25 patients--19 to 73 years old--who underwent maxillofacial operations, received Spontavix for 10 to 12 days via a nasogastric tube. In 20% of the patients nausea, vomiting, diarrhea and/or abdominal pain occurred and disappeared after finishing nutrition with Spontavix. Mean frequency of defecation was 0.5/patient/24 hours. Body weight, serum electrolytes, blood gases, pH and base excess in the arterial blood, urea-nitrogen, hemoglobin and albumin content of the blood did not change significantly. Lipids in the serum increased insignificantly without leaving normal limits. During nutrition with Spontavix serum transaminases (SGOT, SGPT) showed a statistically significant increase which is believed to be caused by general anesthesia.

Wound healing and time of hospitalisation were the same as in a control group. Mean frequency of defecation was 0.5/24 hours. Body weight, serum electrolytes, fat content of the blood, blood gases, pH, base deficit, serum transaminases, urea-nitrogen and creatinine in the serum did not change significantly.

Congenital carbamyl phosphate synthetase deficiency was diagnosed by liver biopsy in a 13-year-old girl, alpha-Keto analogues of essential amino acids have been shown to spare nitrogen by reducing urea formation; hence, they were given to this patient in the hope of reducing hyperammonemia and improving protein tolerance. After intravenous infusion of the keto analogues of valine, leucine, isoleucine, methionine and phenylalanine, the corresponding plasma amino acids, including alloisoleucine and tyrosine, rose sharply. Twenty-four hours later, fasting plasma ammonia had fallen from the preinfusion value of 0.050 to 0.028 mM. Protein intake was kept at 0.5 g per kilogram for two weeks. Addition of keto acids by mouth reduced plasma ammonia and alanine to normal or near normal levels. Seizures and episodes of vomiting and lethargy decreased in frequency. Urinary nitrogen decreased, suggesting that nitrogen balance improved. These data indicate that keto acids may be useful in the treatment of congenital hyperammonemia.

Domestic kittens were used in four experiments to establish quantitative requirements for lysine and arginine. A purified L-amino acid diet (by calculation, 4,700 kcal metabolizable energy/kg diet) was employed throughout. Weight gain, gain:feed and nitrogen retention data of cats fed dietary lysine levels ranging from 0.48 to 1.92% suggested a requirement not exceeding 0.80%. The dietary arginine requirement for maximal gain was assayed at this level of lysine and found to be not greater than 0.83%. A dietary arginine level of 0.33% resulted in vomiting and extreme lethargy within 4 hours of ingestion.

In a 10-day study, carbenicillin indanyl sodium cured urinary-tract infections in 22 of 30 patients (ages, 24-91). In 3 of the remaining patients the treatment was a failure; in 3 others the drug had to be discontinued because of diarrhea and vomiting; and in 2 instances it induced overgrowth of Candida albicans in the urine. Carbenicillin was lethal to Pseudomonas aeruginosa in all 9 cases, to Proteus mirabilis in all 6 cases, and to enterococcus in all 3 cases. A trimethoprim/sulfamethoxazole combination cured urinary-tract infections in 18 of 30 other patients (ages, 28-91), but failed in 3. In 3 patients it gave rise to a skin rash; in 2 to elevation of blood urea nitrogen and creatinine levels; in 1 to neutropenia; and in 1 to overgrowth of Candida albicans in the urine. Reinfection occurred in 2 patients. Carbenicillin indanyl sodium was more effective than the sulfonamide/trimethoprim combination.

Labrador Retriever puppies (3 kg) were fed L-amino acid (L-AA) diets, containing the equivalent of 14% protein, to determine dietary argnine requirements for optimal growth and maintenance of normal intermediary metabolism. Growth and food consumption were depressed by decreasing the dietary arginine concentration. Urinary citrate and orotate increased with decreasing dietary arginine. Elevated blood orotate, urea and NH4+-N were detected in arginine deficient dogs. More than 0.56% arginine was required to support optimum growth and prevent abnormal loss of urinary metabolites. The effect of dietary nitrogen concentration (14, 21, or 28% L-AA) on arginine requirements was examined in immature Beagles. All arginine deficient dogs and dogs fed the 28% L-AA with arginine showed signs of emesis, excessive salivation and muscle tremors. Hyperammonemia and hyperglycemia were observed 2 hours after force feeding an L-AA diet devoid of arginine. Only hyperammonemia was observed in the Labrador Retrievers fed the same diet but incorporated into a 2% agar gel. Dietary nitrogen concentration or dietary arginine content dit not significantly influence glucose tolerance response to oral glucose loading. These data show that dietary arginine is required in the immature dog and that the requirement is influenced by dietary nitrogen concentration.

Hypoglycin A is a toxin which causes Jamaican vomiting sickness. In rats treated with this compound, we observed significant increases in a number of amino acids in the plasma and the urine, and detected several unidentified amino compounds in the urine. These changes suggest that hypoglycin, in addition to its known inhibitory effect on acyl-CoA dehydrogenases interferes with some steps in nitrogen metabolism as well. The amino acid abnormalities seen in hypoglycin-treated rats are compared with those seen in Reye's syndrome.

We assessed the severity of Acute Mountain Sickness (A.M.S.), indices of pulmonary gas exchange and nitrogen washout curves in healthy volunteers acutely exposed to high altitude. Symptoms of A.M.S. ranged from malaise to vomiting with intractable headache. The slope of phase III of the nitrogen washout curve increased most in those subjects with the most severe A.M.S. and who were most hypoxemic. The sickest subject also had the greatest increase in (A-a)DO2 and the largest increase in the slope of phase III. These abnormalities in gas exchange and nitrogen washout curves in the subjects with the most marked A.M.S. suggest that the manifestations of cerebral and pulmonary dysfunction at altitude develop simultaneously, although not necessarily by identical mechanisms.

4-Pentenoic acid, an analog of hypoglycin which is believed to cause Jamaican vomiting sickness, was administered intraperitoneally to rats in an attempt to produce the features of Reye's syndrome in rats. Mean ammonia levels in plasma were found to be elevated approximately four-fold after injection of 200 mg/kg pentenoic acid in fed rats. Pentenoic acid caused hypoglycemia in fasted rats and hyperglycemia in fed rats. In chronic experiments rats were injected intraperitoneally every 4 hr with 50 mg/kg body weight of pentenoic acid for 10 doses, followed by a single dose of 200 mg/kg. The livers of the treated group were enlarged and yellow and showed extensive fatty degeneration. The blood-urea-nitrogen (BUN) was significantly higher and the free fatty acids (FFA's) significantly lower in these rats. This study shows that pentenoic acid administered to rats produces findings similar to those of Reye's syndrome and Jamaican vomiting sickness.

Kwashkorkor is associated with malabsorption of energy and nutrients. Standard diets often initiate diarrhea and a high mortality is still prevalent. A synthetic monomolecular formula has been evaluated and compared with a standard diet in the early rehabilitation phase of 21 children with kwashiorkor. The formula group had significantly less vomiting and reached minimum weight faster than the group on standard diet. Weight gain and diarrhea were similar. The rise of albumin and BUN was faster on standard diet. A significant increase in haemoglobin was seen only in the formula group. A rise in body temperature after a meal was evident in most patients and significantly more pronounced in the formula group. The lower total nitrogen content of the formula may explain the observed slower rise in albumin and BUN but the ready utilization was indicated by the favourable weight changes as well as the rise in rectal temperature. As high energy per volume was desirable the formula was not diluted to isoosmolality. However, the high glucose concentration in the experimental diet probably caused some negative effects.
