The investigation of Raynaud's phenomenon in angiological practice has not yet been standardized. Two points need to be resolved. The cold test is either too empirical and inaccurate or non-uniform. None of the tests of functional vascular exploration have been proven to be reliable, except, perhaps, for capillaroscopy. Laser Doppler studies the cutaneous microcirculatory velocity and its response to thermal stimulation. An isothermic chamber can be used to
chill
the skin and reheat the skin according to a simple and standardized technique. This protocol was performed in 14 normal subjects, 28 patients with Raynaud's disease and 19 patients with Raynaud's phenomenon, which is clearly individualized. In Raynaud's disease, there appears to be an exageration of cutaneous sympathetic tone compared to normal subjects. This methodology is simple to perform and it provides rapid diagnostic results in the consulting rooms.

A 57 years old man had a two months history of
chills
and fever with abdominal pain. Blood cultures were positive for Salmonella ohio. He suddenly became hypotensive with oedema of lower limbs. Angiographic findings were infrarenal aortic rupture with pseudoaneurysm formation and inferior vena cava compression. After ligation of the aorta and both common iliac arteries, an axillo-bi-femoral graft was constructed to bypass the infection area. Subsequent occurrence of lumbar osteomyelitis required debridement and drainage of retroperitoneum. The patient was discharged on oral amoxicilline given for eleven months. Two years postoperatively he is able to walk without evidence of further infection. With a review of 32 other cases in the literature, emphasis is placed on theories of pathogenesis and on modes of surgical management.

We report a highly probable case of moderately severe blackwater fever. A French woman, living in Guinea Bissau, was used to taking self-medication halofantrine for malaria. On this occasion, she felt unusual
chills
and pyrexia after a non documented bout of malaria, followed by nausea, then jaundice with dark-red urines despite another treatment with halofantrine. A sepsis was eliminated by two negatives thick peripheral blood drop examinations. Hemolysis was noted with 8.1 g/dl of hemoglobin, Coombs positive, and LDH at 1,452 IU/l, associated to renal failure with 34 ml per minute of clearance. The outcome was favourable with rehydration. Blackwater fever has been described with the three aminoalcohols, but mainly in severe presentations. Clinicians are not familiar with this disease, even though it has major therapeutic implications: quinine, halofantrine, and mefloquine become strictly contra-indicated. Moderate forms may be unknown, and this observation should be taken into account to prevent mistreatment in future patients.
