Choledochocele
is the least common form of cystic dilatation of the biliary tree. Whether this condition is a congenital abnormality or the result of inflammation at the papilla of Vater is not clear. In most cases, the clinical presentation is that of intermittent abdominal pain, nausea, and

, with biliary colic and/or jaundice. As described in our case, choledochocele has characteristic radiologic features, but the lesion can be easily overlooked if one is not aware of its distinctive appearance.

Choledochocele
belongs to type III biliary cysts in Todani's classification and are considered the least frequent cysts in this class. The usual definition of choledochocele is that of a cystic expansion of the distal intramural portion of the bile duct that protrudes into the duodenal lumen. Endoscopic retrograde cholangiopancreatography (ERCP) is one of the tests of choice both for diagnosis and treatment. We report the case of a 77-year-old patient with chronic epigastric pain, nausea/

and episodes of cholangitis due to a choledochocele. Magnetic resonance cholangiography and other radiological procedures (ultrasound, computed tomography, magnetic resonance) were not diagnostic. ERCP was essential to diagnosis and treatment since this procedure provided the treatment by means of biliary sphincterotomy.

Choledochocele
, or type III choledochal cyst, is a rare congenital disease and is even less common among adults compared with children. In this case, a 75-year-old female was admitted to our hospital presented with epigastric pain and
vomiting
for one day. Abdominal computed tomography revealed dilated common bile duct, pancreatitis and peripancreatic effusion. The patient was treated with fasting, fluid resuscitation, anti-acid agents, somatostatin and antibiotics. Endoscopic retrograde cholangiopancreatography was employed for the further diagnosis of choledochocele, choledocholithiasis and biliary stenosis. Endoscopic sphincterotomy, stone extraction and plastic stent placement were performed for treatment. The patient recovered quickly after the treatment and no signs of recurrence and complications were observed during the first follow-up. Endoscopic management may be a promising and alternative therapy for choledochocele although long-term follow-up is necessary to confirm the efficacy and safety of this procedure in the future.
