Tissue typing applied to the Russian population was used to study distribution of HLA antigens, classes I and II, in patients with
essential hypertension
(EH), coronary heart disease (CHD), hypertrophic cardiomyopathy (HCMP), dilated cardiomyopathy (DCMP) and virus
myocarditis
(VM). As control use was made of the data on HLA antigen distribution in 267 healthy persons (donors) of the Russian nationality. The genetic markers of the predisposition to the indicated diseases were revealed: in EH, DR 1 (RR-3.56), DR 4 (RR-2.17); in CHD, B 12 (RR-2.91), DR 1 (RR-3.41), DR 4 (RR-3.14); in HCMP, DR 1 (RR-2.25), DR 4 (RR-3.29); in DCMP, DR 4 (RR-3.90); in VM, DR 3 (RR-5.26), DR 4 (RR-3.51). DR 4 turned out to be the common marker of the predisposition to cardiac diseases. Besides, DR 1 was discovered to be the marker of the predisposition to EH and DR 3 to VM. The data obtained may be of importance for the clinical practice in forming risk groups.

A 60-year-old woman was admitted with congestive heart failure,
essential hypertension
and abdominal distension. Her son reported that she appeared with red spots in the body and that she was under dapsone therapy. Seven months ago there was sudden increase of the skin lesions. In the 11th day after admission she underwent a stroke that progressed to decerebration and she expired on the fourth day. Autopsy confirmed CHF due to chronic
myocarditis
related to Chagas' disease. Aneurysm of the apical region of the left ventricular chamber was also observed leading to thrombosis and systemic embolism with brain and spleen hemorrhagic infarct. In the encephalous there was edema, uncus herniation and hemorrhagic infarct of the brain stem. The skin lesions were due to reactional tuberculoid hanseniasis (RHT) with focal lesions in axillary lymphnodes, nasopharyngeal mucosa and in the posterior tibial nerve. The pathogenesis of RHT is discussed as well as its differentiation with the BT group of Ridley and Jopling and its probably relationship with the secondary tuberculoid hanseniasis reported by Ridley. The focal lesions are also discussed with END to the involvement of a peripheral nerve trunk what is said to be uncommon in this form of Hansen's disease.

Hypertension is the commonest cardiovascular disease in Africans occurring in more than 15% of the adult population in some studies. It occurs in the lower as much as in the higher socio-economic groups. Recent studies have confirmed earlier findings that
essential hypertension
in Africans is characterised by volume loading, low plasma renin activity, high salt taste threshold, high urinary sodium and low potassium excretion and high plasma aldosterone. The commonest complication of hypertension in Africans is congestive cardiac failure followed by cerebrovascular accidents. Coronary heart disease is rare. Even in the absence of overt heart failure and compounding factors like obesity, alcoholism, cigarette smoking, diabetes mellitus and
myocarditis
, evidence of abnormal left ventricular morphology and function is often present in newly diagnosed patients with moderate or severe hypertension. Response to monotherapy with beta-blockers or ACE inhibitors is usually poor but is good with thiazide diuretics or calcium channel blockers. The diuretics are an essential component of a two or three drug regime containing other classes of antihypertensive drugs. Cost of drugs is the most important determinant of compliance with drug treatment and consequently the likelihood of progression of the diseases to more severe forms in long term follow-up.

By April 28th 2020, the global number of people that were viciously infected with the newfound novel corona virus (COVID-19) stood at a staggering 3 077 133 cases, as per the confirmed data released by the WHO. It has been reported that women from the Chinese Han population are associated with
essential hypertension
due to their relation with the 5 SNPs, namely, rs1514283, rs4646155, rs4646176, rs2285666, and rs879922, which belong to the ACE2 gene. The level of ACE2 activity was very low in normal healthy younger persons, and was reported to be increased in patients with cardiovascular diseases. Thus, there might be severe
myocarditis
, that may result in acute heart failure and cardiac complexities in the elderly subjects.
