The case of a girl who presented with gastrointestinal upsets with nausea,
vomiting
and occasional hypoglycaemic attacks during childhood is reported. At about 5 years of age generalised muscular weakness with severe amyotrophy, cardiomegaly with a cardiothoracic ratio of 0,63, left ventricular hypertrophy on electrocardiography and left ventricular dilatation with hypokinesis on echocardiography were observed. A few weeks later she developed severe cardiac failure. Muscle biopsy showed muscular dystrophy with lipid infiltration due to carnitine deficiency )serum carnitine 9 nmoles/ml, normal values: 46 +/- 6,9 nmoles/ml; muscle carnitine 0,27 nmoles/mg, normal values: 3,0 +/- 0,79 nmoles/mg fresh frozen weight).
She
improved rapidly with carnitine chlorhydrate and a diet low in lipids and high in medium chain triglycerides. Regression of muscular symptoms and cardiac failure was observed. After 13 months follow-up with no tonicardiac therapy she is much improved; the signs of heart failure have disappeared, the cardiothoracic ratio is now 0,55 and the electrocardiogramme and echocardiogramme are normal.

In 1958 a mediastinal tumor was discovered in an asymptomatic patient.
She
had previously complained of
vomiting
and an oesophageal abnormality had been discovered though not treated. The mediastinal tumor was resected and diagnosed as a non chromaffine chemodectoma. In the follow-up the mediastinal image never appeared normal and the heart shadow was enlarged. In 1974 an infiltrate appeared in the left lung. In 1975 the patient was operated : a pulmonary osteochondroma, a pericarditis and an intrapericardial aortopulmonary chemodectoma were discovered. The mediastinal chemodectoma is a rare tumor and its frequency, location and signs are discussed. The importance of angiography for diagnosis is stressed. The authors consider malignant degenerescence to be rare, and the treatment is surgical as the tumor is radioresistant. In the present case there was either a malignant recurrence or multiple focuses with a possible oesophageal location.

A 69-year-old woman had a large gallstone eroded through the wall of the gallbladder and into the duodenum. The gallstone became fixed within the cholecystoduodenal fistula and produced a duodenal obstruction. At the time of the diagnosis of this problem, the patient had had a three-week period of epigastric pain and prolonged
vomiting
.
She
was treated by removal of the stone and cholecystectomy. The duodenal fistula was closed in two layers, and a gastrojejunostomy and a truncal vagotomy were carried out to protect the duodenal suture line. A leak from the duodenal closure developed on the fourth day, but this subsided spontaneously after 15 days with the use of sump drainage. Six months later, the patient is doing well and has a normal duodenum and gastrojejunostomy as shown by upper gastrointestinal barium study.

A 16-month-old infant ingested an alkaline battery, 22 mm x 5 mm, which became entrapped in her esophagus.
She
was brought to the emergency department with a four-day history of
vomiting
. After tube thoracostomy, the battery was removed by esophagoscopy. However, she experienced cardiac arrest in the intensive care unit. Post mortem examination revealed exsanguination by perforation of the posterior aspect of the aortic arch as the cause of death. Microscopic examination showed liquefaction necrosis through the mucosa and submucosa of the esophagus into the upper mediastinum and right pleural space. The possible role of the alkaline center of the battery causing perforation is presented.

Case history is reported of an infant who was changed from breast to bottle feeding at fourth day of life, who on ninth day developed bilious
vomiting
and bloody diarrhoea. Later gas was found in colonic wall on X-ray. Necrotizing enterocolitis was diagnosed and infant managed with total intravenous alimentation. On recovery although she tolerated breast milk, return to cow's milk formula lead to return of bloody diarrhoea. Subsequently she thrived on a cow's milk-free diet based on chicken meat formula. At four months after total of 15 ml cow's milk she had an acute anaphylactic reaction.
She
recovered and is well on milk-free diet. Thus, this infant has recovered from necrotizing enterocolitis but has evidence of persistent cow's milk protein intolerance. The hypothesis is put forward that in this child cow's milk protein played a major role in the pathogenesis of necrotizing enterocolitis.

An 8-year-old white girl with a history of vertigo, nausea, and
vomiting
developed a progressive hearing loss, bilateral retinal arteriolar narrowing in each eye, vasoproliferation, and subsequent intravitreal hemorrhage. An attempt at peripheral retinal ablation with cryotherapy in the left eye resulted in retinal detachment. Spontaneous retinal detachment occurred in the right eye and was successfully repaired. Repeated intermittent hemorrhages occurred despite intraocular diathermy. Three years after onset, visual acuity was R.E.: 6/21 (20/66) and L.E.: light perception.
She
remains totally deaf. A 20-year-old white woman developed severe bilateral sensorineural hearing loss with poorly functioning labyrinths, followed by midperipheral retinal arteriolar occlusions and vasoproliferation on the optic nerve head. Progressive retinal neovascularization was followed by rubeosis iridis and repeated episodes of intravitreal bleeding. Six years after onset, visula acuity was R.E.: hand motions, and L.E.: 6/3 (20/100).
She
remains totally deaf. Both patients were of normal gestation, development, and mentality, without evidence of other systemic disease. The cause of this disease was not found.

