Of 4087 women (aged 16 to 44 years old; 218 primigravida) subjected to induced abortion during the last 3 years, 762 (18.6%) developed complications. The complications were divided into three groups: group 1 included 64 women who developed complication within the first 7 days after abortion, group 2 included 46 women who developed complications within 7-30 days after abortion, and group 3 included 652 women with late complications. Of 64 women in group 1, 7 had perforation of the
uterus
, 28 had hematometra, and 29 has incomplete abortion. All 46 women in group 2 has endometritis. Of 652 women in group 3, 4 has placental polyps, 232 has inflammation with associated secondary infertility (hysterosalpingography indicated tubal obstruction in 194 and peritubal adhesions in 38), and 416 had disorders of the menstrual function. In addition to general clinical examination, 50 women with inflammation and tubal infertility underwent immunological examination and allergy testing. Of 50 patients, 35 were found to have skin rush,
hives
, drug or food hypersensitivity. The patients with allergy manifestations had marked inhibition of cellular immunity seen as decrease in T-lymphocyte count and 2 items increase in null lymphocyte count (B-lymphocyte count did not differ from that in healthy controls). The patients with tubal infertility received combined treatment. Conservative treatment including hydrotherapy, anti-inflammatory agents and electric stimulation was effective 123 of 232 women.

A 15 year old female with
uterus
bicornis bicollis was admitted for operation. She had a history of atopic dermatitis and allergy to buckwheat, raw egg and latex. Two months previously she had developed whole body flushing during dental treatment, and latex glove used by the dentist had been suspected as the cause. Prior to the operation she underwent internal examination and intrauterine echogram in which a latex glove was carelessly used by another gynecologist who had not confirmed her past history. After 30 minutes, dyspnea and
urticaria
without itching, appeared suddenly. Blood pressure decreased to 80/50 mmHg and heart rate increased to 120 beats.min-1. She was then transferred to our ICU. Methylprednisolone was administered intravenously for dyspnea and circulatory collapse. After 3 hours, the patient made an uneventful recovery. The increased plasma latex protein-specific IgE levels confirmed anaphylaxis to latex. The increasing incidence of potentially life-threatening allergic reactions to latex has caused mounting concern over recent years. We may suspect latex allergy when an anaphylaxic reaction or shock of unknown origin occurs. In hospitals, latex free products must be prepared for use with latex allergic patients and for protection of medical staff with this allergy.

The sexually transmitted disease (STD), chancroid, is the greatest factor for HIV infections in Africa like syphilis is in the US. 3 physicians suggest that reducing the incidence of STDs may reduce the spread of HIV. Risk factors for HIV include current or history of STD in women and bisexual men, pelvic inflammatory disease, semen, copper releasing IUDs, contraceptive dermatitis, malnutrition/food allergy, environmental pollutants, genetic make up, and prostitutes. HIV infected persons should use condoms to not only protect partners but to prevent repeated contact with HIV which influences the clinical outcome. Condom use for contraception is not widely practiced in some areas, however, including Central Africa and Haiti. Condom use has increased in the US because IUDs have been removed from the market, fear of HIV infection, and discontinued use of oral contraceptives in older women.
Urticarial
reactions secondary to a copper IUD often occur in adolescent women, but clears when the IUD is removed. Traces of nickel in the copper wire used in IUDs often induce an allergic reaction. Allergic reactions are cofactors of HIV which can be made worse if coupled with excessive menstrual bleeding and HIV infected semen cells entering the
uterus
via the IUD tail. Many countries have integrated family planning services with other public health services, such as STD clinics that address AIDS. Integrated services should provide STD services and contraception and involve males and be accessible to them. Comprehensive school based clinical model should be implemented into schools and colleges. Counselors should advice HIV infected women not to have any more children. These women should get top priority to family planning services. HIV antibody testing for women should be done at any center where women may be including family planning centers and prisons.
