Thirty-eight in-patients with endogenous- and 20 in-patients with non-endogenous depression, took part in a multi-centre 3-week double-blind trial where patients were randomly allocated to treatment with either 6 g L-tryptophan or 150 mg imipramine daily. Item analysis of Hamilton ratings, before the investigation and weekly during the trial period demonstrated few statistically different mean scores on individual items between the two treatment groups. Identical principal components factor analyses of total Hamilton Rating Scale for Depression scores were conducted for two separate sub-samples (N = 183 and N = 182) drawn from the same clinic population of moderately depressed outpatients. A comparison of the two factor analyses revealed substantial agreement for four factors across the two sub-samples (i.e., four factors from the first sub-sample correlated at least 0.80 with a homologous dimension in the second sub-sample). The four factors were labelled Somatic Complaints, Anorexia, Sleep Disturbance, and

/Retardation. Some additional factors emerged in the analysis of one sub-sample but failed to appear in the other. Overall, these results suggest that the
HRS
exhibited a relatively stable factorial structure based on a large sample of outpatients with unipolar depressive disorders. Methodological problems with earlier research are discussed in light of the current findings.

A particular importance can assume, especially during the onset of Alzheimer's disease, the psychiatric symptomatology characterized by depressive mood, auto and hetero-aggressive behaviour,

, anxiety disorders, sleep disorders, delusions and hallucinations, these pictures showing a prevalence of 30%. 13 patients affected by Alzheimer's disease at the onset, of both sexes, age ranging from 45 to 80 years, were included in the study. The clinical picture was assessed by BDS, GDS, ICS, IADL, BIMC, MMSE, ADAS, a neuropsychological test battery such as Token Test, verbal fluency test for semantic categories, prose memory test, scribble discrimination test, numeric matrix test, Raven test, judgement of line orientation. A computerized test battery by NYU Computerized Test Battery have been also administered. The psychiatric clinical picture has been evaluated by
HRS
-D and the non cognitive session of ADAS. There was a positive significant (r = 0.85, p = 0.0004) correlation between depressive symptomatology evaluated at
HRS
-D and deterioration of daily living activities evaluated at the Blessed Dementia Scale (BDS). On the other hand the deterioration of cognitive capacities was not significantly correlated with a worsening of behavioural aspects. In conclusion seems to have a particular importance, at the onset of Alzheimer's disease, the psychopathological component that often seems to be the principal component in the determination of the early deterioration of daily living and behaviour or patients.

We examined the factor structure of the 17-item Hamilton Rating Scale for Depression (HRS-D) in 206 community-dwelling elderly patients. Using principal components analysis and quartimax rotation, a four-factor structure involving all 17 items and accounting for 57.7% of the variance was desired. The factors represented the following dimensions of depressive symptomatology and illness: depressed affect, vegetative symptoms, anxiety, and
agitation
/insight. This factor structure reflects the presentation of depressive symptomatology and depressive illness in this population. Findings suggest that the
HRS
-D can be used for clinical assessment of depressive symptomatology along major dimensions of depressive illness in community-dwelling elderly.
