We present a case of a 79-year-old woman with periods of syncope, complete heart block (CHB), and ventricular standstill during periods of increased vagal tone following sublingual nifedipine for hypertension. The
syncopal episodes
were associated with periods of elevated vagal tone (micturition and
vomiting
) with one monitored episode showing a clear time course of
emesis
; CHB then ensued, progressing to ventricular standstill with loss of consciousness that resolved over several minutes. Although nifedipine is not thought to affect conduction at current clinical dosages, it seems likely that the additive effects of nifedipine and elevated vagal tone produced the observed conduction abnormalities. This is the first case report of nifedipine administration followed by syncope and conduction disturbances.

30 healthy women between 14-37 and between 14-20 weeks' gestation were administered with a single injection of 40 mg of PGF2 alpha over a 5 minute period to induce abortion. 28 patients aborted within 48 hours; 14 aborted completely and 14 required surgical removal of the placenta; 1 patient required intravenous oxytocin, and 1 failed to abort within 48 hours. . A single injection of PGF2 alpha seems superior to intraamniotic hypertonic saline solution administration alone, and is effective as administration of hypertonic saline and intravenous oxytocin. The multiple dose technique of PG administration does reduce the mean abortion time, but entails an increased incidence of gastrointestinal side effects and requires repuncture or the use of an indwelling catheter, which may increase the incidence of infection. The mode of administration described can be used in selected patients when hypertonic saline solution is contraindicated.

Dobutamine echocardiography has become widely used in the past decade in the evaluation of patients with suspected coronary artery disease who are unable to undergo exercise treadmill or bicycle testing. The safety of this procedure has been studied in a hospital-based setting. However, no studies thus far have evaluated the safety of this procedure in an office-based setting, remote from a hospital. We performed dobutamine echocardiography on 127 patients in an office-based setting, remote from a hospital. Throughout the course of this study there were no deaths, myocardial infarctions, sustained episodes of ventricular tachycardia, or
syncopal episodes
associated with dobutamine infusion. The frequency of noncardiac side effects was 29%, the majority of which were nausea,

, and paresthesias. Three patients had nonsustained ventricular tachycardia, none of whom had symptoms. We conclude that dobutamine echocardiography is safe, well tolerated, and useful in an office-based setting.

Chiari type I malformation is characterized by herniation of the cerebellar tonsils through the foramen magnum. An association between Chiari type I malformation and cystic fibrosis (CF) has not previously been established. We report on five children and adolescents with CF in whom Chiari type I malformations were diagnosed. Three patients were 17-18 years old at time of diagnosis, one was 3 years old, and one was 10 months of age. All patients were followed at the Cystic Fibrosis Center at St. Christopher's Hospital for Children and were diagnosed with the malformations between June 1988 and June 1997. Over this same period, 400 CF patients 18 years or younger were followed routinely. All patients had the diagnosis of Chiari type I confirmed by brain-stem MRI. Neurologic findings included swallowing dysfunction,

, and headaches. No two patients had the same presenting neurologic findings. Our data suggest that Chiari type I malformation is more common in CF than in the general population. This is the first report of such a complication after LAGB, demonstrating the hemodynamics of HOCM in a unique setting and underscoring the need for a structural cardiac assessment prior to bariatric surgery.

This report is of particular interest, as it suggests that NMOSD should be included in the differential diagnosis of patients with intractable hiccups and heart rhythm disorders.
