Ceruletide, a decapeptide, is a potent cholecystokinetic agent with a direct spasmogenic effect on the gallbladder muscle and bile ducts in humans and animals. It was recently approved by the Food and Drug Administration for use as an adjunct in x-ray examination of the gallbladder and small bowel. The drug causes a coordinated propulsive activity from the duodenum to the ileum and segmenting activity in the colon. Because of this stimulatory effect, ceruletide is useful not only diagnostically as an aid in x-ray examination of the small bowel, but also therapeutically for treatment of postoperative
ileus
, intestinal atonia, and chronic fecal statis. Because of its pancreatic stimulatory action, it is useful in evaluation of exocrine pancreatic function. In therapeutic doses the adverse effects noted are mild, transient extensions of the drug's pharmacologic actions and are manifest as nausea,
vomiting
, abdominal pain, and rarely hypotension and tachycardia. On the basis of current evidence, ceruletide is a safe and effective cholecystokinetic agent and small bowel and exocrine pancreatic stimulant.

It is reported on the diagnosis of a gallstone
ileus
in a 73-year-old female patient. In an otherwise quiescent anamnesis an increasing
vomiting
has been since 10 days. The X-ray examination of the stomach with visotrast 370 resulted in the suspicion of a pyloric stenosis. By means of emergency gastroscopy an independent disease of stomach and duodenum was excluded, but in the duodenal bulb 2 gallstones of the size of a hazel-nut and a kernel of the hazel-nut with references to a cholecystoduodenal fistula were established. In this case the emergency gastroscopy resulted in the indication to the immediate operation on account of gallstone
ileus
with previously unclear symptomatology of an
ileus
. Additionally a nearly hen's egg-sized stone was removed aborally 30 cm from Treitz's fascicle.

An attack of gallstone
ileus
observed in a 60-year-old female patient is reported. In this patient who previously had been cholecystectomized, instant extraction of giant residual gallstones was unsuccessful despite a large endoscopic sphincterotomy. Three days later, she developed colicky abdominal pain and
vomiting
. At laparotomy nine days after the endoscopic procedure an impacted gallstone measuring 3.5 cm in diameter was removed from the the jejunum, some 50 cm below the ligament of Treitz. This observation demonstrates an unusual complication of endoscopic sphincterotomy and clearly outlines that very large stones can, after an initial delay, pass into the duodenum despite an apparently "unsuccessful" sphincterotomy.

, and obstipation often occur in children and young adults with cystic fibrosis (CF). The common causes include meconium
ileus
equivalent, intussusception, and adhesions from previous surgery. One of our patients with CF who had meconium peritonitis as a neonate presented with duodenal obstruction during childhood. This was caused by colonic polyps arising in the hepatic flexure and eroding through the colonic and duodenal walls into the duodenum. She was treated with total parenteral nutrition, right colectomy, gastric diversion, and a controlled duodenal fistula that healed uneventfully. She has remained well 1 year after discharge from the hospital. To our knowledge, a similar case has not been reported previously.

Of 109 dogs submitted with complications following ovariohysterectomy, twenty were found to show enterological problems. Presenting symptoms in these animals consisted in
vomiting
, diarrhoea and emaciation. A large number of animals also showed body temperatures above 39 degrees C. An irregular, often painful mass was palpable on examination of the mesogastrium in seventeen cases. Plain radiography revealed the features of
ileus
in ten dogs, a mass of soft tissues in the abdominal cavity in six and the pattern of peritonitis in five cases (two animals were classifiable with two groups). One dog did not show any changes. Laparotomy was performed in all twenty dogs. In view of the extent of the lesions, euthanasia was performed in ten animals. Partial enterectomy was performed in the other ten; some of these dogs are still alive at the time of writing. The findings at laparotomy and subsequent post-mortem examination, if any, may be summarized as follows: - there were adhesions between the ovarian-stump granulomas and an intestinal loop in five animals; - twelve animals showed adhesions between the ovarian-stump granuloma and the intestines which also adhered to each other; in five of these bitches, the intestinal loops were also attached to the wall of the abdomen, and also to the cervix stump in two cases; - three dogs only showed adhesions between intestinal loops; the cervix stump was involved in the adhesions in one dog.

Maytansine is an experimental antitumor agent that has shown minimal efficacy against breast cancer with minimal myelosuppression in phase I trials. Forty-one patients with advanced drug-resistant breast cancer were treated with a 5-day intermittent iv infusion of maytansine repeated every 21 days. All patients had been heavily pretreated with cyclophosphamide, methotrexate, 5-fluorouracil, or doxorubicin, and 12 had received vinblastine, mitomycin C, or investigational drugs. All patients had measurable disease and an expected survival of 6 weeks. The average performance status was 2.5. Twelve patients did not complete one full cycle of therapy, leaving 29 evaluable for response. One patient had a partial regression of pulmonary disease, seven had transient responses of less than 50% reduction in tumor or stable disease, and 21 had progressive disease. Toxic effects (
vomiting
, diarrhea,
ileus
, lethargy, and altered mentation) were considerable. Since maytansine is a relatively nonmyelotoxic metaphase inhibitor, we feel that even minimal efficacy in heavily pretreated patients justifies further evaluation of the agent in combination therapy.

Two fatalities due to an occlusive
ileus
following the ingestion of foreign bodies in patients with psychiatric disorders are described. A severely mentally handicapped young man developed a temperature and died 1 h after admission to a surgical ward. At autopsy, approximately 2000 cm3 of foreign material, including broken glass and porcelain, branches, buttons, parts of clothing and other material were found in the gastrointestinal tract, leading to a complete obstruction of the distal intestine and colon with resulting faecal
vomiting
. The other case was even more unusual as hair fetishist had swallowed a thick strand of his own hair, 50 cm long, also resulting in mechanical obstruction of the distal intestine.

Previous data have suggested that laparoscopic colon and rectal surgery may shorten the length of hospitalization. These claims have been attributed to a reduction of the length of
ileus
. The definition of "ileus" is variable and in all cases is subjective. In this study it was defined as the length of time until the patient passed flatus or stool without nausea,
vomiting
or abdominal distention. This prospective study was undertaken to compare the duration of
ileus
and of hospitalization after laparoscopic-assisted (LAC) and standard laparotomy (SC). After restorative proctocolectomy with an ileal-pouch anal anastomosis (IPAA) in both sets of patients. Twenty-two patients underwent LAC and 20 age, sex, and diagnosis-matched controls underwent SC. Mucosal ulcerative colitis (MUC) was the diagnosis in 16 LAC and in 15 SC patients while polyposis was the diagnosis in 6 LAC and in 5 SC patients. The mean time to resolution of postoperative
ileus
was 4.2 (4-11) days in the LAC group and 3.3 (2-5) days in the SC group. Hospital discharge was similar in each group occurring at a mean of 8.7 (7-13) days after LAC and 8.9 (6-18) days after SC. Neither the length of time for
ileus
resolution nor the length of hospitalization were reduced in the LAC group. Laparoscopic-assisted IPAA conferred none of the theoretical advantages associated with other laparoscopic procedures.