A 23-year-old woman had oligomenorrhea, underdevelopment of the breasts, moderate hirsutism and increased serum testosterone values associated with a benign noncystic granulosa cell tumour of the left ovary.
She
was frail, irritable and apathetic. Since the age of 7 she had had periodic abdominal pain with nausea,
vomiting
and dizziness; irritability and occipital headache appeared when she was older. Her symptoms resolved and the masculinization did not progress after the tumour was removed. Only six similar well documented cases have been reported.

Report is made of a case of an unusual localization of a trichobezoar in a 12 year-old girl, who presented
vomiting
and abdominal pain as chief complaints.
She
was found to have an 18 X 5 cm palpable abdominal mass, which upon surgery was localized in terminal ileum and in the pathology report was found to be a trichobezoar. The interest of this case is the unusual localization of a single bezoar and the fact that a resection of 25 cm. of small bowel had to be done, as it was found necrotic at the time of surgery.

40 patients--5 to 50 years of age--voiding eggs of Schistosoma haematobium in their urine--underwent treatment with Metrifonate at the OPD of Bong Mine Hospital, Liberia. The patients received 10 mg/kg body weight 3 times at a fortnight's interval. The drug was swallowed under medical supervision. 27 patients no longer passed eggs after the 1. dose of Metrifonate, 37 no longer voided eggs after the 2. administration. 1 patient did not show up for control after the 3. dose. Theoretically he may not be healed. 1 other patient who came for control after 12 months had been exposed to reinfection and again voided eggs in her urine.
She
had been negative after the 2. treatment. Reinfection may have happened. Patients could be controlled over a period of 6-14 months. Thus, Metrifonate seems to be an effective drug in the treatment of urinary schistosomiasis. Side-effects (nausea, abdominal pains, and--very rare--
vomiting
) were mild and disappeared spontaneously within less than 24 hours after medication. Patients did not have to interrupt school, daily activities, and treatment. Statistically, Metrifonate did not show any influence on transaminase SGOT, SGPT, and LDH during and after the course of treatment. The same evaluation applies to eosinophilia. There is no increase or decrease of this particular type of cell during and after treatment 7 patients showed alterations of their ECG curves. There were changes of the T-wave in V1-4. In adults traces were normal again several months after completion of treatment. It seems to be difficult to interpret ECGs in West African youngsters. X-ray photos of the lungs never revealed any pathological findings which could be connected to the course of treatment. Metrifonate seems to be a valuable drug in treatment of Schistosoma haematobium-infection. The drug is well tolerated if the treatment scheme--mentioned above--is used.

A report from a medium-sized community hospital of 4 additional cases of hepatocellular adenoma is presented. Of these, 3 were pedunculated tumors. A 29-year-old woman, gravida 7, had been taking Enovid-E for 8 years. After 3 attacks of epigastric pain a mass was detected in her right upper abdomen. Surgical exploration revealed the mass pedunculated from the right hepatic lobe. The mass was excised. 3 years later a liver scan was normal. The histological appearance of the tissue resembled mature hepatic architecture without protal areas or bile ducts. A 34-year-old woman, gravida 4, para 2, had been taking Enovid-E for 6 years.
She
previously had had a partial thyroidectomy for carcinoma of the thyroid and later radical neck dissection for metastatic thyroid carcinoma. A mass was detected in the right abdomen. At operation, a pedunculated mass was found attached to the right hepatic lobe with 2 smaller nodules. All lesions were excised. The microscopic picture was similar to that of the other patient. A 24-year-old woman, gravida 4, para 3, had been taking Oracon for 1 year. Upper abdominal pain,
vomiting
, and diarrhea were complained of. An upper abdominal mass was present. At operation, a 9 cm mass was excised. The histology was the same as in the previous cases. A 28-year-old woman, gravida 2, para 1, had been taking Ovulen for 5 years. An abdominal mass was present. Surgical exploration revealed a nodular mass pedunculated from the left lobe of the liver. The mass was excised. Histological appearance was similar to the other 3 tumors. Of 42 other reported cases of similar tumors, the mean age was 27.3 years. About 40% had been admitted as emergency cases with hemoperitoneum. Almost 20% had complained of upper abdominal pain. A palpable mass was usually present. Multiple nodules were found in 13% of cases. For nonemergency cases, selective hepatic angiography has been a reliable method of diagnosis. The treatment has been resection of the tumor when feasible. However, regression of tumors has occurred when exogenous hormone influence ceased. The outcome has been fatal in 8 cases. The low incidence of this tumor is not considered sufficient cause to justify discontinuing use of oral contraceptives.
