Report is made of a case of an unusual localization of a
trichobezoar
in a 12 year-old girl, who presented
vomiting
and abdominal pain as chief complaints. She was found to have an 18 X 5 cm palpable abdominal mass, which upon surgery was localized in terminal ileum and in the pathology report was found to be a
trichobezoar
. The interest of this case is the unusual localization of a single bezoar and the fact that a resection of 25 cm. of small bowel had to be done, as it was found necrotic at the time of surgery.

A twenty year old girl was referred to the surgical out-patients clinic with a history of intermittent epigastric pain and
vomiting
of five months duration. The patient enjoyed good health prior to this and her past medical history was uneventful. Findings on physical examination included pallor, patchy alopecia and a soft mobile, non-tender mass in the epigastrium and left upper quadrant of the abdomen. When questioned she admitted to a history of trichophagia for as long as she could recall. A provisional diagnosis of gastric
trichobezoar
was made. Radiological investigations included an abdominal ultrasound which showed a large ill-defined mass lesion with poor transonic features, situated in the upper abdomen and extending from the left upper quadrant across the midline to the liver margin. Barium meal revealed that the greater part of the lumen of stomach was occupied with material of an indeterminate nature. Her haematological investigations showed Haemoglobin 9.5 gm/dl and Leucocyte Count 9.0 x 10(9)/L. Her Urea and Electrolytes were within normal range. At laparotomy a large hair ball extending from the Stomach into the duodenum and proximal jejunum was removed through a vertical gastrotomy incision. The patient had a satisfactory post-operative convalescence and was discharged two weeks later.

For several weeks a 15-year-old girl had complained of increasing abdominal pain with
vomiting
. On admission to hospital the bloated abdomen was diffusely sensitive to pressure and the bowel sounds were high pitched and loud. Erythrocyte sedimentation rate was increased to 23/40 mm and the white cell count to 12,000/microliters. Ultrasound examination revealed an echo-dense area with dorsal echo loss at the gastric side of the pylorus. X-ray films of the stomach showed fluid levels and a soft-tissue mass in the left upper abdomen. At laparotomy a large (12 x 6 cm)

was found in the middle of the small intestine and two smaller ones at the pylorus. Subsequently the patient admitted to trichophagia but refused any psychological treatment.

extending throughout the bowel. A 8-year-old girl was admitted to our institution with a six months history of
vomiting
and astenia. Upper gastrointestinal contrast study showed a lot of filling defects in stomach and the first bowel loops. At laparotomy a large intraluminal mass is observed. Gastrotomy revealed a large tricobezoar with intestinal extension to jejunum, that were extracted. In rare instances, this syndrome presents with a confusing clinical picture. A detailed medical history is an essential point in the diagnosis.
