The combined diphtheria-pertussis-tetanus and combined diphtheria-pertussis vaccines, of which the contents of endotoxin, LPF and HSF were reported in the previous paper, have been tested for the respective potencies of pertussis, diphtheria and tetanus components. In order to obtain some information concerning possible adjuvanticities of the toxic components of pertussis cells, relationship between respective toxic activities and the potencies of the respective antigenic components were tested by applying the multiple regression analyses. The results showed that the effects of the toxic components were fairly complicated. Within the ranges of contents of the respective toxic components of the vaccines used in the present experiment, LPF appeared to enhance all the three potencies as the content increased, while the effects of endotoxin and HSF were either enhancing or suppressive depending on the antigenic components.

Quadruple vaccines (DTP-P), prepared by mixing crude adsorbed Salk poliomyelitis vaccines, heat-killed pertussis vaccines and adsorbed purified diphtheria and tetanus toxoids were found to be toxic for mice. However, by using purified poliomyelitis and heat-killed formalinized pertussis components, vaccines were prepared that were non-toxic. The pertussis component of these vaccines retained potency over extended periods of storage. Stable, potent DTP-P vaccines offer an excellent alternative to DTP and live poliomyelitis vaccine in the basic immunization of infants and children.

Serum antibodies (AB) have been measured before and after vaccination in haemodialysed children. The main finding was a normal increase in AB titres following adsorbed vaccines (diphtheria, tetanus, pertussis and poliomyelitis) demonstrating a normal humoral immune ability. By contrast, a poor response to live attenuated viruses (poliomyelitis and measles) was observed. Two additional findings were an inhibitor effect of tetanus toxoid in mice by uraemic serum, and a constant negative Schick test indicative of suppressed non-specific skin reactivity.

Fifteen guinea pigs were immunized with diphtheria-tetanus-pertussis [DTP] vaccine once a week throughout the period of 10 months. The control group included 5 guinea pigs. ECG tracings were recorded every second week in both groups of animals. ST-T changes were the most important findings. Sometimes the ECG resembled the picture found in endocardial fibroelastosis in children. ECG abnormalities appeared 3 months after the beginning of the experiment, and were frequently variable. Less pronounced ST-T changes were found also in control animals, but they disappeared after the administration of Inderal. The histological pictures revealed a thickening of the endocardium and degenerative changes of the ganglion cells of the atria; these changes did not always correlate with electrocardiographic tracing.

Investigation of measles outbreaks during the fall of 1976 led to the discovery that Alkaska's school immunization law was not being enforced. In an effort to control a large outbreak of measles in Fairbanks, children were required to show proof of measles vaccination or be excluded from school. Of the Fairbanks schoolchildren, 25% were vaccinated against measles; 1,251 (11%) of 11,727 unvaccinated schoolchildren were excluded in January, and no further cases of measles occurred. Subsequently, the school law was enforced statewide, and on March 1, 1977, all children not immunized against diphtheria, tetanus, pertussis, polio, measles, and rubella were excluded from school (7,418 [8.3%] of 89,108). One month later, fewer than 51 children still did not meet immunization requirements. More than 35,000 children were vaccinated in the immunization campaign; no adverse side effects to any vaccine were reported.

Three infants developed severe hemolytic anemia following the second or third diphtheria-pertussistetanus vaccination. Direct antiglobulin tests were positive, and the infant most severely affected also had reduced serum complement levels, indicating an immunological mechanism for the hemolysis. The presence of IgM on the erythrocytes from 2 of the infants could be demonstrated by antiglobulin tests or immunization experiments. Heat eluates of the erythrocytes from one of the infants contained antibodies to tetanus and diphtheria toxoids, as well as to Bordetella pertussis, suggesting that these antibodies were antigenically bound to the erythrocytes. Virus antibodies or isoagglutinins, present in the serum, were not found in the eluate. No antibodies against the vaccine components could be demonstrated in eluates of erythrocytes from control subjects. In vivo experiments showed that tetanus and diphtheria toxoids were easily bound to human erythrocytes. This finding could help explain the pathogenesis of the autohemolysis.

