A 20-year-old woman was referred for evaluation after about 2 years of recurrent episodes of localized urticaria during handling of several kinds of raw fish in a sushi shop, where she had worked part-time for 2 years. She had also experienced allergic symptoms such as itching and swelling of her lips, generalized urticaria, laryngeal tightness, stridor and dyspnea immediately after ingestion of raw and cooked seafood, including sole, horse mackerel, sea eel and shellfish, over the previous 1 year before referral. Skin prick tests and blood test for specific IgE antibodies were positive for many kinds of seafood, including sole, horse mackerel, sea eel, eel, crab, and abalone, which belonged to different taxonomic phyla, including Chordata, Arthropoda, and Mollusca. A challenge with a piece of broiled sole induced swelling of the lips, obstruction of the larynx, difficulty with deglutition, and abdominal pain. In addition, serum-specific IgE antibodies to two major fish allergens, parvalbumin and collagen, were detected by ELISA, suggesting that allergic symptoms could be induced by many kinds of seafood in the present patient. She was therefore diagnosed with occupational contact urticaria and oral allergy syndrome due to seafood. At the time of this report, the present patient had been followed for one year and no reactions have occurred since she started to avoid the causative types of seafood.

A case of cat-pork syndrome with subsequent follow-up in a 17-year-old male patient is reported. At the initial observation, the patient was sensitized to cat epithelium--along with house dust mites and grass pollen--from two years. In 2001 he had an immediate reaction with urticaria, angioedema and dyspnea after eating grilled meat and sausage, and skin tests and CAP/RAST revealed a sensitization to pork meat, with a value of 4.7 KU/L for pork meat, and of 55 KU/L for cat epithelium. The patient was followed up for three years with annual repetition of diagnostic tests. The elimination of pork meat from the diet was incomplete, with slight skin reactions to small amounts of cooked pork meat but tolerance to seasoned pork products such as salami. A challenge test with pork meat in 2004 was positive, with angioedema and asthma symptoms, and CAP/RAST showed a value of 43 KU/L for cat epithelium and 4 KU/L for pork meat. RAST inhibition confirmed the significant cross-reactivity between the two allergen sources. These findings provide some knowledge on the natural history of the cat-pork syndrome, and confirm that very prolonged avoidance of the offending foods are needed to expect a loss of sensitization.

A 35-year-old Chinese man was administered praziquantel for clonorchiasis, and 30 minutes later he experienced a sudden onset of itching urticaria over his entire body with dyspnea, palpitation, and dizziness. The patient had no recent medication history except for the praziquantel. He was diagnosed as having an anaphylactic reaction with skin eruptions induced by praziquantel. His symptoms subsided after a stomach wash and the administration of anti-anaphylactic medication in emergency department of a regional hospital in the People's Republic of China. The immediate type of hypersensitive reaction to praziquantel is plausible, and this reaction should be considered for praziquantel therapy.

Brazil nuts are the second most frequent cause of nut allergy in the United Kingdom. We report the case of a 20-year-old woman with documented Brazil nut allergy who developed widespread urticaria and mild dyspnea after intercourse with her boyfriend who had earlier consumed Brazil nuts. Skin prick testing with the boyfriend's semen after Brazil nut consumption confirmed significant reactivity whereas a sample before nut consumption was negative. We believe this to be the first case of a sexually transmitted allergic reaction.

There are very few reports on allergic reactions to lychee fruit in the literature. We describe the case of a 26-year-old man who developed pruritus, generalized urticaria, and severe angioedema of his lips and tongue with dyspnea within 15 minutes after lychee fruit intake. Although we found no lychee-specific immunoglobulin E antibodies, a basophil activation test (BAT) and a cellular antigen stimulation test (CAST) to lychee were both positive, as was a prick-to-prick test with fresh lychee fruit. The patient also suffered from an oral food allergy syndrome to parsley and was sensitized to mugwort but not to latex or profilin. BAT and CAST are helpful tools in the diagnostic workup for exotic food allergy. Mugwort is suggested as the allergen responsible for,the cross-reactivity presented by this patient, as he had no sensitization to latex or profilin.

