Clinical and laboratory data were extracted from the medical records of 285 consecutive children after completion of their evaluations for suspected atopic disease. Blood eosinophilia, determined from a routine complete blood count, was present in more than 60% of patients tested. Nasal smear for eosinophils or a direct eosinophil count provided little additional evidence of atopic reactivity in simultaneously tested patients. Total serum IgE concentration was elevated in 75% to 81% of the children with asthma, nasal allergy and atopic dermatitis and an elevated serum IgE concentration or blood eosinophilia, or both, was noted in 85% of the patients studied.

was documented in three children and hyperimmunoglobulinemia was found in 20 others. Serum complement activity was normal in 10 children with persistent

The serum concentrations of "pregnancy-associated" alpha 2-glycoprotein (alpha 2-PAG) were measured in 129 healthy women and 141 healthy men to establish a normal range, using a sensitive enzyme linked immunosorbent assay. In the normal population 2.8% of men and 5.4% of women had low serum alpha 2-PAG concentrations. Low concentrations occur, however, much more commonly in patients, particularly male patients, with certain diseases, including dermatitis herpetiformis (three of 12 or 25%) and
urticaria
(two of five or 40%). One female patient with absolute deficiency was also identified. In view of the recently confirmed association of alpha 2-PAG with IgA and the fact that alpha 2-PAG seems to have immunosuppressive properties, it seems likely that deficiency of alpha 2-PAG could result in the subject becoming sensitised to various dietary antigens. Interestingly, none of the 24 patients with
IgA deficiency
showed concomitant deficiency of alpha 2-PAG.

A 61-year-old woman presented with circumscribed eczematous eruptions with maceration, erosions and patchy infiltration in the perineum and inframammary regions. A diagnosis of eosinophilic granuloma (cutaneous histiocytosis X) was established. T lymphocytes from a skin biopsy were grown in vitro for three weeks after which chromosomal studies revealed a break or gap at chromosome 16q22 in 15% of the lymphocytes. The addition of alpha-interferon increased the percentage of affected cells to 28%. T lymphocytes from the patient's blood did not show the defect. The biological significance of the chromosomal defect is uncertain. It has been described before in healthy persons, malignant lymphoma, cold
urticaria
and
IgA deficiency
, and mental retardation. It has not been seen in patients with eosinophilic granuloma.
