The haemodynamic parameters were studied dynamically by way of noninvasive techniques--tetrapolar thoracic rheography in the course of treatment of 48 patients with essential hypertension Stage IB and IIA urth beta-blocking agents Obsidan and Visken. It was found that as early as by the 5th day of therapy the stroke volume and heart rate decreased, the maximum fall being noted after 2-3 weeks of treatment. The reflex elevation of the total peripheral resistance was moderate, as a result of which a gradual reduction of the arterial pressure developed. The state of the cerebral circulation significantly improved in the course of the treatment (as shown by rheoencephalography) and the tone of the cerebral arteries decreased. After 2-3 weeks of beta-blockers therapy favourable shifts were observed in the response of the general and cerebral haemodynamics to the orthostatic test.

Thirty patients suffering from essential hypertension were treated successively with placebo, hydralazine alone, and pindolol (Visken) combined with hydralazine. Hydralazine caused a statistically significant reduction in blood pressure and an increase in pulse rate as compared with the placebo: palpitations were an irritating side-effect. The combination of hydralazine and pindolol resulted in an additional fall in blood pressure (p less than 0.001) and the pulse rate decreased (2 p less than 0.001). The combined treatment also had a favourable effect on palpitations and angina pectoris.

1. In 20 of 20 patients (100%) with borderline hypertension Visken normalized the labile blood pressure. The high renin values after stimulation due to orthostasis and saluresis decreased significantly (p less than 0,01). 2. In 3 of 12 patients (25%) with hyporeninemic essential hypertension Visken alone normalized the blood pressure. The low renin values increased to the normal range. 3. In 4 of 18 patients (22%) with normoreninemic essential hypertension Visken normalized the blood pressure. The normal renin values showed a decreasing tendency within the normal range. 4. In 4 of 10 patients (40%) with hyperreninemic essential hypertension Visken normalized the high blood pressure. In 3 of these patients renin decreased distinctly. 5. In 4 of 20 patients (20%) with renal hypertension a therapy with Visken alone normalized the blood pressure. In 3 patients the high renin values decreased to the normal range. 6. In the most other patients of the groups II to V the additional therapy with diuretics and reserpine normalized the blood pressure. In these cases the renin values showed different reactions corresponding to the different effects of betablocking agents, saluretics or reserpine on the plasma renin activity [7]. 7. It is interesting, that Visken not only suppresses high renin values (borderline hypertension [6], hyperreninemic essential hypertension), but also increases low renin values to the normal range in patients with hyporeninemic essential hypertension. Because in essential hypertension the high blood pressure per se may be responsible for the renin suppression [3,4], this increase of renin activity is possibly the consequence of blood pressure reduction, while the decrease of renin activity after Visken may be the cause of blood pressure reduction.

Using microcatheter blood pressure telemetry, the pressure-lowering effect of prindolol (Visken), a beta-blocking drug, was tested at a dose of 10 mg three time daily by mouth in eight patients with essential hypertension (WHO stage II) and low to normal plasma-renin activities. After about ten days of treatment, the hypertensive reaction to everyday physical exercise (walking, climbing stairs, bicycle ergometry) was especially favourably affected. The average blood-pressure reduction at rest was 18.6/12.1 (systolic/diastolic), on stair climbing 41.5/17.2 mm Hg.
