The author correlated in a group of 1100 autopsies made in 1987-1989 the clinical and autoptic diagnosis of active tuberculosis of the lungs and organs, bacterial meningitis, acute and subacute endocarditis, septicaemia, acute cholangitis, diffuse suppurative peritonitis, renal infections, and pneumonias. Class I diagnostic errors, i.e. those where knowledge of the diagnosis before autopsy would have probably changed therapy and the prognosis, were encountered in all groups of correlated diseases, however, with a high frequency in tuberculosis of the lungs and organs, septicaemia, renal infections and pneumonias. It seems that the diagnostic vigilance to some common serious diseases caused by infections declines in the practice of general hospitals. This experience was recorded also in hospitals abroad. Causes of diagnostic errors are discussed.

We herein report a case of severe acute cholangitis complicated by bacterial meningitis. A 56-year-old Japanese man was admitted to our hospital due to a consciousness disorder that had developed while he was being treated for acute cholangitis. The levels of both hepatobiliary enzymes and inflammatory markers were high, and computed tomography revealed common bile duct stones and cholangiectasis. A diagnosis of acute cholangitis was made, and meningitis was subsequently confirmed on a cerebrospinal fluid analysis. The patient recovered successfully after receiving emergency endoscopic drainage and antibiotic therapy. This case demonstrates that a disturbance of consciousness complicated by acute cholangitis may result from septic encephalopathy as well as meningitis via bloodstream infections.

Listeria monocytogenes is a well-known cause of meningitis in immunocompromised patients. This organism has a growing significance for community-acquired meningitis, which should have ampicillin added to the usual regimen. We describe a case of L. monocytogenes meningitis preceded by cholangitis. This case suggests gastrointestinal symptoms preceding meningitis may be a clue of listeriosis. It is important for physicians to consider L. monocytogenes as a cause of bacterial meningitis in patients with altered mental status preceded by gastrointestinal symptoms, especially in the immunocompromised population.
