essential hypertension
(EH) and 70 cases of normal subjects were researched for correlation between
TCM
differential types and plasma levels of renin, angiotension II, aldosterone, atrial natriuretic factor (ANF) in patients with EH. Results indicated that: (1) basic level of renin was lower in patients with EH than that in normal subject. There were significant differences of plasma levels of renin between different
TCM
types. Plasma renin level of excessive Yang patients was higher than that in normal subject group and groups of deficiency of Yin essence combined with excessive Yang as well as deficiency of both Yin and Yang (P less than 0.01-0.001). Plasma level of angiotension II was significantly higher in group of excessive Yang than that in normal subject and other two groups (P less than 0.01-0.001). It was indicated that there were correlation between plasma basic level of renin, angiotension II and
TCM
types. (2) Plasma ANF level in patients with EH was significantly lower than that in group of normal subject (P less than 0.01). There were significant differences between groups of three different
TCM
types (P greater than 0.05). The result suggested that lower plasma ANF level was general character in three groups with EH. The prognosis of these patients was discussed.

A study which enrolled 65 middle-aged and elderly essential hypertensive patients with Kidney-deficiency pattern was carried out to evaluate the effects of Longevity-Antihypertensive-Mixture (LAM). As LAM was composed of Kidney-tonifying herbs, all the subjects chosen fell into the pattern of Kidney-deficiency in
TCM
. The subjects were randomized into two groups: The LAM group had 34, and the control group taking Apocyhum Venetum L. Fluid (AVLF) 31. The duration of medication was 12 weeks. The main results were as follows: (1) At the end of week 4, the supine systolic blood pressure (SBP), diastolic blood pressure (DBP) and mean blood pressure (MBP) in LAM group declined from 171 +/- 16 (mmHg, the same below), 101 +/- 6 and 127 +/- 7 to 153 +/- 12, 93 +/- 7 and 113 +/- 7 respectively. At the end of week 12, the supine SBP, DBP and MBP were 151 +/- 14, 93 +/- 6 and 112 +/- 8. There was significance of difference in the reduction of supine blood pressure compared with baseline (P less than 0.001). AVLF produced similar changes in the reduction of supine SBP, DBP and MBP. However, the magnitude of reduction in SBP and MBP was smaller than those with LAM (P less than 0.001). (2) There was no significance of difference in attaining goal blood pressure between LAM and AVLF groups (P greater than 0.05). At the end of week 4, 79.41% and 77.42% were achieved respectively; at 12, 79.41% and 80.65%. (3) LAM had stronger effect on relieving symptoms of
essential hypertension
and Kidney-deficiency than AVLF (P less than 0.05). (4) In LAM group the left ventricular mass index (VMI) decreased from 114.75 +/- 42.40 g/m2 to 100.39 +/- 36.08 g/m2 (P less than 0.001). The LVMI in AVLF group increased from 117.27 +/- 36.90 g/m2 to 117.68 +/- 38.37 g/m2 (P greater than 0.05). The results supported the therapeutic principle of
TCM
: Treating patients according to their pathophysiological patterns.

In this study 31 normal child-bearing women, 62 postmenopausal women, 93 cases of female type II diabetes (18 child-bearing and 75 postmenopausal cases), 53 cases of coronary heart disease (11 child-bearing and 42 postmenopausal cases) and 38 cases of
essential hypertension
(8 child-bearing and 30 postmenopausal cases) were investigated. The average score of Kidney deficiency was 22.9-8. 5 before treatment with the combination of
TCM
and WM. With the treatment of
TCM
in diabetes and coronary heart disease and of Qigong in
essential hypertension
, the score decreased to 11.5-4. 4 (P less than 0.001). Serum/saliva estradiol (E2), the ratio of E2 to testosterone (T, E2/T) and progesterone (P) decreased before treatment of
TCM
or Qigong. After treatment E2 and P value increased; the ovarian endocrine function was improved; the special symptoms of the diseases relieved, fasting blood glucose levels in diabetics, the frequency and severity of angina pectoris in coronary heart disease and the blood pressure in

significantly decreased respectively (P less than 0.01). The study suggested that there are certain relations between ovarian endocrine disfunction and Kidney deficiency. The more severe the "Kidney deficiency" was, the more significant the changes of sex hormone were.

Nitric oxide (NO) production is reduced in patients with
essential hypertension
and in some experimental models. We have investigated the effect of trichlormethiazide and captopril on NO synthase (NOS) activity and glomerular damage in the kidney of deoxycorticosterone acetate (DOCA)-salt hypertensive rats. DOCA-salt rats were induced with weekly injections of DOCA (30 mg/kg body weight (BW) and 1% saline in drinking water after right nephrectomy. As antihypertensive therapies, CAP (captopril, 40 mg/kg BW) and
TCM
(trichlormethiazide, 10 mg/kg BW) were given after induction of DOCA-salt hypertension. The increased blood pressure was significantly lowered by
TCM
, but not by CAP after 5 weeks. Nitrite production in kidney slices was suppressed in DOCA-salt rats, and immunoreactivity for both brain-type NOS (B-NOS) in macula densa and endothelial-type NOS (EC-NOS) in renal vessels was decreased.
TCM
significantly increased the nitrite production in the kidney slices and B-NOS immunoreactivity, whereas these changes were less in CAP. Glomerulosclerosis score was significantly higher in DOCA-salt rats, and
TCM
ameliorated renal damage more effectively than CAP. These results indicate that the reduced nitrite production in the kidney of DOCA-salt hypertensive rats was increased more effectively by trichlormethiazide than by captopril, via increased immunoreactivity for B-NOS in the macula densa, and prevented renal damage.

