An unusual case of
volvulus
of the transverse colon associated with
scleroderma
is presented. The morphologic changes in the large bowel due to progressive systemic sclerosis, resulting in pseudo-obstruction, are considered in the etiology and pathogenesis of colonic torsion.

Fifty out of 228 patients recorded on the U.K. Home Parenteral Nutrition Register have died. The earliest to die was at 10 days following the commencement of home parenteral nutrition (HPN), and the longest to die was after 5 1/2 years. Half of the patients who died, did so within 6 months of commencing HPN. Sixty % died of their underlying disease. Most patients with
scleroderma
or an underlying malignancy are dead within a year of commencing HPN. In contrast, patients with Crohn's disease or the short bowel syndrome due to
volvulus
do well. In only 14 patients was death attributable to the administration of HPN. In this group the main causes were septicemia, SVC thrombosis, and hepatic failure. Our study suggests that HPN should be used in patients with malignancy and
scleroderma
only in exceptional circumstances and that further work is necessary for the prevention of SVC thrombosis.

Colonic volvulus may infrequently occur in the transverse colon, and synchronous double
volvulus
is a rarely reported phenomenon in the literature. Additionally, intestinal
volvulus
is a rare but serious complication that has been reported in
scleroderma
and systemic lupus erythematosus (SLE) patients. We report a 26-year-old patient with a history of
scleroderma
-SLE overlap syndrome who presented with an acute abdomen. CT imaging revealed a transverse colon
volvulus
and a cecal bascule (cecal
volvulus
). To our knowledge, this is the first reported case of synchronous double
volvulus
of the transverse colon and cecum. Additionally, this is the second reported case of transverse colon
volvulus
occurring in a patient with
scleroderma
and the first case in a patient with
scleroderma
-SLE overlap syndrome.
