This study was conducted to evaluate the relationship between the prevalence of subjective symptoms in workers using vibrating tools and the duration of chain saw operation and to examine whether the symptoms were relevant to factors other than the usage of vibrating tools. The statistical model of multivariate analysis was adapted to analyze individual data on the subjective symptoms of 317 chain saw operators. The obtained results were as follows: 1. In analysis of covariance, age-adjusted operating year in workers with peripheral circulatory, peripheral neurological and musculoskeletal disturbances was significantly longer than that without such disturbances. For these disturbances, partial regression coefficients of operating year were significantly high in multiple regression analysis. These results show that these disturbances and age-adjusted operating year are mutually closely related. 2. Multiple regression analysis showed that the partial regression coefficients of both operating year and age were low for nine symptoms, i.e. dulling sense of touch, joint pain, headache, dizziness and/or tinnitus,
profuse sweating
, discomfort of stomach, palpitation and/or dyspnea, hearing disturbance and lumbago. These results suggest that such symptoms were not related to either age or chain saw operation. 3. The results of principal component analysis were visualized in three-dimensional space in order to evaluate the relationships among the symptoms. The analysis showed that peripheral circulatory and neurological disturbances appeared independently and that general symptoms such as easy fatigability, headache, forgetfulness,
vertigo
and/or tinnitus, easy irritability, sleep disorder,
profuse sweating
, discomfort of stomach, palpitation and/or dyspnea, shoulder stiffness, hearing disturbance and lumbago consisted of four independent groups. Each group has no relationship with duration of chain saw operation. This suggests other harmful factors of the groups play a role in the prevalence of the symptoms.

Toxic reaction is the most common side-effect accompanying the local anesthetic administration. Toxic reaction symptoms may manifest as CNS symptoms or cardiovascular systems symptoms. Initially, the toxic reaction symptoms in CNS undergo the stage of stimulation, followed by the stage of depression. A case of toxic reaction to the usual dose of Cystocain DS for mandibular anesthesia in an 11-year-old girl is presented. The symptoms began with poor general condition, nausea, severe
vertigo
, pallor and
excessive perspiration
, followed by clonus-type muscular convulsions, with consciousness preserved. Upon hospitalization, convulsions were interrupted by i.v. administration of diazepam. Blood pressure returned to normal, circulation recovered and normal frequency and depth of breathing were resumed. To date, only one similar case of reaction to Cystocain DS was reported to the WHO. Mere possibility as well as seriousness and risk of the occurrence of such a situation require close therapist's observations of each patient receiving any type of local anesthetics.

In 12 out of 51 maladjusted diabetics treated with insulin chronic insulin overdosage could be established as the reason. Increasing insulin requirements, avid hunger,
profuse sweating
,
vertigo
and emotional lability as well as grossly variable glucosuria are evidence of insulin overdosage. Treatment consists of low reduction of daily insulin dosage.

Abnormal sensations in the throat (AST) are commonly encountered in general and otolaryngological practice, especially among middle aged women. These sensation may occur alone, or more often, in combination with the systemic symptoms, including those of menopause. We analyzed 26 Japanese women with a complaint of an abnormal sensation in the throat who were taking a synthetic sex hormone (Methermon-F). Multianalysis of variance (MANOVA) was used to evaluate the efficacy of the treatment using the quantitative scores of Kupperman's climacteric symptoms. The presence of the
excessive perspiration
and
vertigo
, insomnia, and the absence of fatigue were associated with clinical efficacy of the hormone, while the presence of hyposthenia and tingling sensation were associated with a poor response. Results suggested that the clinical efficacy of this hormone in treating middle-aged menopausal women with AST was related to the menopausal symptoms.

Episodic spontaneous hypothermia is an infrequent disorder, with unknown pathogenic mechanisms. A systemic cause or underlying brain lesion has not been found for the disease. We report four new patients, 3-9 years old, with episodic hypothermia lower than 35 degrees C, marked facial pallor, and absent shivering. The episodes could last a few hours or four days, and recurred once a week or every 2-3 months. Two patients also demonstrated bradycardia, mild hypertension, and somnolence during the events; in one of them,
profuse sweating
was also a feature, and all four presented with either headache, a periodic childhood syndrome, or both (recurrent abdominal pain, cyclic vomiting, or
vertigo
). Three patients reported a family history of migraine. Neurologic examination, endocrine function, and imaging studies were normal. Migraine prophylactic therapy was of moderate efficacy. Spontaneous resolution was observed in one patient. The clinical characteristics of the syndrome allow for its inclusion as a childhood periodic syndrome related to migraine.
