volvulus
of which, 141 cases concerning the small intestine, 2 of the transverse, 13 of the caecum and 80 of the sigmoid. Detailed discussions are made on each type of
volvulus
, the etiology, pathogenesis, pathological anatomy, while particular extensions is given to the description of the results obtained that are compared with the data found from the examination of the literature. A separate treatment is reserved to the anaesthesia in the cases of intestinal acute
volvulus
and particularly to some technical measures to which is to be submitted the patient who suffers from mechanical occlusion with serious intestinal ache.

Between April 1977 and June 1978, 214 babies born of 210 mothers infected with Onchocerciasis, were examined at the maternity of Po Hospital (Upper Volta). In four of them (1.9%) dermal microfilariae of Onchocerca
volvulus
were found during the first week after birth. In two other cases microfilariae were seen in the tissue of the umbilical cord. All specimens from placentas and amniotic fluid gave negative results. The infected babies were reexamined monthly monthly from 2 to 18 months, depending on the cases. One child remained positive until the 6th month follow-up. The base-line examination of 5.757 children less than 2 years old from different villages in the WHO Onchocerciasis Control Programme in the Volta River Basin Area showed that 1% of children less than one year old and 2% of children of 12 to 23 months old were carriers of dermal microfilariae. These results indicate that in meso- or hyperendemic areas children less than one year old and carriers of microfilariae of Onchocerca volbulus may have been infected in utero. The existence of such cases, in an area of vector control, does not prove a residual vector borne transmission.

Species identification of reinvading females. 2. Distribution of biting populations of S. damnosum complex species. 3. Identification of eggs and small larvae found within the OCP area. 4. Host preferences of species within the complex. 5. Vector potential of S. damnosum complex species for Onchocerca

and O. ochengi. 6. Provision of identified material for morphological investigation. The technique of indirect adult identification using reared larvae, may eventually by largely superseded by progress in morphological taxonomy.

in children are reported, one acute and one chronic. The diagnosis should be suspected as an unusual cause of chronic recurring upper abdominal pain or in acute gastric obstruction. The radiographic appearances of these patients are presented. Anterior gastropexy has produced good results in both the patients described.

Six drugs in common use for the treatment of parasitic infections of man were given to 18 adult patients suffering from onchocerciasis. None of the six (metronidazole, tinidazole, mebendazole, trichlorophone, oxamniquine and pyrantel pamoate) showed any evidence of substantial activity against the microfilariae or adult worms of O.

A technique of mesocoloplasty for the prevention of recurrence of sigmoid
volvulus
is described. The manoeuvre is simple, and does not open the gut. It can be performed confidently by junior doctors undergoing surgical training and appears to be an eminently safe technique as a routine procedure in any emergency service.

This is a review of 261 patients operated for 271 instances of mechanical intestinal obstruction over a 5-year period in a developing country in the tropics. The pattern of intestinal obstruction in Chinese is similar to that in Caucasians, where adhesions account for the largest number of cases. The occurrence in Malays, Indians, Pakistanis and Ceylonese is similar to that in other developing communities where external hernia is commonest while adhesive or tumour obstruction is rare; however, these racial groups do not exhibit the high incidence of intussusception and
volvulus
found in Africa and India. The operative mortality was 13-9 per cent, which is comparable to that in Western series. The major adverse factors in intestinal obstruction, i.e. extremes of age, associated disease, gangrenous bowel, large bowel obstruction and malignancy, were confirmed. Fluid and electrolyte imbalance was frequent, as in other tropical series, but with intensive preoperative correction it was not an important adverse factor.

Twenty-six cases of acute
volvulus
of the sigmoid in Khartoum Hospital are analysed. The condition accounts for 17.8 percent of all cases of acute intestinal obstruction. The majority of patients were from the Southern tribes whose diet contains a large amount of fibre. Of the different methods of surgical management simple operative detorsion and deflation gave the best results. The overall mortality rate was 23.1 percent.

Rabbits, pre-immunized by intravenous inoculations of live, or of freeze-killed microfilariae of Onchocerca
volvulus
, were later challenged by subconjunctival inoculation of live microfilariae. Those pre-immunized with live microfilariae showed a marked chemotic conjunctivitis and reactions in the cornea (stromal keratitis and limbal abscesses), starting within one day of challenge. They were classed as sensitized. Those pre-immunized with dead microfilariae produced minimal reactions also starting on day 1, and were classed as tolerant. Histologically the reactions were distinctly greater in sensitized than in tolerant animals. The predominant cells in the inflammatory exudate were polymorphonuclear leucocytes and lymphocytes. The immunological basis for the differences between sensitized and tolerant rabbits is discussed, together with its possible bearing on human onchocerciasis. The effects of long-term subconjunctival and sclerocorneal inoculation of microfilariae were compared in one sensitized, one tolerant, one previously exposed, and one sensitized control rabbit. The sensitized test animal showed lesions resembling the sclerosing keratitis of human onchocerciasis. Immature microfilariae taken from intra-nodular fluid showed little ability to penetrate the cornea of the rabbit, and were minimally pathogenic.
