Lichens are abundant in forests, living on trees, soil, stones and rocks. They contain usnic acid and other lichen acids that are contact allergens. Lichens and liverworts cause woodcutter's dermatitis, eczema that appears in the forest on the bare skin areas, especially in cold and wet weather. Occupational allergic contact dermatitis from lichens occurs in forestry and horticultural workers and in lichen pickers. Lichens can cause immediate allergy, contact urticaria, rhinitis and asthma and probably also photoallergic contact dermatitis. Lichens are used for the manufacture of oak moss absolute, a fragrance constituent. Oak moss absolute contains lichen acids and is one of the commonest contact allergens. Lichen acid allergy develops either from contact with lichens or from fragrances. We describe 4 cases of occupational allergic contact dermatitis from lichens during the past decade: 2 were farmers and 2 gardeners. 3 of them had allergic reactions to fragrance mix and oak moss absolute. Lichen contact allergy is an old, partly forgotten, syndrome that should be remembered for symptoms in contact with barked wood or wood dust.

Hyperreactivity to environmental factors is objectively expressed as respiratory, cutaneous and gastrointestinal disease. Approaching the diagnosis of an occupational disorder, a practical distinction was made between toxicity and allergy. The study of occupational allergic disease included particular procedures that led to standardized models and concepts. The contribution to the improvement of medical knowledge is reviewed according to selected experiences. The diagnostic aspects of asthma, rhinitis, dermatitis and urticaria are considered as regards to methodology of assessment of the occupational etiology with attention to demonstrative examples, which are worthwhile for the general medicine, too. Basic steps of risk agent identification, exposure assessment, threshold dose response measurement, allergen challenges and interaction are the original contribution of the occupational medicine to the diagnosis of allergic disorder. The clinical picture of asthma was clarified by the results of the specific bronchial provocation test, proving the important role of di-isocyanates and metal salts. Occupational rhinitis showed to be connected to asthma with predictive aspects in bakers' disease. Occupational dermatitis was linked to the development of experimental patch tests. Occupational urticaria included the concept of airborne contact allergy or nickel interactive food and occupational sensitivity. Occupational allergic diseases are emerging as a consequence of low environmental exposure, but they were remarkably studied in the past either for pathogenesis or for diagnostic procedures. Methods and acquisition are available also for the general medicine when an individual's specific reactivity is under investigation.

Occupational dermatitis to personal protective equipment (PPE), particularly to gloves, mainly occurs in healthcare workers. They are all irritant and/or allergic contact dermatitis (eczema and contact urticaria). Prolonged glove wearing by healthcare workers favours skin irritation of the hands and wrists. It is very important to consider characteristics and materials of gloves used in the medical field. Rubber additives are the main allergens in gloves. Latex or natural rubber remains by far the most frequent cause for occupational contact urticaria from gloves. The problem of prevention of infections risks in the health environment and the choice of medico-surgical gloves is described. In a surgical environment (in ORS), double gloving is recommended. Today, it appears as the best protection, even if in France it is far from being systematically used. Choosing the appropriate medical or surgical gloves requires having sufficient preliminary information on the assets, drawbacks and use limits of each of them. In cases of known contact allergic dermatitis, advice from dermatologists or allergologists is essential when it comes to suggesting substitution gloves.