Although the etiology of eosinophilic cholecystitis is still obscure, the postulated causes include allergies, parasites, hypereosinophilic syndrome, and eosinophilic gastroenteritis. It is sometimes accompanied by several complications, but a simultaneous onset with pericarditis is very rare. A 28-year-old woman complained of acute right hypocondrial pain and dyspnea associated with systemic eruption. Several imaging modalities revealed acute cholecystitis and pericarditis with massive pericardial effusion. A marked peripheral blood eosinophilia was observed, and the eruption was diagnosed as urticaria. Her serum had a high titer of antibody against Ascaris lumbricoides. Treatment with albendazole drastically improved all clinical manifestations along with normalization of the imaging features and eosinophilia. We report herein a rare case of simultaneous onset of acute cholecystitis and pericarditis associated with a marked eosinophilia caused by parasitic infection.

Hypersensitivity reactions (HSR) to oxaliplatin in patients with colorectal cancer include facial flushing, erythema, pruritus, fever, tachycardia, dyspnea, tongue swelling, rash/hives, headache, chills, weakness, vomiting, burning sensations, dizziness, and edema. We report a patient with fever as the sole manifestation of initial HSR, review the literature and discuss the management of HSR. A 57-year-old female with T3N2M0 rectal adenocarcinoma received modified FOLFOX-6. She tolerated the first 8 cycles without any toxicities except grade 1 peripheral neuropathy and nausea. During 9th and 10th infusions, she developed fever to a maximum of 38.3 centigrade with stable hemodynamic status despite medications. During 11th infusion, she developed grade 3 HSR consisting of symptomatic bronchospasm, hypotension, nausea, vomiting, cough, and fever. On examination, she was pale, cyanotic, with a temperature of 38.8 centigrade, BP dropped to 95/43 mm Hg, pulse of 116/min and O(2) saturation of 88%-91%. She was hospitalized for management and recovered in 24 h. Fever alone is not a usual symptom of oxaliplatin HSR. It may be indicative that the patient may develop serious reactions subsequently, as did our patient who developed hypotension with the third challenge. Treatment and prevention consists of slowing the infusion rate, use of steroids and antagonists of Type 1 and 2 histamine receptor antagonists, whereas desensitization could help to provide the small number of patients who experience severe HSR with the ability to further receive an effective therapy for their colorectal cancer.

Insulin allergy in patients with diabetes mellitus on insulin treatment is a rare condition. It is suspected upon noticing immediate symptoms following insulin injections. The immediate vital implications for the patient call for prompt diagnosis and management of insulin allergy. We review current knowledge and procedures based on four diabetic patients who presented in our clinic. Insulin allergy was suspected as they showed immediate symptoms after insulin injection (urticaria, rash, angioedema, hypotension, dyspnea). A detailed allergologic work-up was performed and adequate therapy was initiated. In three of the four patients, a specific immunotherapy was started whereas in one patient a switch to oral antidiabetics was possible and consequently initiated. By standard prick testing and measurement of specific IgE antibodies, a type 1 IgE-mediated allergy was confirmed. After initiation of insulin immunotherapy, the symptoms completely resolved in two out three of patients and significantly improved in the third patient. The fourth patient was successfully switched to oral antidiabetics. Insulin allergy is a rare but severe condition that calls for immediate allergological work-up. It can be managed well in close cooperation between the diabetologist and the allergologist. Specific immunotherapy is efficient and should be considered.

We herein describe a 33-year-old female who recurrently exhibited urticaria accompanied by vomiting, diarrhea and dyspnea after taking red-colored food. From her history, we suspected the cochineal dye, the commonly used natural red dye in red-colored food and beverage, to be the cause of her symptoms. Oral provocation test using cochineal dye-stained red-colored boiled-fish-paste induced urticaria and respiratory symptoms. Furthermore the prick tests and the scratch tests with cochineal dye and carminic acid, the major ingredient of cochineal dye, were also positive. These results indicate that type 1 allergy to cochineal dye caused urticaria in this patient. Thereafter, she avoided the foods containing a cochineal dye and showed a complete clinical remission. Recently, the number of literatures described about increased incidence of type 1 allergy to cochineal dye. As the usage of cochineal dye is increasing in the Japanese market, we should keep in mind that cochineal dye can be a cause of urticaria in daily practice.
