Very little of the original primary forest remains in Sierra Leone and the savanna is mainly woodland or a forest-savanna mosaic. The prevalence of microfilariae of Onchocerca
volvulus
, nodules and moderate or severe skin lesions was higher in forest than savanna villages. In forest villages the prevalence of microfilariae was 71.8% at the iliac crest, 36.6% (outer canthus), 12.8% (cornea) and 34.1% in the anterior chamber of the eye. Corresponding figures for the savanna villages were 51.9%, 20.5%, 5.6% and 21.8%. The overall prevalence of nodules in the forest and savanna was 70.5% and 53.2% respectively, while the prevalence of head and upper body nodules was 14.8% (forest) and 11.0% (savanna). The prevalence of moderate or severe skin lesions was 17.7% in forest and 13.0% in savanna villages. Lesions of the groin and
scrotum
were few in both zones. In persons aged 30 years or more the prevalence rates of severe eye lesions--sclerosing keratitis, iritis, optic atrophy and choroidoretinitis--were 4.3%, 16.1%, 13.9% and 14.8% respectively in forest villages. Corresponding figures for the savanna villages were 3.7%, 8.7%, 14.2% and 11.3%. Males were more commonly affected than females. At least one of these lesions was found in 32% persons in forest and 24% in savanna villages.

A study was carried out in selected parts of the Niger Delta Basin on human filariasis and its relationship to clinical signs. One hundred and seventy-six (13.0%) of 1351 individuals examined were infected with microfilaria. Forty-six percent of microfilaria positive cases had M. perstans; 25.6% had W. bancrofti, 19.3% had L. loa, and 9.0% had O.
volvulus
. The prevalence of filariasis increased with age. Microfilarial density was generally low, 28.4% of infected individuals had counts of 50-100 microfilaria per 20 mm3 blood, 12.5% had counts less than five microfilaria per 20 mm3 blood. Onchocerca microfilariae did not exceed five microfilaria per snip. Microfilarial density was high among individuals aged between 20 and 49 years, but declined with increasing age above 50 years. Acute clinical signs of febrile attack, sink eruptions, and chronic clinical signs of chyluria, hydrocoele, elephantiasis and ocular lesions were observed in 61.9% of infected individuals, 73.4% of clinical cases were febrile attack and skin eruptions, 5.5% were chyluria, 12.8% were elephantiasis of the leg and
scrotum

We have experienced a case of shunt tube perforation into the transverse colon in which a Pudenz infantile V-A shunt tube was installed for a hydrocephalic baby. The shunt operation was performed at a year and one month of his age in this patient and bowel perforation was disclosed and corrected twenty months following the operation. From a retrospective point of view, however, slight sign of meningitis could be suspected two months earlier and judging from his symptoms of transient intracranial hypertension, the bowel perforation might be traced back even earlier. In the past twenty year history of V-P shunt maneuver, quite a number of papers on various kinds of specific complications have been reported. On reviewing these reported cases out of English and Japanese literature, numerous abdominal complications such as twenty-eight perforations, fifty CSF ascites or pseudocysts, three umbilical perforations, two vaginal perforations, five migrations into
scrotum
, and some
volvulus
and bladder perforations can be collected. These are summarized and discussed briefly in this paper.

In a cross-sectional epidemiological and clinical study of human filariasis, 630 individuals were examined for Onchocerca
volvulus
, Wuchereria bancrofti and Mansonella perstans infections in five communities in the Kaiyamba Chiefdom, Moyamba District, Sierra Leone. The overall prevalence of O.
volvulus
infection in males 144(39.1%) and females 94(35.9%) was not significantly different and the sex prevalence rate between communities was also not significant (G = 3, d.f. = 4, P > 0.05). Prevalence of O.
volvulus
was significantly lower (G = 42.331, d.f. = 5, P < 0.001) in the 5-9 age group (13.3%) compared to the 40-49 age group (61.9%). Sixty-four (10.2%) and 38(6.0%) of individuals examined were positive for W. bancrofti and M. perstans infections respectively and prevalence of both infections in the five communities was not significant. Mixed infections with the all three filaria parasites were recorded in 10(3.2%) of the individuals. One hundred and sixty-four (71.3%) clinical cases due to W. bancrofti were inflammatory in nature; 36.5% were chronic, of which, 26.6% were hydroceles and 9.4% involved elephantiasis of both the
scrotum
and the lower legs. All 19(3.0%) of M. perstans-related clinical cases were inflammatory. Ninety-three(63.3%) of O.
volvulus
positive individuals that presented symptoms were inflammatory in nature, 14(9.5%) had ocular symptoms and 57(38.8%) had subcutaneous nodules. These data indicate that infections due to O.

. In the newborn, a hydrocele and scrotal mass can be observed; the ultrasound will show a heterogeneous image with calcifications. Meconium periorchitis or meconium vaginalitis resolves spontaneously. The lack of awareness of this disease could lead to unnecessary surgery in the newborn. We present a 33 days old patient with a scrotal mass in whom surgery was performed with the pathological diagnosis of meconium periorchitis.
