Toremifene was given within the dose range of 3-680 mg as a single dose or on five consecutive days to 72 postmenopausal volunteers. Blood samples for clinical chemistry were taken hourly up to 7 h and 1, 2, 3, 7, 10 and 15 days after the last dose of toremifene. The concentrations of serum bilirubin, creatinine, amylase, free thyroxine, cortisol, prolactin, electrolytes and blood glucose remained unchanged at all dose levels. A statistically significant decrease was observed in liver enzymes (ASAT, ALAT, ALP) at the dose levels of 220-680 mg, whereas gamma-GT remained unchanged. A decrease in the concentration of LH and FSH was observed at the dose levels of 46 mg or higher and 220 mg or higher, respectively. These hormonal changes including the increase of SHBG at the dose levels of 220-680 mg and the decrease of antithrombin III (220-680 mg) may be attributed to a weak estrogen-like effect of toremifene. Side effects were minimal: pulse rate, blood pressure and ECG remained unchanged during the test period. Only two patients on 680 mg dose suffered from nausea and vertigo, and one of them discontinued the medication.

Myocardial infarction is very rare in women under the age of 40 amounting to only .3-.57% of cases according to Polish studies, and it is 10 times less in women than in men. Nevertheless, the use of contraceptives has been implicated in triggering it by diminishing the synthesis of antithrombin III. The use of alcohol can cause the reduction of the fibrinolytic activity of the blood. The case of a 33- year old female patient, a laborer in a brewery is presented, who sought medical help from the company medical staff after 2 weeks of generalized weakness, vertigo, chest pain, and loss of breath under strain. Physical labor had to be discontinued. EKG (electrocardiogram) showed extensive primary and secondary myocardial infarction in progress. She had been taking contraceptives for 3 weeks (1 pill of Femigen once a day). She was drinking 50 g of alcohol daily in the form of 1 liter of beer. She did not smoke, and no circulatory disorder was ascertained. Laboratory tests showed no disorder of aminotransferase or LDH activity, but fibrinogen level (23.5 mcmol/1), antithrombin III level (.124 g/l), and fibrinolysis time (320 minutes) values were abnormal. There was no disorder in the lipid and carbohydrate metabolism, and thyroid hormone level was normal. traditional therapy was prescribed for 2-3 weeks of the infraction: nitrates, rest and avoidance of physical exertion. After administration of castor oil, 2 weeks late an EKG test showed the normalization of all parameters of blood coagulation, and the HDL cholesterol level also dropped. The concomitant use of contraceptives and alcohol can increase the risk of myocardial infraction, therefore they should not be used together, but if such a situation occurs, the control of blood coagulation parameters must be undertaken.

Tamoxifen is a nonsteroidal anti-estrogen frequently used in breast cancer therapy. Side effects to tamoxifen are uncommon (2%) but should be recognized and detected early by careful follow-up. Tamoxifen adjuvant therapy is absolutely indicated in postmenopausal breast cancer with estrogen-receptor--positive nodes. Recently, this indication has been extended to negative-node postmenopausal breast cancer. Mild acute side effects are the most frequent: hot flushes, menstrual irregularity, nausea, headache, vertigo, minimal modifications in blood cell counts. However, more serious accidents can occur. Increased risk of thromboembolism is linked to a fall in the level of antithrombin III. Ocular toxicity can occur. If such ocular lesions are diagnosed early enough, they can be cured by promptly withdrawing treatment. For patients given tamoxifen, there appears to be a small increase in risk of endometrial carcinoma, especially if the daily dose is > 30 mg. This over-risk requires adequate detection based on sufficient knowledge of the usual tamoxifen-related modifications in the endometrium. Physicians should also be aware of two favorable effects. Tamoxifen therapy leads to decreased cardiovascular morbidity and mortality in postmenopausal women and is associated with a significant increase in lumbar bone density. Risk of interaction with oral anticoagulants has been reported. We discuss here practical steps in the follow-up of women treated with tamoxifen.
