Between March and November 1976, 30 patients with sudden hearing loss (SHL) were treated with vasodilators and diatrizoate meglumine (
Hypaque
). Nine (30%) of the patients had a good response, 7 (23%) had a moderate response, and 14 (47%) had no response to treatment. Most of the patients who responded were treated within the first month and had no
vertigo
associated on their onset of SHL. Their hearing loss was also less than 90 dB for any of the frequencies tested. Using the criteria of SHL of less than one month's duration, no
vertigo
associated with onset, and a loss less than 90 dB, seven patients were treated with vasodilator plus
Hypaque
during the period between January and August 1977. Six (86%) of the seven patients had return of serviceable hearing.

During the 24 month period ending in December 1978, 31 patients with sudden idiopathic sensorineural hearing loss were treated with vasodilators and
Hypaque
. Fifteen patients met the treatment criteria of being treated within the first month following onset, no
vertigo
, and the hearing loss was not complete. Twelve patients (80%) had complete return of their hearing. Two patients (13%) had partial return of their hearing, and 1 patient (6%) had no response to treatment. Of the 16 patients treated who did not meet the criteria, 6 patients (38%) had complete return of their hearing. Eight patients (50%) had partial return of their hearing and 2 patients (12%) had no return of their hearing.

One hundred nine patients with idiopathic sudden hearing loss were treated with a "shotgun" regimen that included dextran, histamine,
Hypaque
, diuretics, steroids, vasodilators, and carbogen inhalation. Thirty-three patients received the entire protocol and 76 patients received most, but not all, of the protocol drugs. Some improvement of hearing (greater than 10 dB in pure-tone average) was seen in 52% of patients treated with the complete protocol and in 54% of patients who received the partial protocol. All patients were analyzed for potential prognostic indicators. Patients with thresholds at 8000 Hz better than at 4000 Hz fared better than the group as a whole.
Vertigo
at the time of onset of hearing loss was a sign of poor prognosis. There was no correlation between hearing improvement and the age of the patient or the sedimentation rate. Most importantly, there was no statistically significant difference in outcome between patients treated with the complete protocol and those who received only part of the protocol. Furthermore, when the effect of each drug was examined individually, there was no significant difference between those patients receiving and not receiving treatment. The results suggest that this "shotgun" approach for treatment of sudden hearing loss offers no better outcome than is reported in the literature for spontaneous recovery.

Postoperative complications resulting from bariatric surgery can lead to severe vitamin-deficiency states, such as Wernicke's encephalopathy (WE). We present a 29-year-old woman with BMI 41.7 with no history of alcoholism who developed acute WE after a gastric bypass for morbid obesity. After persistent vomiting for 2 weeks postoperatively, symptoms began with headache,

, diplopia, nystagmus, tingling and weakness in both upper and lower extremities, urinary incontinence, and memory loss to recent events. All investigations, including upper GI endoscopy,

meal and even MRI, were normal. A dramatic improvement occurred in 24 hrs after starting 100 mg thiamine infusion daily. We recommend that patients undergoing bariatric surgery should be started on thiamine supplementation once oral intake begins, because this case showed that postoperative acute WE can develop before 6 weeks, unlike other reports.
