We describe a patient with symptomatic multiple lipomas with volvulus. He complained of gradually increased abdominal pain and distention. Preoperative roentgenograms showed many rounded filling defects of the small intestine. Operation was performed to reduce intestinal obstruction and ileal resection was performed. The dilated ileum was rotated 720 degree clockwise and it contained 115 submucosal and 26 subserosal lipomas. Histologically, the lipomas were benign and consisted of well-developed fat tissue. The patient's postoperative course was satisfactory, and neither residual lipoma nor recurrence were found at barium enema examination two years after operation.

Four horses were evaluated for clinical signs consistent with small intestinal disease. During exploratory surgery, primary hematomas in the mesentery were found in 2 horses. The third horse developed a hematoma secondary to a mesenteric rent, whereas in the fourth horse, a hematoma developed secondary to strangulation of the bowel by a mesenteric lipoma. Two horses were euthanatized at surgery, because the extensive nature of the hematoma precluded ligation and there was substantial risk of continued hemorrhage postoperatively. The remaining horses recovered from surgery but developed complications. One horse developed functional ileus and required a second surgery to decompress the bowel, whereas the second horse eventually was euthanatized for severe, recurrent signs of colic. Clinical signs and management of small intestinal lesions related to volvulus, epiploic entrapment, intramural hematoma, mesenteric rent, and intussuception have been described. To our knowledge, reports of small intestinal infarctions secondary to hematomas in the mesentery have not been published. Primary hematomas in the mesentery should be included in the differential diagnoses for horses examined for signs of depression, anorexia, and progressive signs of colic.

Volvulus of the small bowel is not so frequent as is volvulus of the colon. A delayed diagnosis and surgical treatment result in high rate bowel infarction which can lead to perforation and stercoral peritonitis. If perforation does not take place, the infarcted bowel has to be resected causing multiple complications and mortality. The small bowel volvulus is caused by mesenteric lipoma in about 5% of cases. We present a 77-year-old man operated on for intestinal obstruction. The patient was admitted in a serious condition with a five-day history of abdominal pain and vomiting. On admission he vomited a small bowel content (miserere), he was dehydrated and with high blood urea and creatinine values. Plain X-ray showed a number of air fluid levels in the small bowel. At operation a small bowel volvulus caused by mesenteric lipoma (18 x 11 x 10 cm in diameter) with bowel infarction but without free perforation and peritonitis, was found. The tumour was removed together with 10 cm of resected bowel with end-to-end anastomosis. The recovery was uneventful. The patient is still symptom free.

The authors report a case of a bowel obstruction secondary to jejunal volvulus in relation with the torsion of a large pedunculated subserosal lipoma. To our knowledge, no such case has ever been reported. The differential diagnosis of intra-abdominal fatty tumors and differential diagnosis of such tumors in association with small bowel obstruction will be discussed. Small bowel volvulus secondary to torsion of a subserosal lipoma should be considered when CT shows a large encapsulated fatty tumor in a patient with suggestion of mechanical small bowel obstruction.

A 31-year-old man underwent a laparotomy for acute intestinal obstruction symptoms, which he had intermittently experienced for 14 years. The cause of the obstruction was due to a volvulus of the small bowel caused by a mesenteric lipoma. This is a rare finding, which is ideally diagnosed by computed tomography, with surgery the best and most highly recommended treatment. This particular presentation, to the best of our knowledge, has not yet been previously reported in the English language.

Mesenteric lipoma is a rare benign tumor of mature fat cells. Although generally asymptomatic, it occasionally causes abdominal pain, ileus, and small bowel volvulus, depending on its location and size. A definitive diagnosis can be made by pathological examination. Ultrasonography and abdominal computed tomography show this lesion as a well-defined, homogeneous mass with fat density surrounded by a thin capsule. Because of its rare etiologic origin, we report the case of a 7-year-old girl with a mass in the abdomen and ileus, found to be caused by a mesenteric lipoma.

Caecal volvulus is a well described but unusual condition. We report here, a case of caecal volvulus in a 53-year-old Caucasian woman associated with intussuscepted submucous lipoma of the ileocaecal region. The imaging and pathology are presented. Submucous lipoma of the ileocaecal region is uncommon but well described. It can be mistaken as carcinoma of the ascending colon on barium enema and on computed tomography scan. The combination with caecal volvulus is a rare occurrence.

We report on a 14-year-old girl with acute intestinal obstruction due to volvulus of the small bowel caused by a giant mesenteric lipoma. To the best of our knowledge, this presentation of a mesenteric lipoma is unknown in the previous paediatric literature.

Primary mesenteric lipoma is rare, with fewer than 50 cases described in English-language literature, and those causing bowel obstructions are even more uncommon. The long stalk of the lipoma that caused secondary volvulus and rapid ischemic change in our patient is worth reporting because of its rarity and distinctive picture in emergency abdominal computed tomography.
