On a new dry filled doxycycline derivative for intravenous administration, some clinical studies were performed. The results obtained were summarized as follows; 1. Intravenous administration was effective in three cases of pulmonary abscess, in one case of bronchopneumonia, in one case of urinary tract infection complicated to nephrolithiasis, in one case of Mycoplasma pneumonia and in one case of tsutsugamushi disease. 2. In one case of FUO (fever of unknown origin), in one case of non-bacterial meningitis and in one case of ampicillin induced hemorrhagic colitis, from whom Klebsiella oxytoca was isolated in feces, satisfactory clinical course was observed with the drug administration. However, as it was impossible to give any comments for the causative agent of each disease, the effectiveness of the derivative was not necessarily clear. 3. Neither side effects nor abnormal laboratory finding caused by the derivative were noticed in any cases.

Twenty-four of 66 (363.6 +/- 116/1000) post-neonatal children with acute bacterial meningitis treated between August 1991 and November 1992 at the Emergency Paediatric Unit, University of Maiduguri Teaching Hospital, Nigeria, had co-existing localized extracranial infections (LEI). Nineteen had bronchopneumonia (of which 14 were in combination with other infections), two had dysentery and one each acute otitis media, orbital cellulitis, and purulent conjunctivitis. LEI were significantly more frequent in children < or = 2 years, and in children with delayed presentation and delayed diagnosis after presentation. Mortality rate was higher in children with LEI (500 +/- 200/1000) than in those without (102.6 +/- 95.2/1000) (P < 0.001). There was no significant relationship between the incidence of LEI and the causative organisms of meningitis or nutritional status of children with meningitis. We conclude that co-existing LEI are not infrequent in children with acute bacterial meningitis; they contribute to delayed diagnosis and are associated with a poorer outcome.

A 60-year-old man presented with progressive and unique neurological symptoms. Investigations identified an isolated cerebellar lesion. This lesion fulfilled the histological criteria for lymphomatoid granulomatosis, and in situ hybridization and deoxyribonucleic acid (DNA) dot blot techniques revealed significant amounts of Epstein-Barr virus DNA within the tumor cells. The patient underwent cranial radiation therapy, and 16 months after the initial presentation the lesion evolved into a malignant lymphoma. He subsequently died secondary to subdural empyema, bacterial meningitis, and bronchopneumonia. The unique clinical and etiological aspects of this case are addressed.
