Many studies, concerning cardiovascular reactivity in hypertensives, show contrasting data. The aim of the present study was to check, also measuring extracardiovascular variables, a procedure able to identify a peculiar characteristic of the prehypertensive phase. We studied 47 normotensives, who referred high blood pressure values, but that we did not find in our visit. The cardioneurovascular assessment was evaluated, by means of a non invasive, beat to beat technique, measuring SBP, DBP, HR, muscular contraction and skin conductance level (EMG,
SCL
), peripheric temperature (PT), during a psychophysiologic session. This was composed by 4 stressors (mental arithmetic and Sacks test, acoustic and electric stimulations), 5 minutes each, preceded and followed by an observation period of 10 minutes. After 18 month follow-up, we could distinguish 26 hypertensives (H), and 21 subjects maintained normal blood pressure values (N). The obtained findings showed, with statistical significance, 1) the hyperresponsiveness of
SCL
and PT; 2) the failed recovery, with consequent hyperdysreactivity, of
SCL
and PT; 3) the presence of both these phenomena in the H., while SBP, DBP and HR responses did not result a discriminative tool. These data seem 1) to reinforce the hypothesis that a hypersympathetic phase can characterize the prehypertensive stages of
essential hypertension
and 2) to suggest the psychophysiological approach as a useful method to diagnose prehypertension.

Sixty-one male subjects with mild untreated
essential hypertension
were classified by renin-sodium profiling as high renin (HR--13 Subjects), normal renin (NR--33 Subjects), or low renin (LR--15 Subjects). The HR subjects reported significantly more symptoms of sensitivity, depression, anxiety, hostility, paranoia, and psychotic thought than LR subjects on the Symptom Checklist (
SCL
-90). The NR subjects also reported more symptomatology than LR subjects. Similar differences between HR and LR subjects were found with the Cattell 16 Personality Factor Questionnaire (16PF).

Previous research has yielded inconsistent results in the control of
essential hypertension
. One explanation for response to drug or behavioral therapy may be the patient's psychological status upon entering treatment. Thirty-five borderline hypertensive males entered into a self-management program with biofeedback and cognitive restructuring components. The
SCL
-90 (Symptom Checklist-90) and the Holmes' Schedule of Recent Events were used to determine if responders and nonresponders could be distinguished prior to treatment on the basis of psychological status. Patients whose hypertension was resistant to treatment (diastolic greater than or equal to 90 mm Hg) reported significantly greater levels of psychological distress and greater life changes than did controlled patients. Pretreatment differences could not be explained by compliance or expectation of success. This study suggests that a relationship exists between psychological distress, life changes and the subsequent control of hypertension; this has implications for treatment selection and design for psychologically distressed individuals.

A consecutive unselected series of 20 outpatients suffering from
essential hypertension
is included in this study. These patients were compared to a control group of 20 patients other than hypertensives, matched for age, sex, marital status, years of schooling, social class and duration of illness. Stressful life events prior to illness onset, psychological distress as measured by
SCL
-90, and alexithymia were investigated. Patients with hypertension were exposed to undesirable life events before disease onset and exhibited alexithymic traits significantly more than the control group.
