Sixty-three patients with unresectable hepatocellular carcinoma (HCC) were treated with cisplatin-phosphatidyl-choline-Lipiodol (CPL) suspension. Partial response (PR) and minor response (MR) were obtained in 3 of 14 cases (21.4%) by one shot therapy, and in 13 of 43 cases (30.2%) by TAE therapy.
AFP
decreased in 11 of 15 patients (73.3%) by one shot therapy, and in 32 of 33 patients (97%) by TAE therapy. PIVKA II also decreased. The one-year survival rate was 74% in TAE therapy, and 52% in one shot therapy. The two-year survival rate was 53% in TAE therapy, and 28% in one shot therapy. Nausea,
vomiting
and fever were noted in most cases as adverse effects, but they were slight. The concentration of free-CDDP in the peripheral venous blood was lower and continued longer than that of CDDP on the market. These results suggest that CPL was useful as an anticancer agent for arterial chemotherapy or TAE therapy for unresectable HCC.

Thirty-six patients with advanced non-seminomatous germ-cell testicular tumours and two patients with advanced seminomas were treated with cisplatin-containing combination chemotherapy. Thirty-four patients received cisplatin 100 mg/m2 iv, vinblastine 0.3 mg/kg iv and bleomycin 30 mg iv (PVB) and three patients received this combination with etoposide (VP16-213) 120 mg/m2 iv on 3 consecutive days substituted for vinblastine (BEP). One patient received cisplatin and bleomycin only. All 35 evaluable patients with non-seminomatous tumours responded; 22 patients (61%) achieved a complete response (CR); 16 of these (73%) are alive with no evidence of disease at follow-up ranging from 18 to 55 months (median 36). Of 13 patients achieving a partial response (PR), 11 have died of progressive disease at 7 to 30 months (median 11) and two are alive with disease which has continued to regress following chemotherapy. Of 32 patients who received adequate chemotherapy, 16 (50%) are alive and disease-free and three (9%) are alive with evidence of disease. The chances of achieving a CR were reduced in those patients with bulky disease or high levels of

or beta hCG at presentation but not in those who had received prior radiotherapy. Toxicity was considerable, including alopecia and nausea or
vomiting
in all patients, and haematological toxicity, neurotoxicity, hearing loss and dyspnoea in a substantial number of patients.

Twenty patients with primary hepatic carcinoma (PHC) treated by hepatic arterial embolization in our department from Dec. 1986 to Mar. 1987 are reported. There were 15 males and 5 females. The ages ranged from 34 to 75 years with an average of 50.7. Preoperative diagnosis and localization of the tumor were done by

, B-us, CT and angiography (right lobe 15 cases, left lobe 1 case, both lobes 4 cases). Celiac and superior mesenteric angiography was carried out by femoral artery approach and then highly selective hepatic catheterization was utilized for hepatic arterial embolization. Antitumor agent (5-Fu, adriamycin), iophendylate and foamy gel sponge were used for peripheral and proximal embolization. Manifestations were improved in most of the patients after embolization, such as relief of abdominal pain, improvement of appetite, decrease of tumor size. Total necrosis of the tumor was found in 2 patients who underwent surgery 1 month after embolization. The side effects of the posthepatic embolization such as, nausea,
vomiting
, abdominal pain and fever could be relieved by symptomatic treatment. No severe complications, such as gangrene of the gall bladder, hepatic failure, liver abscess, intestinal necrosis or pulmonary embolization were found except 3 patients who died of renal failure after the procedure. The liver dys-function returned to normal within 2 weeks. Hepatic arterial embolization provides an alternative treatment for the patients with PHC who has compensated liver function without severe systemic diseases, especially renal endocrine problems and severe portal hypertension. They should have patent portal system as proved by angiography. The authors considered that this therapeutic embolization with hepatic chemotherapy infusion is safe and effective in the management of PHC. It may increase the resectability and provide palliative means for the advanced and terminal cases.

Twenty-two patients with malignant chest tumor, mainly primary lung cancer, were given 73 courses of combined administration of cisplatin and bleomycin. The following results were obtained. Of 13 evaluable patients one CR and four PR, with an overall response rate of 38.5% were observed. Of seven patients who received 3 courses or more, four showed therapeutic effect, producing a response rate of 57.1%. CR was obtained in a patient with a relapse of malignant germ cell tumor showing positive HCG and

preoperatively. The patient survived for 445 days after the start of this treatment. Of nine patients with non-small cell lung cancer in whom therapeutic effect could be evaluated, three PR, four MR, and two PD with a response rate of 33.3% were obtained. Side effects due to cisplatin such as nausea,

and impairment of renal function were all transient. Furthermore, myelosuppression caused by this chemotherapy was relatively mild compared with other chemotherapy regimens.

