27 patients with IIB stage of
essential hypertension
(EH) and left ventricular hyperthropy (LVH), 14 of them with IHD, were studied. In all of them coronaro- and ventriculography was performed. In 14 cases atherosclerotic alterations of coronary arteries were present. Nobody had abnormal pump function or contractility of myocardium. End-systolic, end-diastolic and stroke volume indexes in EH patients with pronounced coronary atherosclerosis were significantly increased. The examined patients demonstrated significant alteration of the left ventricular myocardium relaxation function. The conclusion is drawn that on a certain stage LVH is of compensatory character and plays an important role in supporting adequate myocardial pump function.

Effects of dynamic exercise on lymphocyte beta 2-adrenoreceptor density (BARD) and adenylate cyclase activity (ACA) basal and stimulated by isoproterenol (I). Gpp(NH)p, Gpp(NH)p+I, and forskolin (F), on plasma adrenaline and noradrenaline levels, renin activity (PRA) were compared in 6 healthy donors and 12 patients with
essential hypertension
(EH). Acute stimulation of sympathetic activity by dynamic exercise leads to a rapid increase in lymphocyte BARD in normotensive and hypertensive subjects. The rise in BARD was accompanied by a significant increase in lymphocyte basal ACA in normotensive subjects, but not hypertensive patients. In patients with EH, basal ACA changed after exercise according to their renin status: patients with low baseline PRA showed a decrease in basal ACA after exercise, whereas patients with normal baseline PRA showed an increase as did normotensive subjects. Patients with decreased basal ACA after exercise also showed a lower ACA stimulation by IPR, F, Gpp (NH)p and Gpp(NH)p+IPR, which did not change after exercise. These patients also exhibited substantial rises in PRA with exercise. These was an inverse relationship between exercise-induced changes in PRA and basal ACA. So the acute regulation of lymphocyte BARD-ACA system is changed in a subset of patients with EH. The pattern of the regulation of this lymphocyte system is associated that of baseline renin activity and of its response to exercise in patients with EH.

Lymphocyte beta 2-adrenoreceptor density and affinity for catecholamines (CA) were examined in 22 patients with
essential hypertension
(EH) which was resistant to therapy. Lymphocyte beta 2-adrenoreceptor density was 1.5 times more in EH patients than in normotensive donors, but the receptor affinity for CA was the same. Prostaglandin E2 infusions permitted the resistance to antihypertensive therapy be overcome in 17 patients; it was accompanied by a decrease in of lymphocyte beta 2-adrenoreceptor density, receptor's affinity for 1-isoproterenol showed a 7-fold increase. There was no steady antihypertensive effect on prostaglandin E2 infusions in 5 EH patients: in these cases lymphocyte beta 2-adrenoreceptor density increased and their affinity for CA decreased.

(EH). The authors proved relationship between GcI-I-genotype and predisposition to EH. Heterozygote type 1-2 of the Gc-system was more resistant to the disease. The authors conclude that genetics markers may make the basis for studying the EH etiology and determining its more etiologically similar forms.

Circadian variations of blood pressure (BP) in dependence on neurogenous baroreceptor control in patients with
essential hypertension
(EH) or renovascular hypertension (RH) were studied. Patients with EH demonstrated significantly greater decrease of systolic as well as diastolic BP at night compared to those with RH. Circadian variation rates of systolic and mean BP were also higher in patients with EH. Baroreceptor test exhibited higher cardiochronotropic effect in patients with EH. They had also negative correlation between BP variation rates and BP reaction to baroreceptor test which is absent in patients with RH. The data obtained suggest that neuroregulation mechanisms have less effect on BP level in RH than in EH.

MRT may show early aortic alterations and dependence of their degree on duration and severity of the disease. The method makes it possible to assess the state of inner organs simultaneously with the investigation of the vessels which is of great importance for the patients to whom surgical treatment is recommended.

Lymphocytes beta 2-receptor density (LBRD) was studied by binding with 125ICYP in 18 male patients with mild/moderate
essential hypertension
(EH) before and after propranolol monotherapy. In 5 of these patients LBRD was determined before and after dynamic exercise also. Propranolol therapy evoked different changes in LBRD in patients with EH. According to these results, all the patients were divided into 2 groups: the patients who respond to propranolol administration by an increase in the LBRD were attributed to the 1 group. If the LBRD decreased after propranolol, the patients were attributed to the 2 group. Propranolol had pronounced hypotensive effect in the 2 group and no hypotensive effect in the 1 group. Heart rate was significantly higher initially and decreased significantly more after treatment in the 2 group. In this group initial values of LBRD were significantly higher than in the 1 group. Exercise produced different changes in LBRD, which correlated with the changes produced by propranolol treatment. The positive correlations were found between LBRD and left ventricular myocardial mass and interventricular septum thickness. The data obtained indicate that LBRD is regulated in a qualitatively different manner in the two groups of patients with EH which appears to be related to baseline renin or perhaps catecholamine levels.

99 patients with arterial hypertension (AH) were studied: 17 patients with pheochromocytoma, 37 with aldosteronoma, 4 with corticosteroma, 38 with

. Control group included 16 healthy subjects. The investigation was performed by means of a NMR-tomograph BMT-1100 ("Brucker", FRG) in axial and frontal planes according to Spin Echo (SE) technique with TR = 2.0 s, TE = 34 ms. Qualitative (visual) as well as quantitative criteria for evaluation of adrenal glands state were developed. Normal size of adrenal glands, relative intensity of their signal and T2 were calculated. The data obtained proved that relative intensity of the signal is a highly informative parameter for differential diagnosis of adrenal gland tumors (its value of less than 1.0 is evidence of cortex lesion, and that of more than 1.4--of medulla lesion). High informative value of MR-tomography for evaluation of adrenal glands state in patients with AH is proved by comparative data obtained by other methods, such as computer tomography aortography and histomorphology.

23 patients with hypertrophic cardiomyopathy (HCMP) and 19 patients with stage II of
essential hypertension
(EH) with left ventricular asymmetric hypertrophy (ASH) were examined. Echocardiography demonstrated significantly more manifest hypertrophy of interventricular septum and higher coefficient of asymmetry in patients with HCMP compared to those with EH while left ventricular myocardium mass was equal in both groups. Phase-volumetric analysis exhibited in all the patients deterioration of myocardial relaxation processes. EKG-monitoring showed a significant increase in frequency of supraventricular rhythm disturbances in patients with HCMP while no reliable differences in total frequency of rhythm and conduction disturbances were registered. Regular adequate antihypertensive therapy resulted in patients with EH in decrease of ASH degree caused by regression of interventricular septum hypertrophy. In natural course of EH the ASH degree also decreased, but in this case because of increased hypertrophy of left ventricular posterior wall. The patients with HCMP were followed for 1-1.5 years. Drug therapy produced no significant changes in left ventricular myocardium mass as well as character of degree of ASH.

Clonidine-treated patients with
essential hypertension
demonstrated reliable positive correlations between echocardiographically determined signs of myocardial hypertrophy (thickness of left ventricular posterior wall and interventricular septum, left ventricular myocardial mass) and hormone-dependent increase in thrombocyte calcium concentration induced by ADP and TAF. Antihypertensive therapy was more effective in patients with relatively weak cellular effect of Ca-increasing hormone response having no signs of myocardial hypertrophy.
