Small bowel diverticulosis represents an uncommon disorder (except for
Meckel diverticulum
) often misdiagnosed since it causes non-specific gastrointestinal symptoms. Most of times the diagnosis is carried out in case of related complications, such as diverticulitis, hemorrhage, perforation or obstruction. Intestinal obstruction can be caused by inflammatory stenosis due to repeated episodes of diverticulitis,
volvulus
, intussusception or jejunal stones. Herein we report a case of multiple jejunal diverticula causing chronic gastrointestinal obstruction.

Meckel's diverticulum
is the most common anomaly of the intestine. It is usually asymptomatic but approximately 4% are symptomatic with complications such as bleeding, intestinal obstruction, and inflammation. Gastrointestinal bleeding is the most common presenting symptoms of
Meckel's diverticulum
in children, however, intestinal obstruction is the most common complications in adult patients. Reported mechanism of intestinal obstruction in
Meckel's diverticulum
include intussusception, adhesion, and
volvulus
. Recently, we experienced a case with
Meckel's diverticulum
associated with ileal stricture causing recurrent partial intestinal obstruction in a 48-year-old man. In contrast to other published cases of small bowel obstruction in
Meckel's diverticulum
, this case was caused by ileal stricture associated with
Meckel's diverticulum
.

This is a case of an otherwise asymptomatic
Meckel's diverticulum
, which became fibrously adherent to a previous umbilical laparoscopic port site, causing
volvulus
and small bowel obstruction in a pediatric patient. The diverticulum was diagnosed and resected laparoscopically, remaining bowel viability was maintained, and the child recovered without further sequelae. This complication, though rare, should be considered in the differential diagnosis when a child presents with abdominal pain after undergoing previous laparoscopic surgery. More important, this supports the consideration for the resection of asymptomatic Meckel's diverticulm when discovered incidentally, which is currently a controversial topic.

This paper refers to 50 unusual cases of 542 consecutive adult patients who underwent surgery because of acute intestinal obstruction. Of the 38 small bowel cases, 5 were caused by hernias in anomalous recesses (1 prevesical, 2 left paraduodenal, and 2 paracecal hernias), 6 by a gallstone ileus, 14 to the presence of a bezoar or foreign body, 8 to extended postradiation perivisceritis, 3 to
Meckel diverticulum
volvulus
, 1 to transepiploic hernia, and 1 to ileus-Meckel hematoma during anticoagulation treatment. The 12 large bowel cases included 3 diaphragmatic hernias (1 late post-trauma), 3 cases of colo-colic intussusception, 1 case of obstructive cholecystitis, and 5 cases of Ogilvie's syndrome. Major technical problems have to be immediately solved in the case of left paraduodenal, prevesical, or diaphragmatic hernias; however, during laparotomy, there may also be some difficult and unpredictable problems caused by widespread postradiation perivisceritis.

Meckel diverticulum
is the most common congenital anomaly of the gastrointestinal tract. Its prevalence is approximately 2% of the general population. Complications because of Meckel diverticula are rare and almost nil beyond the age of 40 years. This report details the case of an 86-year-old patient with
Meckel diverticulum
that perforated because of a tumoral lesion at its apex, also known as
volvulus
of
Meckel diverticulum
.

Situs ambiguus is a rare lateralization anomaly that is frequently associated with other malformations, including preduodenal portal vein (PDPV), intestinal malrotation, and cardiovascular anomalies. This is a case report on a newborn that was clinically diagnosed with situs ambiguus and midgut
volvulus
. During surgery the patient was found to have intestinal malrotation,
Meckel's diverticulum
, and PDPV that was not a direct cause of duodenal obstruction. The patient was treated with Ladd's procedure and resection of
Meckel's diverticulum
. It is important to be familiar with the spectrum of situs anomalies to prevent trauma to the portal vein with serious complications during surgery.

Abdominal pain in child could be related to surgical pathologies in 10 to 20 % of cases. The more frequent etiology remains appendicitis. Regarding to clinical presentation, age and medical history, intestinal intussusception, incarcerated hernia, adhesive occlusion and
Meckel's diverticulum
could be evocated. But the most dreadful diagnosis is malrotation with
volvulus
, because of mortality and morbidities induced by bowel necrosis. Usually, medical history and clinical exam allowed diagnosis. Ultrasound remains the more helpful exam in children with surgical pathologies and in some selected cases, CT scan and others biological and/or radiological exams could be performed.

Meckel's diverticulum
is the most common congenital abnormality of the gastrointestinal tract. Most cases are asymptomatic; however, when symptomatic, it is often misdiagnosed at presentation. Common complications presenting in adults include bleeding, obstruction, diverticulitis, and perforation. Tumors within a
Meckel's diverticulum
are rare. Herein, we present a gastrointestinal stromal tumor arising from the
Meckel's diverticulum
that led to intestinal obstruction by
volvulus
.

Meckel diverticulum
is the most common congenital anomaly of the small intestine, occurring in about 2%-4% of the population.
Meckel diverticulum
results from incomplete closure of the omphalomesenteric duct. The presentation of symptomatic
Meckel diverticulum
includes gastrointestinal hemorrhage, intestinal obstruction,
volvulus
, intussusception, diverticulitis, and neoplasms. The development of fistula is an extremely rare complication. Previous literature has even shown an enterocolonic fistula, a vesicodiverticular fistula, ileorectal fistula, and fistula-in-ano. To the best of our knowledge, we present the first case of the fistula complicated between
Meckel diverticulum
and the appendix in a review of the English literature.

Meckel's diverticulum
has several known complications including diverticulitis and perforation. The presence of mesodiverticular band or a band from the diverticulum to the anterior abdominal wall is also described and can cause obstruction or rotation of the small bowel leading to
volvulus
.
Meckel's diverticulum
is also well known as the lead point for intussusception. It may be lined by ectopic gastric mucosa and can cause life-threatening gastrointestinal bleeding. We report a neonate who presented with acute intestinal obstruction secondary to a large, mobile
Meckel's diverticulum
which due to a direct compression effect on the adjacent small bowel caused mechanical intestinal obstruction. Diagnosis was confirmed at laparoscopy, and treated by curative surgical resection. This is the first report of a large mobile
Meckel's diverticulum
causing small bowel obstruction due to direct compression that was managed by minimally invasive surgical resection.
