A case of gold-induced
enterocolitis
and a review of the literature are reported. Gold-induced
enterocolitis
appears to be an uncommon reaction, occurring in middle-aged females who have received low doses of gold preparations. Symptoms may include fever, nausea,
vomiting
, abdominal cramps, and diarrhea with or without blood. The whole of the gastrointestinal tract may be involved and fatal cases occur. The diagnosis of a primary enteropathic arthropathy may be suggested. Cessation of gold, glucocorticoids and supportive therapy are indicated. The mechanism of the reaction is still unknown.

Two cases of postoperative
enterocolitis
due to methicillin-resistant Staphylococcus aureus (MRSA) after gastrectomy were experienced. Case 1: A 59-year-old male underwent subtotal gastrectomy for advanced gastric cancer. Diffuse peritonitis progressed after the first operation, so reoperation for drainage was required. Two days after the second operation, a profuse watery diarrhea developed. Case 2: A 46-year-old male underwent total gastrectomy for early gastric cancer. On the fourth postoperative day, frequent
vomiting
and cholera-like diarrhea started, followed by profound shock several hours later. Both cases were treated successfully by the administration of vancomycin. Stool cultures of both cases revealed MRSA and it had the same minimal inhibitory concentration, coagulase type and enterotoxin type, so that nosocomial infection was indicated.

Fatal neutropenic
enterocolitis
was seen in a patient undergoing autologous bone marrow transplantation for non-Hodgkin's lymphoma. Excessive drug action due to a mildly diminished creatinine clearance could have contributed to the pathogenesis. Computed tomographic scanning and ultrasonography demonstrated pneumatosis of the gastrointestinal tract, but the disease had become extensive by then. Necrotizing enterocolitis should be suspected early in a granulocytopenic patient with abdominal pain and diarrhea or
vomiting
. Aggressive surgical or medical management may avoid a fatal outcome.

The clinical experience obtained while treating 43 dehydrated newborns due to diarrhea with oral rehydration solution (ORS) using the formula recommended by the World Health Organization is reported. Of the 43 patients, 26 were severely dehydrated (greater than equal to 10% of weight recovery once rehydrated). The averaged time need to correct the dehydration was 4.7 +/- 2.7 hours, with a average intake of ORS of 26.5 +/- 7.5 mL/kg/hour. Children who were being breastfed continued so during the rehydration period. Two of the patients were hospitalized for intravenous treatment, one was due to persistent
vomiting
during rehydration and probably due to sepsis, and the other due to necrosing
enterocolitis
. The oral rehydration therapy was successful in 95% of the newborns included in the study, which proved the method to be safe and adequate for the correction of dehydration due to diarrhea among these patients. Similar experiences are reported in Mexico as well as from other countries, which also suggest the use of this therapeutic procedure in children of this age.

Neutropenic enterocolitis is a recognized complication of immunosuppression or chemotherapy for leukemia. It presents as severe abdominal pain and tenderness, fever, and diarrhea associated with granulocytopenia. Gastrointestinal symptoms associated with chemotherapy for head and neck neoplasms include nausea and
emesis
, but not acute abdominal distress. We present, to our knowledge, the first case of neutropenic
enterocolitis
in a patient receiving cisplatin and fluorouracil chemotherapy for metastatic head and neck cancer.

Neutropenic enterocolitis is a symptom complex of fever, abdominal pain, distention, nausea,
vomiting
, diarrhea, and bloody stools occurring in a patient with a low neutrophil count and is most often seen in patients with acute leukemia after a course of chemotherapy. In most cases, neutropenic
enterocolitis
is a self-limited condition, but complications of transmural intestinal necrosis and bowel perforation may occur in a small number of patients. Surgical management should be reserved for those patients with bowel wall necrosis or perforation; however, early identification of these patients is difficult. We report our experience with the use of diagnostic peritoneal lavage in three patients with the symptoms and signs of neutropenic
enterocolitis
. In each case, Gram's stain of lavage fluid revealed no evidence of polymicrobial contamination of the peritoneal cavity. All three patients were managed medically, with resolution of their abdominal symptoms. Peritoneal lavage is helpful in excluding bowel perforation and avoiding unnecessary surgical intervention in patients with neutropenic
enterocolitis
.

A case of methicillin-resistant staphylococcal (MRSA)
enterocolitis
following combination chemotherapy for acute promyelocytic leukemia is presented. MRSA
enterocolitis
has characteristic clinical features of high fever, frequent
vomiting
and watery diarrhea, and its mortality rate is very high without a proper antibiotic therapy. When the patient with hematological malignancy has the above-mentioned clinical manifestations during antineoplastic chemotherapy, appropriate antibiotics for MRSA should be promptly begun before a bacteriological diagnosis.

Some clinical manifestations following exchange transfusion (ET) could result from graft versus host disease secondary to the introduction of viable foreign T lymphocytes: skin rash, fever, acute and sometimes bloody diarrhea or
enterocolitis
. Between February 1985 and January 1989 the blood used for 31 ET was irradiated at 40 grays. We compared the manifestations occurring during the days following ET to those occurring after 44 previous ET with non irradiated blood during the period January 1981 to January 1985. From 1981 to 1985, 13 of 44 infants developed problems within 3 days following ET: an erythematous macular skin rash in 4; gastrointestinal manifestations (diarrhea,
vomiting
and rectal bleeding, necrotizing enterocolitis) in 7; both skin lesions and a gastrointestinal problem in 2. Since 1985, 27 infants had no problems whereas only 4 developed gastrointestinal or cutaneous manifestations: NEC in a preterm infant, abdominal distension with rectal bleeding, fever and petechial rash in 2 infected infants. These data show a dramatic decrease of complications since the irradiation of blood products has been started: 30% with non irradiated, 13% with irradiated blood.

Most patients with Hirschsprung's disease have symptoms of constipation and abdominal distension in the first several months of life. The development of
enterocolitis
in these patients, characterized by diarrhea, abdominal distension, and
vomiting
, is most common in the first 3 months of life and has been associated with a 30-50% mortality. We report a child with previously normal daily bowel movements who first developed constipation at 16 months of age; only when severe colitis and chronic lower gastrointestinal bleeding appeared at 3.5 years of age was the diagnosis of Hirschsprung's disease made.

A case of gold associated
enterocolitis
is described. A review of all 27 previously reported cases revealed that the syndrome induced has common characteristics. The reaction occurs within three months of instituting gold therapy, is characterised by profuse diarrhoea and
vomiting
with abdominal pain, fever, and sometimes eosinophilia. Petechial changes are prominent on endoscopy and the endoscopic and histological features of the gut lesion do not resemble inflammatory bowel disease. The overall mortality is 26% but has decreased in recent years. There is no specific therapy but in severe cases diversional surgery may be justified.