Forty-five patients of
essential hypertension
differentiated into two
TCM
types, i.e. Gan Yang Shang Kang Zheng (GYSK) and Yin Xu Yang Kang Zheng (YXYK) were randomly selected. Among them, the 31 patients received qi-gong therapy including 12 GYSK cases (group b) and 19 YXYK cases (group c) and 14 patients (group d) received nifedipine therapy. It was found that the plasma 6-K-PGF1 alpha was increased and TXB2 as well as TXB2/6-K-PGF1 alpha ratio were decreased after the therapy (P < 0.05) in group b, c and d. No statistical significant difference was found between group b and group c (P > 0.05). The results suggest that qi-gong is regulatory on TXB2 and 6-K-PGF1 alpha in patients with
essential hypertension
and is identical in the two different
TCM
Zheng types.

. Methods. Five major electronic databases were searched up to August 2012 to retrieve any potential randomized controlled trials designed to evaluate the clinical effectiveness of LWDHW combined with antihypertensive drugs for
essential hypertension
reported in any language, with main outcome measures as blood pressure. The quality of the included studies was assessed with the Jadad scale and a customized standard quality assessment scale. Results. 6 randomized trials were included. The methodological quality of the trials was evaluated as generally low. The pooled results showed that LWDHW combined with antihypertensive drugs was more effective in blood pressure and the scale for
TCM
syndrome and symptom differentiation scores compared with antihypertensive drugs alone. Most of the trials did not report adverse events, and the safety is still uncertain. Conclusions. LWDHW combined with antihypertensive drugs appears to be effective in improving blood pressure and symptoms in patients with
essential hypertension
. However, the evidence remains weak due to the poor methodological quality of the included studies.

Objectives. To assess the clinical effectiveness and adverse effects of Zhen Gan Xi Feng Decoction (ZGXFD) for
essential hypertension
(EH). Methods. Five major electronic databases were searched up to August 2012 to retrieve any potential randomized controlled trials designed to evaluate the clinical effectiveness of ZGXFD for EH reported in any language, with main outcome measure as blood pressure (BP). Results. Six randomized trials were included. Methodological quality of the trials was evaluated as generally low. Four trials compared prescriptions based on ZGXFD with antihypertensive drugs. Meta-analysis showed that ZGXFD was more effective in BP control and
TCM
syndrome and symptom differentiation (TCM-SSD) scores than antihypertensive drugs. Two trials compared the combination of modified ZGXFD plus antihypertensive drugs with antihypertensive drugs. Meta-analysis showed that there is significant beneficial effect on
TCM
-SSD scores. However, no significant effect on BP was found. The safety of ZGXFD is still uncertain. Conclusions. ZGXFD appears to be effective in improving blood pressure and hypertension-related symptoms for EH. However, the evidence remains weak due to poor methodological quality of the included studies. More rigorous trials are warranted to support their clinical use.

Background. Tianma Gouteng Yin (TGY) is widely used for
essential hypertension
(EH) as adjunctive treatment. Many randomized clinical trials (RCTs) of TGY for EH have been published. However, it has not been evaluated to justify their clinical use and recommendation based on
TCM
zheng classification. Objectives. To assess the current clinical evidence of TGY as adjunctive treatment for EH with liver yang hyperactivity syndrome (LYHS) and liver-kidney yin deficiency syndrome (LKYDS). Search Strategy. 7 electronic databases were searched until November 20, 2012. Inclusion Criteria. RCTs testing TGY combined with antihypertensive drugs versus antihypertensive drugs were included. Data Extraction and Analyses. Study selection, data extraction, quality assessment, and data analyses were conducted according to the Cochrane standards. Results. 22 RCTs were included. Methodological quality was generally low. Except diuretics treatment group, blood pressure was improved in the other 5 subgroups; zheng was improved in angiotensin converting enzyme inhibitors (ACEIs), calcium channel blockers (CCBs), and "CCB + ACEI" treatment groups. The safety of TGY is still uncertain. Conclusions. No confirmed conclusion about the effectiveness and safety of TGY as adjunctive treatment for EH with LYHS and LKYDS could be made. More rigorous trials are needed to confirm the results.

Background. Chinese herbs are potentially effective for hypertension. Qi Ju Di Huang Wan (QJDHW) is a commonly used Chinese herbal medicine as a monotherapy or in combination with other antihypertensive agents for the treatment of
essential hypertension
(EH). However, there is no critically appraised evidence such as systematic reviews or meta-analyses on the effectiveness and safety of QJDHW for EH. Methods and Findings. CENTRAL, PubMed, CBM, CNKI, VIP, and online clinical trial registry websites were searched for published and unpublished randomized controlled trials (RCTs) of QJDHW for
essential hypertension
up to January 2013 with no language restrictions. A total of 10 randomized trials involving 1024 patients were included. Meta-analysis showed that QJDHW combined with antihypertensive drugs was more effective in lowering blood pressure and improving
TCM
syndrome for the treatment of
essential hypertension
than antihypertensive drugs used alone. No trials reported severe adverse events related to QJDHW. Conclusions. Our review suggests that QJDHW combined with antihypertensive drugs might be an effective treatment for lowering blood pressure and improving symptoms in patients with
essential hypertension
. However, the finding should be interpreted with caution because of the poor methodological quality of included trials. There is an urgent need for well-designed, long-term studies to assess the effectiveness of QJDHW in the treatment of
essential hypertension
.