Live attenuated measles vaccine was administered to Cameroonian children 12 to 39 months of age alone or with either diphtheria-tetanus toxoids or diphtheria and tetanus toxoids and pertussis (DTP) vaccine. Among children who were initially seronegative for measles hemagglutination inhibition antibodies, seroconversion rates and postvaccination geometric mean titers were similar in all groups. Pertussis antigen in the DTP vaccine was judged to be potent by laboratory potency testing and serologic response in recipients of the vaccine. Thus, the two vaccines may be administered simultaneously without compromising their immunogenicity. These results allow greater flexibility in planning individual or mass immunization schedules.

Kinetic studies on the adjuvanticities of several substances with different modes of action were performed in guinea pigs by using tetanus and diphtheria toxoids as the antigens. When injected subcutaneously into animals, aluminium, endotoxin, pertussis vaccine and water in oil in water (w/o/w) showed very similar adjuvanticities to tetanus toxoid at the beginning stage of immunization, but the activities except that of aluminium became less significant at later stages after the primary stimulus when antitoxin was produced abundantly. Poly L-lysine and to a less extent poly A:U showed potent adjuvanticities next to that of aluminium throughout the whole immunization period. Combination of poly L-lysine with 0.03 mg aluminium showed a similar adjuvanticity to that of 0.9 mg of aluminium alone. In contrast to rather low adjuvanticities to tetanus toxoid, more distinct adjuvanticities were observed to diphtheria toxoid throughout the whole period of immunization with various substances such as aluminium, poly A:U, w/o/w and poly L-lysine but not with endotoxin.

The vaccination status was investigated in 1482 patients between the ages of 1 and 14 years admitted to hospital with scarlet fever. Most of the patients were vaccinated against tuberculosis (97.7%), diphtheria, tetanus and whooping-cough (95.3%) and poliomyelitis (94.1%), relatively few against measles (21.1%) and very few indeed against mumps (0.7%) and tick-borne encephalitis (1.9%). The booster vaccination against tetanus and diphtheria had been omitted in more than 40%. Although the beneficial results of vaccination against tuberculosis, diphtheria-pertussis-tetanus and poliomyelitis remained more or less the same, the tendency towards vaccination did not spread as might have been anticipated. On the contrary, the extent of vaccination decreased, especially during the past years. In the same way the tendency towards vaccination against measles showed a sudden slowing down after a period of rapid increase. This implies that vaccination of children does not tend towards perfection. The vaccination rates differ widely between foreign children living in Vienna and natives. Although the foreigners show a similar vaccination distribution pattern as the natives, the numbers of unvaccinated children are much higher.

The amounts of endotoxin, histamine-sensitizing factor and lymphocytosis-promoting factor contained in combined diphtheria-pertussis-tetanus and combined diphteria-pertussis vaccines recently produced in Japan were determined and the relationships among them were statistically analyzed. The estimated values in histamine-sensitizing units (HSU) and lymphocytosispromoting units (LPU) per human dose ranged from 0.3 to 6 and from 0.4 to 5, respectively. A fairly wide range of endotoxin contents, which were expressed in body weight-decreasing units (BWDU), was also demonstrated. All the ranges were larger than those expected from experimental error. There was a positive correlation between the HSU and LPU values although the individual HSU-LPU ratios ranged from 0.3 to 3, but the range was markedly larger than the width of the 95% confidence interval of the mean ratio. Negative correlation between BWDU and LPU was also significant, probably because of the relatively large number degrees of freedom, though the coefficient was fairly small; while that between BWDU and HSU was not significant. The frequency distribution of individual values of HSU was divided into two lognormal distributions. With one of the two groups obtained by this subdivision, a distinct positive correlation bewteen HSU and LPU was demonstrated, but with the other group there was little correlation. Some differences between the two groups were also found in the correlations between BWDU and the other two variates. No factors responsible for the separation of the groups have yet been found.
