregnancy denial is rare yet reported and is often the result of complex psychosocial circumstances. We present an unusual case of pregnancy denial associated directly with both remote and ongoing trauma. A woman suddenly gave birth to a child in a hospital while visiting her other daughter, resulting in emergent labor and delivery as well as social work and psychiatric evaluation. Various atypical findings were noted, including pathological hair-pulling,
alexithymia
,
indifference
, and pregnancy denial. We offer a biopsychosocial conceptualization of the case, commenting on various possible processes including dissociation. The case also explores current states of knowledge regarding the interaction between impulse control disorders such as trichotillomania, dissociation, and trauma, with a call for future clinical and investigational attention to these interactions.

This study determined convergence and divergence in the constructs of
alexithymia
,
apathy
, and depressive symptoms. Understanding of similarities and differences between these constructs will improve diagnostic accuracy for clinical and research purposes. Community-dwelling participants (N=622, M age=35.6 years, SD=13.1) completed online measures of
alexithymia
, depression, and
apathy
; 12.2% were alexithymic, 37.8% reported significant depressive symptoms, and 24.9% reported significant
apathy
. Exploratory Factor Analyses (EFAs) determined the best factor structure for the
apathy
,
alexithymia
, and depressive symptoms was comprised of three factors and accounted for 45.1% of item variance. The Depression,
Apathy
, and
Alexithymia
factors were defined most strongly by item content that is at the core of each construct. Depression was defined most highly by items assessing sadness, low self-esteem, and loneliness. The strongest item loadings for
Alexithymia
were difficulty identifying and describing feelings.
Apathy
was characterized by poor motivation, low interest, and lack of initiative. However, each of these core and defining features had significant cross-loadings on one of the other two factors. Negative affect shared variance with
Apathy
, low motivation shared variance with Depression, and difficulty describing and identify feelings shared variance with Depression and
Apathy
. Clinical and research implications are discussed.
