Women who have missed a dose of their oral contraceptive (OC) and/or experienced condom breakage during sexual intercourse may be in need of emergency contraceptive pills (ECPs). Anita Nelson, MD, medical director of the Women's Health Care Clinic, Harbor-UCLA Medical Center in Torrance, CA; Sharon Schnare, FNP, CNM, family planning clinician and consultant; and Susan Wysocki, RNC, BSN, NP, president of the National Association of Nurse Practitioners in Reproductive Health in Washington, DC, offer their recommendations on the repeat consumption of ECPs. Specifically, they explain whether a client who experiences condom breakage 4 days after using ECPs will need to repeat ECPs, and what they recommend regarding repeat ECPs in the same month for a client using antibiotics and OCs. Nelson, Schnare, and Wysocki agree that rifampin and griseofulvin are the only antimicrobial agents known to be capable of reducing OC efficacy. As such, ECP administration is not recommended together with antibiotics and OCs unless the client has not taken her OCs due to illness or unless OC effectiveness may have been reduced by vomiting or severe diarrhea. Schnare and Wysocki recommend repeat ECP administration in the condom breakage scenario; Dr. Nelson made no recommendation.
