A 13-week oral repeated dose toxicity study of suplatast tosilate (
IPD
-1151T), a new anti-allergic agent, as well as a 5-week recovery study were carried out at dose levels of 0 (control), 50, 150, 450 and 1350 mg/kg/day using male and female beagle dogs. The results were as follows: 1. In general conditions, soft feces and diarrhea with specific smell were dose-dependently observed in males and females given 450 mg/kg/day or more. Both sexes given 1350 mg/kg/day, revealed reeling with dropped head, abnormal gait, dysstasia, lying at lateral or prone position, sedation, and
tremor
, and one male and one female in this group died after showing respiratory depression, collapse and cyanosis. 2. There were no significant or remarkable changes in body weight, food consumption, water consumption, ophthalmology, electrocardiogram, urinalysis, hematology, biochemistry, fecal occult blood test, and absolute and relative organ weights. 3. Pathological examination in dead animals revealed hemorrhagic change in the heart and slight vacuolar changes in hepatocytes. In survived animals, there were no pathological changes attributable to the
IPD
-1151T. 4. In electron microscopic examination, there were no abnormalities in the liver and kidney attributable to the
IPD
-1151T. 5. After 5-week recovery period, above-mentioned changes disappeared. 6. From the above results, the non-effective dose level and the toxic dose level were estimated to be 150 mg/kg/day and 1350 mg/kg/day, respectively, and no sex differences were found.

Twenty patients of Idiopathic Prakinson's disease in the early phase were enrolled to study the efficacy and safety of selegiline as monotherapy. The mean duration of time before levodopa had to be initiated was 9.75 months. The UPDRS scores for activities of daily living, motor examination,
tremor
and total score showed significant improvement initially with subsequent worsening. Selegiline was well tolerated and there were no serious side-effects. In conclusion, selegiline may have a short lasting symptomatic effect in
IPD
and is well tolerated.

We investigated the tremolytic effect of long-acting propranolol (propranolol-LA) in six subjects with drug-induced parkinsonism (DIP), using a double-blind, placebo-controlled, crossover experimental design. Subjects received propranolol-LA for 2 weeks and placebo for 2 weeks, with no change in neuroleptic treatment.
Tremor
frequency and amplitude were objectively quantified at the end of each 2-week period by computerized tremorgram recording. There were no significant differences in attenuation of DIP
tremor
by propranolol-LA and placebo. Previous investigations reported in the literature have found propranolol to attenuate the
tremor
of idiopathic parkinsonism (
IPD
). It is expected that DIP and
IPD
tremor
would respond similarly to propranolol if a solely peripheral or spinal cord tremolytic action were operative. A possible differential attenuation of
IPD
tremor
and DIP

provides support for the concept of a higher central tremolytic mechanism of beta-adrenergic receptor-blocking drugs.

We investigated the effect of an anticholinergic (biperiden) and a dopamine agonist (apomorphine) on
tremor
, rigidity, and akinesia in patients with idiopathic Parkinson's disease. In a standardized, crossover study design 17 patients received single-dose challenges of 5 mg biperiden intravenously and a previously determined dose of apomorphine subcutaneously on 2 consecutive days. Resting (RT), postural (PT), and action
tremor
(AT) were assessed using spectral analysis of accelerometer data, and Unified Parkinson's Disease Rating Scale (UPDRS) scores for rigidity and akinesia were determined before and after administration of the study drug. Both single-dose challenges significantly reduced the amplitude of RT, PT, and AT, but only apomorphine significantly reduced UPDRS scores for rigidity and akinesia. In only one patient was
tremor
reduced by the dopamine agonist but not by the anticholinergic. We found that anticholinergic and dopaminergic agents are both effective in reducing
tremor
in
IPD
, and there was no evidence for a selective anticholinergic responsiveness of parkinsonian
tremor
. Akinesia and rigidity, on the other hand, were not improved by biperiden. We therefore conclude that dopaminergic substances are as effective as anticholinergics in patients with parkinsonian
tremor
and additionally improve other parkinsonian signs.
