Meningitis is endemic in Vanuatu and other Pacific island countries and has a high case fatality rate. The incidence in the southern island of Tanna is especially high. This descriptive study of 64 cases in children (under 15 years) on that island was undertaken over a 21-month period from January 1988 to September 1989. Meningococcus was identified in 23 cases (36%) and Pneumococcus in 12 (19%). The age distribution showed a high rate in under 1 year olds. The symptoms of fever, convulsion and
vomiting
were most common.
Bulging fontanelle
(in children under 1 year), neck rigidity, and altered level of consciousness were the most frequent signs. These signs were as frequent in meningococcal as pneumococcal infections and were used to develop a simple protocol for use in primary care. Delay in treatment or referral because of patients seeking traditional medicine is a major problem yet to be overcome. Only a weak association between admissions with meningitis and underweight or crowding was found in this study.

A retrospective review of charts for 650 children who had lumbar puncture for suspected meningitis was undertaken to determine the characteristics of patients with and without meningitis, identify other conditions suggesting meningitis, and evaluate the predictive value of signs and symptoms of meningitis. The incidence of positive lumbar punctures increased with patient age. Younger infants did not present with classical features of meningitis.

, lethargy, and irritability were nonspecific symptoms.
Vomiting
and headache, although not specific, proved to be more sensitive indicators of meningeal infection. Most patients with meningitis (75%) had at least one sign of meningeal irritation, but so did 25% of patients without meningitis. Brudzinski's sign was not specific. In contrast, nuchal rigidity and Kernig's sign had high predictive value. Up to age five, the diseases most often suggesting meningitis were right-sided pneumonia, gastroenteritis, otitis, tonsillitis, exanthema subitum, and urinary tract infections. Of 171 patients with febrile convulsion, one (0.5%) had bacterial meningitis and four had aseptic meningitis.

B.I.H. or Pseudotumor is rare in children particularly under the first year of age. In addition, the syndrome at this age presents some peculiarities as compared with the adult form. This report describes the authors' experience with patients aged 4 to 12 months.

in 4. Treatment was limited to lumbar puncture which was initially performed to rule out an acute CSN inflammatory process. In 8 patients the syndrome was precipitated by medications (sulfenazone - nalidixic acid); 3 had a prior history of 3 days of fever and roseola infantum, in 2 cases precipitanting factors were not found. Since the prompt remission of the intracranial hypertension (24 to 96 hours) neuroradiological investigations were not performed.
