The effect of an antibiotic of the streptothricin class on Echinococcus granulosus and Taenia hydatigena infections has been investigated in a trial involving 160 dogs. The drug showed significant activity in freeing dogs from Thydatigena, but the evidence for similar activity against E granulosus was less well marked. Vomiting and diarrhoea were common sequelae of dosing. The preparation tested consisted of four chemical components and the cestocidal value of each was not determined.

Diuredosan has been tested against Echinococcus granulosus and Taenia hydatigena infections in dogs. The drug showed significant activity against T hydatigena, but there was no significant dose response curve against E granulosus in this trial. Two treatments at 50 mg/kg or 100 mg/kg, however, reduced the number of dogs infected with hydatid worms. Worms were still present in some dogs after three treatments at 50 mg/kg. Vomiting and diarrhoea were relatively common sequelae at and above 50 mg/kg.

The effect of niclosamide (particle size 1 micron m) and its piperazine salt (particle size 5-10 micron m) on Echinococcus granulosus and Taenia hydatigena has been investigated. The piperazine salt of niclosamide was also tested for activity against Taenia ovis. These trials involved 326 dogs. Irrespective of particle size, the drug was found to be virtually inactive against 28-day-old E granulosus. Some evidence was found indicating that there may be a low degree of activity against 70-day-old worms. The drug was found to be effective at 32 mg/kg and 64 mg/kg against T hydatigena and 50 mg/kg against T ovis. Some evidence was obtained that milk decrease the efficiency of the piperazine salt against T hydatigena at the lower dose rates. Micronisation of the drug did not increase the efficiency against this parasite. Vomiting and diarrhoea were common sequelae.

Trials with bithionol sulphoxide involving 130 dogs, infected with either Echinococcus granulosus or Taenia hydatigena, indicated that a single treatment at a dose rate of 400 mg/kg or two treatments at 50 mg/kg eliminated the former, but a single treatment at 50 mg/kg was sufficient to eliminate the latter. Dose rates 200 mg/kg or more were lethal to some dogs. With two doses of 50 mg/kg, there was only minor discomfort from vomiting or diarrhoea in some dogs.

Trials with fospirate carried out with 170 dogs infected with either Echinococcus granulosus or Taenia hydatigena indicated that a dose rate of 40 mg/kg given on three occasions and 10 mg/kg given on one occasion eliminated each species, respectively. Increasing the dose rat substantially above 40 mg/kg did not reduce the number of treatments required to eliminate all hydatid organisms. Vomiting was observed in some dogs.

Trials with nitroscanate carried out on 186 dogs infected with either Echinococcus granulosus or Taenia hydatigena indicated that a single treatment at a dose rate of 1000 mg/kg or two treatments at 250 mg/kg eliminated the former, but a single treatment at 64 mg/kg was sufficient to eliminate the latter. At this dosing schedule, vomiting and diarrhoea occurred, as well as a transient 'tranquillising' effect on some dogs.

Seventy dogs were used to evaluate efficacy of divided doses of fospirate against immature Echinococcus granulosus infections in dogs. Dose rates of 10 to 80 mg/kg given on 1 or 2 occasions resulted in the clearance of 70.6 to 94.5% of expected worm numbers. At least 3 treatments may be required before dogs can be free from E granulosus. Vomiting, which occurred in dogs given doses of 40 mg or more/kg, seemed to interfere with anthelmintic efficacy in some dogs.

Hydatid disease affects sheep and cattle and is caused by the larval form of Echinococcus granulosus. Affected human beings are only chance intermediate hosts. We present a woman admitted in her 22nd week of pregnancy because of upper abdominal pain and persistent vomiting. The presumptive diagnosis of hydatid cyst of the liver was established by ultrasound, X-ray and CT examinations. At laparotomy a 10 x 7 cm hydatid cyst was removed from the liver. She recovered completely and delivered spontaneously at term. There was no evidence of hydatid disease in the newborn or in the placenta.

Three patients are described with hepatic involvement by Echinococcus granulosus, complicated by spontaneous rupture into the biliary tract. Clinical features consisted of upper abdominal pain, jaundice, fever, anorexia, and vomiting. Hepatomegaly and marked epigastric tenderness were consistently observed. Laboratory findings included obstructive liver function tests, leucocytosis, eosinophilia, and hyperamylasemia. Abdominal computed tomography, showing the cystic wall, the presence of wall calcifications, daughter cysts and wall enhancement, provided a correct diagnosis of hepatic hydatidosis in all patients. Dilatation of the bile ducts with the presence of intraluminal material was clearly shown by sonography and endoscopic retrograde cholangiography. These abnormalities were most frequently found in the common bile duct and in the left hepatic duct. On sonography, the intraluminal material appeared as amorphous, sludge-like hydatid sand, and as daughter cysts. On ERCP, the intrabiliary parasitic material appeared as non-homogeneous, irregularly shaped and mobile filling defects. Other findings at ERCP were displacement and distortion of intrahepatic bile ducts by the hepatic cysts and a mild dilatation of the pancreatic duct. In one occasion, evacuation of a daughter cyst through the papilla was observed. The therapeutic value of mebendazole and endoscopic sphincterotomy in our patients is discussed.

The case is described of a 32-year old woman with an 8 year history of oral contraceptive (OC) use who developed vascular complications. Significant findings in the patient's history included an appendectomy and repeated biliary colic dating back 7 years. The patient sought help for an attack of hepatic colic with vomiting, chills, and fever, dyspepsia, and intolerance of fats. Pain was noted on palpation and the clinical and sonographic findings indicated hepatomegaly. Based on the other clinical and laboratory findings, a preliminary diagnosis of infected hepatic hydatidic cyst was made and the intrahepatic hematoma was drained. The postoperative diagnosis was a large hematoma occupying the greater part of the right hepatic lobe. A pleural hemorrhage occurred during postoperative hospitalization and was treated medically, but 4 days after discharge from the hospital the patient returned with a pleural hemorrhage that required drainage. Hydatidosis is endemic in the region of Spain where the case occurred, and the grounds for differential diagnosis are specified. Several illustrations including sonograms, X-rays, and results of computerized axial tomography are included and explained. With the increasing use of OCs in Spain, it is likely that more such cases will be seen.
