To date, only 21 cases of bilateral absence of the internal carotid artery have been reported. The authors report a case of bilateral internal carotid artery agenesis combined with corpus callosum hypogenesis and cavum vergae. A 47-year-old man was admitted to our hospital complaining of occasional vertigo. Physical and neurological findings were normal except for severe mental retardation. Bilateral carotid angiograms showed the absence of both internal carotid arteries. Left vertebral angiography demonstrated filling of the right and left anterior and middle cerebral arteries by the posterior communicating artery. The ophthalmic arteries were opacified from the external carotid artery via the middle meningeal artery. A bone-targeted CT scan demonstrated the absence of the bony carotid canals bilaterally, and an MRI study, revealed that the corpus callosum was hypogenetic, and cavum vergae was observed.