A 5-year-old girl was admitted to another clinic because of

and convulsions. She was brought to our clinic after a ventriculoperitoneal shunt was inserted. CT scan on admission in our clinic showed a tumor in the pineal region with tumoral hemorrhage. Tumor markers such as HCG,

, CEA, P-LAP were within normal range. A biopsy of the tumor was performed and the histological diagnosis was pineoblastoma. Her recovery was excellent and disseminated metastasis was not recognized. A subtotal removal of the tumor was performed through the occipital transtentorial approach. She had no neurological deficits after surgery. She then received two 5-day cycles of chemotherapy, consisting of intravenous administration of 20 mg/m2/day cisplatin and 60 mg/m2/day etoposide, and craniospinal radiotherapy. After these therapies, the tumor responded and disappeared completely. Follow-up radiographic investigations also demonstrated no abnormal evidence except for brain atrophy. She is attending a primary school without any problems. Pineoblastoma is quite rare and remarkably malignant. Hence, aggressive therapies including surgery, radiotherapy and chemotherapy is indicated for this tumor.

, and left facial nerve palsy since 1 month before. An MRI revealed an enlarged mass, 4cm in diameter, in the left cerebello-pontine angle with uniformal enhancement by Gd-DTPA, and bilateral ventricular dilatation. He was found to have increased serum alpha-fetoprotein level (

After a partial resection of the tumor, diagnosed as pure yolk sac tumor, and ventriculo-peritoneal shunt, three courses of combined chemotherapy with cisplatin, bleomycin, vinblastine and cyclophosphamide (modified VAB-6 therapy) were carried out. level returned to normal, and the tumor mass entirely disappeared (a complete response) on MRI after the second course of chemotherapy. However, cisplatin-induced

Irrespective of administration of recombinant human G-CSF and broad spectrum antibiotics, he suffered from pneumonia and died of septic shock and multiple organ failure. Autopsy showed microscopic residual tumors. The combination chemotherapy with cisplatin, bleomycin, vinblastine and cyclophosphamide is effective for initial treatment of childhood intracranial yolk sac tumor. It is necessary, however, to reevaluate the cisplatin dosage and treatment schedule in order to reduce such side effects as bone marrow suppression and renal damage.

Neonatal brain tumor is rare, but is so important a disease when considering the pathogenesis of the brain tumor that numerous review articles have been described. Today, the availability of noninvasive imaging procedures such as CT and MRI make it easy to diagnose, and some successful operative cases have been reported. We report three cases of neonatal brain tumors presented clinically within the first 2 weeks. Case 1: A full term boy admitted with projectile

, enlarged head and left peripheral type facial palsy at the age of 12 days. CT scan revealed a large heterogeneous mixed-density mass in the left cerebellar hemisphere. Partially removed surgical specimen consisted of primitive glial cells differentiating with the ependymal cell immunohistochemically and electron microscopically, and diagnosed as ependymoblastoma. He had whole brain irradiation postoperatively, but died from respiratory distress 7 months later without tumor regrowth indicated on CT. Case 2: A full term boy admitted with progressive enlarging of the head at the age of 10 days. CT scan revealed a high-density mass in the cerebellar vermis and an obstructive hydrocephalus. Partially removed surgical specimen, diagnosed as medulloblastoma. He was irradiated throughout the whole brain and spinal cord, but died from intracranial dissemination 5 months later. A 6-month-old girl was admitted to another hospital because of consciousness disturbance, preceded by 2 weeks of decreased activity and

. She was referred to our hospital after ventricular drainage had been instituted for hydrocephalus and the tumor in the pineal region. The patient was noted to have conjugate upward gaze palsy and papilledema. CT scan and MRI revealed a large tumor in the pineal region with tumoral hemorrhage and a small mass in the right frontal lobe. At surgery, the pineal region tumor was removed subtotally. Histological examination showed the tumor to be composed of sheets of large polyhedra or round cells with an eccentric round nuclei, prominent nucleoli, and cytoplasmic inclusions. Immunohistochemical studies were positive for GFAP, vimentin, S-100, CK, EMA, and SMA, but negative for
AFP
, HCG, PLAP, and CEA. Following surgery, she received three 5-day cycles of chemotherapy, consisting of intravenous administration of cisplatin 20 mg/m2/day and etoposide 60mg/m2/day. After these therapies, MRI showed a decrease in the area of high intensity in the pineal region, but almost no change in the right frontal mass lesion. Follow-up radiological examination showed that the tumor had grown rapidly one month after chemotherapy and the patient died 5 months after her first hospitalization. Malignant rhabdoid tumor of the CNS is rare and remarkably malignant. This tumor should be treated using multidisciplinary management with surgery, intensive chemotherapy, and radiotherapy depending on the patient's age.
