IgG and IgE against salivary gland proteins of bedbug (Cimex lectularius) were assessed in comparison with mosquito (Culex pipiens) and flea (Pulex irritans) antigens in the sera of papular urticaria patients (group I), siblings without papular urticaria (group IIa), patients' parents (group IIb), and healthy controls (group III) (Immunoblotting). Anti-C. lectularius IgG was significantly recognized at 66 and 10 kDa in 40% of group I, besides others ranging from 45 to 107 kDa. Group IIa significantly reacted with 70 kDa (57.1%). Group IIb reacted with 21 and 8.5 kDa (26.7%). Sixty percent of group IIb and 100% of group III significantly identified a band of 12.5 kDa. IgG against C. pipiens was significantly recognized at a range of 18-105 kDa in group I, IIb (115, 7 kDa), and III (58, 50 kDa). Anti-P. irritans IgG was significantly recognized by group I (100, 70 kDa) and group IIa (60, 35 kDa). IgE response was confined to C. pipiens at 115 and 54 kDa in groups I and III, respectively, besides 68 and 58 kDa in group IIa. It is concluded that IgG is present against C. lectularius, C. pipiens, and P. irritans in papular urticaria and may contribute to its pathogenesis.

A 55-year-old herpetologist developed rhinitis, asthma, urticaria and anaphylaxis when handling 4 different viper snake venoms. Allergen characterizations were done using SDS-PAGE, IgE immunoblotting and IgE inhibition experiments. The most prominent immunoreactive proteins were analyzed by MALDI-TOF mass spectrometry, and peptide identity was demonstrated by homology with known peptide sequences. SDS-PAGE showed several protein bands ranging from 5 to 99 kDa in each of the 4 snake venoms. Immunoblotting demonstrated 4 IgE-binding bands in the Bothrops extract of about 60, 28, 14 and 7 kDa. The bands of 28 and 14 kDa were also present in Lachesis muta. Two IgE-binding proteins of about 50 and 35 kDa were found in Bothrops atrox and L. muta, respectively. A strong inhibition of IgE binding to immobilize Bothrops asper proteins was observed after preabsorption of sera with B. asper, B. atrox,Bothrops xanthograma and L. muta extracts. MALDI-TOF analysis showed a 14-kDa phospholipase and the 60- and 28-kDa proteins showed significant similarity with metalloproteinases. In this report we have characterized the snake venom allergens that can elicit IgE-mediated symptoms.

Human seminal plasma allergy is a rare phenomenon. Its clinical manifestations are diverse, and range from mild local pruritus to fatal anaphylaxis. Treatment varies with severity of the reactions: abstinence, condom usage or immunotherapy (subcutaneous or intravaginal) with seminal fluid. Local allergic reactions can be managed by prophylactic use of antihistamines or local cromolyn cream. A 33-year-old female visited the Asthma and Allergy Clinic in Seoul National University Bundang Hospital for the recurrent generalized urticarial reactions after sexual intercourse. She had been suffering from asthma, allergic rhinoconjunctivitis and atopic dermatitis for 10 years. She gave birth to a baby 6 months ago and no problem before. However, recently she began to recognize unexpected generalized urticaria that occurred after the sexual intercourse with husband. She wanted to have the second baby but hesitated because of the recurrent symptoms after the intercourse. She showed positive response to skin prick test with her husband's seminal fluid. The IgE-binding components were 15, 22, 28, and 35 kDa. Considering her moderate cutaneous reactions, we decided to try prophylactic treatments with oral anti-histamine one hour before sexual intercourse. She did not experience urticarial reactions with intercourse while oral anti-histamine was administered in advance. Finally, treatment outcome was successful, and the couple successfully gave birth to their second baby. We suppose that prophylactic antihistamine may be also applied in seminal plasma allergy patients if systemic reactions are limited to mild to moderate generalized urticaria.
