The antiemetic efficacy of metoclopramide and lorazepam (MTC + L) versus alizapride and lorazepam (ALZ + L) was compared in 100 patients receiving chemotherapy, in a prospective randomized double-blind study. In highly emetogenic (HE) regimen (including platinum) patients received MTC 1 mg/kg or ALZ 3 mg/kg x 4 doses, and lorazepam 2.5 mg 30 min before therapy. In moderately emetogenic (ME) regimen patients received MTC 0.5 mg/kg or ALZ 1.5 mg kg x 3 doses, and lorazepam 2.5 mg 30 min before therapy. In both HE and ME regimen groups there was no statistically significant difference between MTC + L and ALZ + L treatments as regards the number of vomiting episodes, the duration of emesis and nausea, the intensity of nausea and side effects, but a statistically significant difference between treatments was found in the HE group where MTC-L was superior to ALZ + L in obtaining complete protection from vomiting (37 vs 11%, p = 0.05). Idiopathic perforation of the bile duct is rare in children. Sixty-seven cases were reported in the English literature to 1980. It is, nevertheless, the second commonest surgical cause of jaundice in the neonate. The etiology is unknown though distal obstruction and weakness in the bile duct wall have been postulated. Limited surgical treatment with external drainage is the preferred therapy. In isolated cases internal drainage procedures or repeated aspiration have been successful. The diagnosis should be suspected in the presence of jaundice and ascites with or without abdominal pain and signs of peritoneal irritation. We describe a 3-month-old girl presenting with anemia, vomiting, jaundice, and ascites. This was initially diagnosed as hepatitis but bilious fluid was found on paracentesis. Computerized tomography with cholangiography and 99 MTC Diisopropyl IDA cholescintigraphy confirmed the diagnosis. The latter seems to be more accurate than I-131 Rose Bengal. The perforation was at the junction of the hepatic and cystic ducts. It was treated successfully by external drainage and a cholecystostomy. Direct attempts to close the perforation, or more complicated surgical procedures, are unnecessary while nonoperative treatment carries a high mortality. At follow-up after 1 year the IV cholangiogram and liver-function tests are normal. Cholecystostomy provided good drainage of the biliary ducts as well as easy access for follow-up cholangiography.

Migraine is a common medical condition affecting more than 10% of population in Poland. Acute migraine attacks are often associated with gastrointestinal symptoms ranging from mild nausea to vomiting. The present study was specifically designed to compare the efficacy of the combination of lysine acetylsalicylate and metoclopramide (1620 mg LAS + 10 mg MTC--Migpriv) and ergotamine plus caffeine (2 mg ERG + 200 mg CAF--Coffecorn forte) in the treatment of migraine headache and nausea. 98 patients with migraine meeting HIS criteria were randomized. The primary endpoint was the number of patients having headache relief and no nausea or vomiting two hours after the drug intake. Significantly more Migpriv treated patients than Coffecorn ones were free of headache two hours after drug administration (58.7% vs. 35.7% respectively: p<0.002). Reported adverse events were rare and transient. Migpriv was significantly more effective than Coffecorn forte in relieving the migraine attack symptoms.

Migraine is a common medical condition affecting nearly 10% of population in Poland. The aim of the study was to evaluate the efficacy, tolerability and safety of migraine abortive treatment with the combination of lysine acetylsalicylate and metoclopramide (1620 mg LAS + 10 mg MTC--Migpriv). 1620 patients with migraine, meeting International Headache Society (IHS) criteria were included in the study. The aim of the study was also determination of the number of patients having headache relief and no nausea or vomiting two hours after the drug intake. In most patients the migraine symptoms disappeared after a single dose administration. Reported adverse events were rare and short lasting. The study showed that metoclopramide was well tolerated and effective at relieving migraine attack symptoms.
