A 45-year-old white man ingested approximately two cups of boric acid crystals dissolved in
water
in a suicide attempt. Nausea,
vomiting
, greenish diarrhea, and dehydration occurred shortly thereafter. Two days later, he presented to the hospital with hypotension, metabolic acidosis, oliguric renal failure, a generalized erythematous rash, and several superficial skin abrasions. His condition failed to improve despite intravenous fluids and vasopressors. He later developed atrial fibrillation with a rapid ventricular response and could not be converted to a sinus rhythm. This rhythm deteriorated to electromechanical dissociation, and the patient died 17 hours after admission. The urine and whole blood boric acid concentrations approximately 52 hours after ingestion were 160 and 42 mg/dL, respectively. These results are equivalent to urine and blood boron concentrations of 28 and 7 mg/dL, respectively. A postmortem urine boron concentration was 29.4 mg/dL. The autopsy report listed boron toxicity as the cause of death. This is the only adult reported to die from acute boric acid ingestion in recent years and may be atypical since the patient was untreated for 3 days and presented with dehydration and renal function impairment. This case suggests that lack of adequate urine flow and dehydration increases the risk of boron toxicity.

-associated food poisoning by Salmonella Enteritidis occurred in Takatoh Town. Nagano prefecture during September 4 to 19, 1989. The major symptoms observed in the 680 patients consisted of diarrhea (70.9%), abdominal pain (51.2%), fever (44.6%), headache (27.9%), nausea (5.9%) and
vomiting
(5.7%). In the outbreak, S. Enteritidis was the only suspected etiological agent isolated from both patients and municipal
water
supply. All of the 21 isolates were lysine decarboxylase activity negative. In other respects, the isolates conformed to the general characteristics of Salmonella. All of them were further characterized by phagovar 8 type, uniform drug susceptibilities and carrying 2.7 kb plasmid. Inspite of the evidence that the infection caused by a peculiar strain linked to the strain in municipal
water
supply used by the patients, it was impossible to determine the exact source of contamination in the
water
. The outbreak above seems to be the most large-scale among those of Salmonella found poisoning associated with drinking

Forty-eight dogs were separated into four groups of six males and six females. Acrolein (0.1% aqueous) was administered in gelatin capsules to three of these groups at dosing levels of 0.1, 0.5 and 1.5 mg kg-1 day-1 based on results of a range-finding study. After 4 weeks, the high dose was increased to 2 mg kg-1 day-1. The fourth group received deionized
water
in the same number of gelatin capsules as the high-dose group. Dosing was 7 days per week for 53 weeks. Blood and biochemical measurements were made pretest and at 3-month intervals thereafter. At termination, all dogs were subjected to full necropsy and histological examination. The major test effect noted was frequent
vomiting
after dosing. This was observed to be dose-dependent and the frequency decreased with time, indicating an adaptive effect. One mid-dose female died during the test and was diagnosed as having died of severe bronchial pneumonia, probably a result of vomitus aspiration. Serum albumin, calcium and total protein values were depressed in high-dose animals throughout the study. Some variability in red blood cell parameters and coagulation times were noted but the significance of these effects was not obvious.

Treatment requires immediate replacement of the massive fluid loss before diagnostic studies are ordered. Clinicians should suspect cholera in any case of massive, shock-producing diarrhea, especially if the patient has traveled to a cholera-affected country. Ketamine was evaluated when used alone, and in conjunction with two benzodiazepines, diazepam or a new

-soluble midazolam. An anti-muscarinic, atropine or glycopyrrolate, and nitrous oxide-oxygen were included in all sedations. Mean treatment times were increased significantly when ketamine and a benzodiazepine were used in combination. Additionally, mean ketamine dosages were decreased significantly when ketamine was utilized in combination with a benzodiazepine. To obtain accurate, timely data on the health status of persons in evacuation camps after the eruption of the Mt. Pinatubo volcano, the Philippine Department of Health (DOH) conducted a survey of the health needs of the evacuees and established disease surveillance in each camp. Surveys of the camps revealed that sources of potable

and diarrhea in the camps. Deaths, primarily caused by measles (31%), diarrhea (29%), and respiratory infections (22%), totaled 349 in the first 12 weeks. Death rates peaked in the seventh week, when a death rate of 26/10,000 occurred among the Aetas, a tribe evacuated from the slopes of the volcano. The surveys guided the DOH in allocating supplies and medicine, while disease surveillance enabled disaster managers to monitor the effectiveness of health programs, identify high-risk groups, and respond appropriately to the situation.

A 32-year-old man was diagnosed as having pseudo-Bartter syndrome due to surreptitious habitual
vomiting
and to maldigestion related to decayed teeth. His chief complaints were muscle pain and weakness. In this case, metabolic alkalosis, hypokalemia, hypochloremia, increased plasma renin activity and aldosterone levels were noticed with marked decreases in urinary chloride excretion. Creatinine clearance (GFR) and renal plasma flow (RPF) were also decreased. Blood pressure was normal, but the pressor response to angiotensin II was attenuated. Before treatment with 0.9% saline infusion, plasma vasopressin (AVP) was not suppressed sufficiently by lowering the plasma osmolality (Posm) with an oral

load (WL), but it normally responded to a rise in Posm due to hypertonic saline infusion. Moreover, plasma AVP was normally suppressed by WL after the replenishment of saline. Plasma atrial natriuretic peptide (ANP) was low before WL, but increased normally in response to WL. However, inconsistent with the normal response in this case, decreases in plasma AVP failed to dilute urinary osmolality and to increase urine flow, irrespective of the levels of plasma ANP. The small size and flat bottom of the TEMPSC predispose most occupants to seasickness, even in relatively calm waters. A combination of metoclopramide, dexamethasone, droperidol, lorazepam, and diphenhydramine was used in prophylaxis of high-dose (greater than or equal to 100 mg/m2) or moderate dose (greater than or equal to 50 mg/m2) cisplatin. Sixty minutes prior to starting cisplatin, 16 mg dexamethasone, 50 mg diphenhydramine, and 0.5 mg lorazepam were given orally (PO). Droperidol 1 mg was given intramuscularly (IM) 15 minutes prior to beginning cisplatin. Repetitive doses of intravenous (IV) metoclopramide, 2 mg/kg in 75 ml 5% dextrose in

over 15 minutes was given 30 minutes prior to, and at 1 1/2, 4 1/2, and 7 1/2 hours after beginning cisplatin chemotherapy. Only patients with nausea and/or vomiting received subsequent doses of 2 mg/kg metoclopramide IV every 3 hours as needed. Slightly increased urinary protein, reticulocyte counts, and fibrinogen were observed in a few animals in each group. The lungs of the moribundly sacrificed animals had moderate atelectasis, localized atelectasis, moderate thickening of alveolar wall and pleura, proliferation of fibroblast-like cells, and abundant fibers in interstitium and alveoli. In the liver there was slight hemorrhage along the gallbladder. On electron microscopy of the lung, proliferation of fibroblasts, myofibroblasts and type II alveolar cells, and some mast cells were observed in thickened alveolar walls.
