Three patients had recurrences of left-sided colonic
volvulus
after allegedly total resections of redundant sigmoid loops. All three patients had chronic, acquired megacolon. Other instances of late recurrence of
volvulus
after sigmoidectomy are documented in the literature. Sigmoidectomy may be an inadequate operation for sigmoid
volvulus
in younger patients and/or in patients with severe chronic megacolon. The decision to perform an interval operation and the selection of the type of operation must take this into account.

A case of transverse colon
volvulus
is reported, bringing the total number of collected cases in the English language medical literature to 45. Although this type of
volvulus
is rare, a definite pattern can be appreciated. Patients tend to be young, female, and give a history of chronic or recurrent difficulty in having bowel movements. A triad of underlying factors predisposes to the development of the
volvulus
: a distal impediment (either organic or functional) to the evacuation of the bowel, a redundant bowel and mesocolon and a fixed point around which the bowel can twist. The best treatment is resection with either anastomosis or exteriorization, depending on bowel viability.

Colonoscopy was employed to attempt detorsion of
volvulus
of the right colon in four patients considered to be high-risk surgical patients. In one patient detorsion was accomplished, so that emergency laparotomy became unneccesary. The site of torsion was reached in every patient. When detorsion does not occur reasonably soon after the colonoscope enters the right colon, persistent efforts are more likely to be harmful than helpful. Abdominal symptoms in a patient who has been ill or injured for some time should suggest the possibility of
volvulus
of the right colon. Early diagnosis and earlier employment of colonoscopy would almost surely result in an increased rate of success in accomplishing detorsion. When detorsion is accomplished, subsequent resection of the right colon should be seriously considered as, without it,
volvulus
is said to recur frequently.

Etiologic and physiopathologic aspects of
volvulus
of the sigmoid colon in Brazil are presented. It is believed that sigmoidal
volvulus
in Brazil is a frequent complication of megacolon caused by Chagas' disease, differing in some characteristics from
volvulus
found in other countries. A review of 230 cases treated between 1938 and 1974 in the Surgical Department of Hospital das Clinicas, University of Sao Paulo School of Medicine, is presented. The successive variations used to treat this disease occurred parallel to those introduced in the surgical treatment of uncomplicated megacolon. From the results, the following treatment is recommended: endoscopic emptying in cases without clinical, roentgenographic or endoscopic signs of intestinal ischemia. Laparotomy should be performed when a complicated
volvulus
is suspected or when it is not possible to empty the loop. When a simple
volvulus
is found, the loop should be untwisted and the gaseous contents siphoned off by menas of a rectal catheter. When there is necrosis of the colon, the Hartmann operation is recommended. It is important to submit patients to a definitive treatment of the megacolon soon after endoscopic emptying or surgical detorsion of the
volvulus
, since recurrences following these measures are frequent.

Volvulus
of the sigmoid colon is a very uncommon cause of acute obstruction in children. Although common in adults in India, it was found to account for only 0.8 per cent of all acute obstructions in infants and children in this institution. It causes a proximal torsional obstruction of the colon with an acute onset of symptoms. The onset of
volvulus
is characterized by colicky pain over the left lower quadrant, vomiting, tenderness, and rigidity in te left lower quadrant. A scout film of the abdomen may be inconclusive, but a barium-enema examination is diagnostic. The number of cases reported is too small to allow conclusions about the best treatment for children who have sigmoidal
volvulus
.

We have reviewed the records of 48 patients who had colonic
volvulus
.
Volvulus
occurred in the sigmoid colon in 27 (56%) and in the right colon in 19 (40%).
Volvulus
elsewhere in the colon is rare, requiring unusual anatomic circumstances of a long mesentery and a mobile colon. The clinical history is characterized by a long history of bowel dysfunction followed by an episode of acute intestinal obstruction. The patient is often aged and is plagued by mental disorders and a number of degenerative diseases. Distention of the abdomen is the most significant finding, and tenderness may indicate peritonitis due to ischemic changes in the bowel. Three-positional films of the abdomen are most valuable, showing great distention of the colon and air-fluid levels in the bowel with regularity. Barium-enema studies will more accurately reveal the site and nature of obstruction. The barium-enema examination must be done carefully. It is omitted when peritonitis is present. Operative treatment is necessary for
volvulus
of the right colon. Non-operative reduction is effective for nonstrangulating
volvulus
of the sigmoid colon as an emergency procedure. Sigmoidoscopic examination and insertion of a long rubber tube will give dramatic relief to a substantial number of patients. Operative intervention is necessary when conservative measures fail. When gangrene is found at operation, exteriorization resection of the colon may be life-saving. Elective resections are recommended for patients who are in otherwise good health in order to prevent recurrences. The mortality rate in this series of 48 cases was 12.5 per cent. Cecal
volvulus
was present in each of the six patients who died. Sepsis and cardiopulmonary diseases were common in patients who died.

In a patient who had celic sprue intractable to a gluten-free diet,
volvulus
and jejunal and colonic ulcers developed. Following segmental colonic resection, these ulcers recurred at the site of the previous anastomosis, and proved a fatal complication because of colonic perforation. Since no obvious etiology of colonic ulcers could be identified in this case, it is possible that they may represent an unrecognized intestinal complication of celiac sprue similar to jejunoileal ulcers.

A patient with recurrent sigmoidal
volvulus
who was treated by elective sigmoidal resection is reported. The symptoms and radiologic findings are sufficient for a confident diagnosis to be made before the onset of a major obstructive epidose. The value of barium-enema examination even in the quiescent phase is emphasized. Definitive treatment by sigmoidal resection in the absence of obstruction is safe and should be undertaken as an elective measure. Awareness of the disease and its occurrence in young patients will aid its recognition and reduce mortality from intestinal obstruction.
