To assess the potential of FDG for PET imaging of nodal tumor metastases, we evaluated its uptake into normal lymph nodes, tumor-involved lymph nodes, and subcutaneous tumor xenografts in rodents. Normal lymph nodes in mice and rats accumulate FDG moderately, developing node/blood ratios of 1.3-11.9/1 at 2 hr following i.v. injection. By contrast, FDG given subcutaneously to healthy Sprague Dawley rats developed very high normal draining lymph node/blood ratios (272/1) versus 7.7/1 by i.v. injection. In nude mice, subcutaneous human ovarian cancer xenografts had 1.27-fold more uptake relative to blood than did normal popliteal lymph nodes. Subcutaneous tumor xenografts of rat breast cancer developed tumor/normal node uptake ratios of 4.91 +/- 0.43/1 and tumor/blood ratios of 6.6 +/- 0.9 at 2 hr postinjection. Mouse nodes involved with 38C13 murine B-cell lymphoma had mean node/blood ratios of 42.9 +/- 6.7/1 and tumored node/normal lymph node uptake of 6.3/1. Thus, FDG given intravenously but not subcutaneusly (due to high normal nodal uptake) has potential as an agent for the detection of metastatic tumors in regional lymph nodes using PET scanning.

Radionuclides have been used for the diagnosis and therapy of cancers. In Japan, about 1.8 million studies are performed annually, especially on bone, the heart, the brain and cancer. In contrast to anatomical studies with X-ray, US or CT, nuclear medicine provides physiological or metabolic images. The characteristics of nuclear medicine come from the use of tracer studies employing various radiopharmaceuticals. The most commonly used radionuclides for cancer studies are 67Ga and 201T1. Recently, however, many other radiopharmaceuticals with tumor specificity have been developed, such as 99mTc labeled monoclonal antibodies and 111In labeled octreotide. 18F-FDG, which images glucose metabolism, is very useful in the management of lung, colorectal and other cancers. Furthermore, radionuclides are also employed in the therapy of cancer, such 131I-labeled anti-CD20 antibody for the B-cell lymphoma and 89Sr for the palliation of bone pain caused by prostate and breast cancer metastases.

Bone marrow involvement can be found in patients with low-, intermediate-, or high-grade non-Hodgkin's lymphoma. A 40-year-old woman experienced onset of low back pain radiating into her entire right lower extremity. Plain x-rays of her right leg and computed tomography of chest, abdomen, and pelvis were unremarkable. Magnetic resonance imaging of pelvis and thighs revealed diffusely abnormal marrow signal (low T1 and high T2 weighted) in the pelvis and femora. The patient underwent (18)F-fluorodeoxyglucose positron emission tomography (FDG-PET) scan to evaluate the extent of her disease. The scan revealed diffuse scattered foci of abnormal FDG uptake in the bone/bone marrow, which was particularly intense in the axial bones. Bone marrow biopsy confirmed extensive involvement of the bone marrow with diffuse large B-cell lymphoma. This case report highlights the utility of FDG-PET in the detection of bone marrow involvement by aggressive lymphoma.

We retrospectively evaluated (18)fluoro-2-deoxyglucose positron emission tomography (FDG-PET) scans in 172 patients with lymphoma and correlated results with pathologic diagnosis using the World Health Organization (WHO) classification system. In total, FDG-PET detected disease in at least one site in 161 patients (94%) and failed to detect disease in 11 patients (6%). The most frequent lymphoma diagnoses were diffuse large B-cell lymphoma (LBCL; n = 51), Hodgkin lymphoma (HL; n = 47), follicular lymphoma (FL; n = 42), marginal zone lymphoma (MZL; n = 12), mantle cell lymphoma (MCL; n = 7), and peripheral T-cell lymphoma (PTCL; n = 5). FDG-PET detected disease in 100% of patients with LBCL and MCL and in 98% of patients with HL and FL. In contrast, FDG-PET detected disease in only 67% of MZL and 40% of PTCL. Comparison with bone marrow biopsies showed that FDG-PET was not reliable for detection of bone marrow involvement in any lymphoma subtype.

Here we describe the evaluation of 3'-[(18)F]fluoro-3'-deoxythymidine [[(18)F]-FLT] as a tracer for positron emission tomography (PET) in a murine model of B-cell lymphoma and in human malignant lymphoma. The human B-cell line DoHH2 expressed high levels of active thymidine kinase 1 (TK-1) as the key enzyme of [(18)F]-FLT metabolism. Immunostaining confirmed high levels of TK-1 in DoHH2 derived xenograft tumors in SCID/SCID mice. In vitro studies demonstrated a time-dependent uptake of [(18)F]-FLT, an efficient phosphorylation to the respective monophosphate and the incorporation of [(18)F]-FLT into the perchloric acid insoluble fraction in DoHH2 cells, indicating the incorporation of this tracer into the DNA. After incubation with [(18)F]FLT for 240 min, 12.5% +/- 1.0% of radioactivity applied to the medium was intracellularly trapped in DoHH2 cells. Specific accumulation of [(18)F]-FLT in the malignant cell clone was confirmed in biodistribution studies in SCID/SCID mice bearing DoHH2-derived tumors. The percentage of injected dose of [(18)F]-FLT per gram of tumor tissue correlated with the tumor-proliferation index as evaluated in BrdUrd-labeling experiments. In a pilot study of 11 patients with both indolent and aggressive lymphoma, [(18)F]-FLT was suitable and comparable to [(18)F]-FDG in the ability to detect malignant lesions by PET scan. Furthermore, we found a close correlation (r = 0.95, P < 0.005) of the [(18)F]-FLT standardized uptake values with the Ki67-labeling index of tissue biopsies (n = 10) in these patients. These results suggest that [(18)F]-FLT represents a novel tracer for PET that enables imaging of proliferation in human lymphoma in vivo.

