Study of ACTH responsiveness to oral metyrapone and insulin hypoglycemia in children with repetitive nervous system manifestations (convulsions, coma, mental confusion apathy, tremor) has led to diagnosis of isolated ACTH deficiency in nine children within a three year period. Hypoglycemia was ascertained in five children; in four cases no hypoglycemia was proved, possibly because of promptly disappearance or because of other mechanisms accounting for clinical symptoms (occurrence of intracellular overhydratation associated with corticol deficiency is considered). The incidence of isolated ACTH deficiency in children is possibly undervalued. Reappraisal of isolated ACTH deficiency in childhood as to be considered in idiopathic spontaneous hypoglycemia and perhaps in some paroxysmal neurologic and/or digestive manifestations without proved hypoglycemia and so far poorly defined or held for epileptic fits. In order to disclose further additional tropic hormone deficiencies, and to differentiate permanent from transient impairment of ACTH responsiveness which often seems to be related to emotional deprivation syndrome, more protracted follow up studies are needed.

Tiapride is the only medication which may have a favorable effect simultaneously on the general behaviour, the tremor, and the digestive symptoms, of alcoholics. This study was carried out in 22 alcoholic patients. Each received, either one intramuscular injection of 100 mg or 200 to 300 mg orally, per day. In eleven cases results were very satisfactory and in nine improvement was very noticeable. The treatment failed in two cases. Anxiety, which was present in 20 cases, disappeared in 18. Tremor was observed in 8 patients and disappeared in all. Tolerance was excellent in all cases. No sedative side effects were observed. This is particularly important for these patients who were continuing their professional activities.

The authors report two computed tomographic observations of Fahr's disease without disturbances in calcium-phosphorus metabolism; the first case is a by chance discovery and the second one is characterized by a cerebellar tremor. From the literature data they discuss successively clinical, radiological and computed tomographic, neuropathological, aetiopathogenetic and therapeutic aspects actually presented by strio-pallido-dentate calcifications (SPD) or Fahr's disease.

One hundred patients, aged between 60 and 92 years, were treated with tiapride for neurological disorders (abnormal movements, buccofacial dyskinesias, dopa therapy complications, ballism, eyelid tics, senile tremor, post-traumatic headache, delirium tremens), psychiatric disorders with more or less marked agitation and of various types (hysteria, depression, mood disturbances, hypochondria, delusions, hallucinations), or for mental deficiency, senile dementia, or arteriopathic dementia. Results were excellent, being satisfactory in 70 p. cent, and even more marked in some groups. Tolerance was very good, with some rare cases of somnolence. The efficacy and safety of tiapride makes it of particular value for treating neuropsychiatric disorders in geriatric patients.

Tiapride was used in 55 chronic alcoholics. It has a sedative effect on the anxiety, aggressiveness and agitation observed during the alcohol withdrawal syndrome. It is also effective against tremor, insomnia and fatigue. Fatigue or depression do not occur as side-effects. Tiapride induces a psychological feeling of wellbeing which is heightened by continuation of detoxication and general management.

The effectiveness of tiapride on psychic disorders was studied in 30 chronic alcoholics by assessing four parameters: sleep disturbances, mood disorders, conduct disturbances, and tremor. In a daily dosage of 600 to 900 mg tiapride proved particularly effective on insomnia, anxiety, passivity and tremor, without adverse side-effects. This drug seems useful for controlling psychic disorders in alcoholics and for motivating acceptance of a detoxification program.

The neuropsychiatric manifestations of alcoholism can be amended by neutralizing the somatic effects of alcohol on nervous centers. Tiapride acts electively on the mesolimbic area. Promising results have been obtained with tiapride in the various clinical forms of alcohol intoxication. Sixty patients (40 men and 20 women) were given tiapride for abnormal symptoms due to alcohol. Tolerance was good: no side-effects were recorded, with the exception of extrapyramidal manifestations in one patient. Symptoms due to alcohol abuse were alleviated. Relief of tremor was significant. Tiapride proved helpful in anxiety and depression and caused hallucinations to disappear in 35 cases.

The alcoholic patient is usually anxious. Anxiety is increased by withdrawal. The anxiety-relieving effect of tiapride was studied in 20 alcoholics, with a mean age of 44 years. 18 patients were male. Alcoholism was chronic in 18 cases and paroxystic in two. At the time of withdrawal each patient was given 3 intramuscular injections daily for 7 days, then 3 tablets per day. Results, which were evaluated according to the Hamilton score, were excellent in 9 cases, good in 10 and poor in 1: no failure was recorded. The symptoms which responded best were fear, somatic and psychic manifestations of anxiety, depressive feelings and sleep disturbances. Concomitantly, digestive disorders, anorexia, tremor and pain were alleviated. Tolerance was excellent: no neurologic, digestive, cardiovascular or biologic manifestations were recorded. In caring for alcoholic patients, the critical time of withdrawal is undeniably facilitated by the use of tiapride.

The physician-patient relationship plays a major part in the management. The consultations should be extended and fruitful for the patient, but the physician should also provide pharmacodynamic help and to achieve this tiapride is of value. Tiapride is effective against disorders of sleep, of affect and of behaviour. Twenty patients were given 300 mg tiapride daily (1 tablet three times a day) for one month. Results were as follows: in disturbances of sleep: 83,3% good results; in disorders of affect: 79% good results; in behavioural disorders: 77% good results; in alcoholic tremor: 100% good results. In the series studied tolerance was excellent in 18 cases. Only two patients had orthostatic hypotension which did not require any modification in the treatment. Tiapride therefore proves a highly effective therapeutic agent in chronic alcoholism, providing rapid action, good results and excellent tolerance with small dosages.

The purpose of this trial was to evaluate the effect of tiapride in alcohol withdrawal. The same very strict protocol was applied to each of 90 hospitalized male patients. Starting from 12 intravenous vials daily, in 4 doses, dosage was slowly brought down to 3 tablets per day, in 3 doses, by the 5th day. Effectiveness of Tiapride proved excellent, as soon as the first four days, mainly on psychiatric and digestive manifestations, while it was satisfactory on pain and less so on tremor. Overall tolerance was good. Attention is drawn to the very rapid action of Tiapride, which results in a 50% decrease in the duration of the hospital stay.