Primary non-Hodgkin lymphoma of the thyroid is an uncommon disease. It is a potentially aggressive disease and diffuse large B-cell lymphoma of the thyroid may result in 5 year survival rates <50%. Hence adequate follow-up and multimodality treatment for recurrent or persistent disease are required. Since 18F-FDG PET is considered the imaging method of choice for the detection and staging of lymphoma, this was used for restaging of a case of diffuse large B-cell lymphoma of the thyroid after chemotherapy and its diagnostic value is questioned in the present case report. In fact, PET showed a false-positive finding which led to unnecessary surgery.

We have reported three cases of central nervous system malignant lymphoma in which FDG-PET was useful in monitoring therapeutic effects. Case 1: A 53-year-old man complained of gait and memory disturbance. An MRI of the patient's brain showed enhanced mass lesions in the bifrontal lobe. An FDG-PET showed markedly high uptake of the tracer, which means a higher metabolism of glucose. The tumor was biopsied and the histological diagnosis was diffuse B cell lymphoma. The patient received chemotherapy and external irradiation therapy. Case 2: A 64-year-old woman suffered memory disturbance and left hemiparesis. An MRI showed a right frontal mass lesion, and FDG-PET showed high uptake of glucose. After the histological diagnosis was determined as diffuse large B cell lymphoma, the patient received the same therapy as case 1. Case 3: A 55-year-old woman suffered right hemianopsia. An MRI showed an enhanced lesion in the right basal ganglia and an FDG-PET showed high uptake of glucose. After the histological diagnosis was determined as diffuse large B cell lymphoma, the patient received the same therapy as case 1 and 2. In all cases, high uptake of glucose disappeared on the PET after initial chemotherapy, although an enhanced lesion continued on MRI even after radiation. FDG-PET was useful in monitoring the therapeutic effects of malignant lymphoma. These results indicate that we were able to confirm the effectiveness of the therapy in the early stage.

Spontaneous regression of non-Hodgkin lymphoma (NHL) has been reported in low-grade tumors but is an extremely rare event in intermediate- and high-grade disease. Documentation of spontaneous regression by serial fluorodeoxyglucose-positron emission tomography (FDG-PET) imaging has not been reported in the literature. We present 3 cases of spontaneous regression, 1 each of follicular lymphoma (FL), mantle cell lymphoma (MCL), and diffuse large B-cell lymphoma (DLBCL), which showed spontaneous regression on serial FDG-PET imaging. All patients underwent serial whole-body FDG-PET scans 60 minutes after intravenous injection of 9-11 mCi of this radiotracer. None of them had any chemotherapy, radiotherapy, or surgery after the baseline PET scan. Spontaneous regression of disease in all 3 cases was correlated with conventional imaging and clinical course. All 3 patients had positive FDG-PET results on their baseline scan. There was complete disappearance of FDG uptake on a follow-up PET scan for the patient with follicular lymphoma. These results suggest complete regression. The patients with MCL and DLBCL both showed a significant reduction in FDG uptake on serial whole-body PET scans, suggesting partial regression in both cases. Although spontaneous regression of lymphoma is uncommon, this phenomenon can be successfully demonstrated by FDG-PET imaging. Therefore, serial PET imaging may play an important role in detecting this unusual event and may further enhance our understanding of the biologic behavior of this malignancy.

Gastric lymphoma and gastrointestinal stromal tumours (GISTs) are rare malignancies of the upper gastrointestinal tract. The most common gastric lymphoma are low-grade marginal zone B-cell lymphoma (MZBCL) of MALT type. They develop as a consequence of chronic Helicobacter pylori infection, the histological hallmark are lymphoepithelial lesions. In early stages of disease, H. pylori eradication alone may lead to complete lymphoma remission in up to 75% of cases. Nonresponder or locally advanced lymphoma should be treated with radiation therapy. Advanced lymphoma may be treated with the nucleoside analogon cladribine within clinical trials. Based on clinical and novel molecular markers a risk stratification and a prediction of response to therapy might be possible in the future. GISTs are mesenchymal tumours that characteristically express CD-117 (c-kit). They are mostly localized in the upper gastrointestinal tract and are frequently diagnosed in an advanced stage. Conventional chemotherapy is ineffective. For resectable non-metastasized tumours surgical therapy is the treatment of choice. Imatinib is the first and so far only effective systemic therapy which is presently indicated in irresectable or metastasized GISTs. More than 80% of patients respond to imatinib therapy either with partial remission or stable disease. FDG-PET plays an important role in the early prediction of response to imatinib therapy. The optimal dosage and duration of treatment and the role of imatinib as adjuvant or neo-adjuvant therapy for GISTs remains to be defined.

An 80-year-old man had symptoms of urinary retention. Biopsy of the prostate revealed B cell lymphoma. To evaluate the extent of the disease, an F-18 FDG study was performed with positron emission tomography/computed tomography (CT). One hour after intravenous administration of 18.5 mCi F-18 FDG, imaging was carried out from the skull base to the upper thigh. An area of moderately increased activity was noted within the prostate bed. A noncontrast CT, for attenuation correction and anatomic localization, revealed maintenance of fat planes around the prostate, indicating disease localized to that organ. Hence, a B cell lymphoma had been localized to the prostate.
