In a double-blind dose response study in 26 children, 3, 6, or 12 microgram/kg of terbutaline sulfate was compared with 10 microgram/kg of epinephrine administered subcutaneously. In the first hour after injection, all doses of terbutaline and epinephrine resulted in improvement in mean clinical score, mean forced vital capacity, mean forced expiratory volume in the first second, and mean forced expiratory flow from 25% to 75% of vital capacity. Terbutaline epinephrine. However, while adverse effects following terbutaline were clinically imperceptible, epinephrine produced unpleasant headache and excitement in a few patients. Terbutaline did not change mean PaO2 or PaCO2 significantly in a subgroup of patients. The 12 microgram/kg dose of terbutaline was superior to 3 or 6 microgram/kg in relieving obstruction to airflow measured at the midportion of the vital capacity. This dose caused tremor in some children, but the tremor was not apparent to patients or their parents.

Bambuterol tablets, 10 and 20 mg, and terbutaline CR-tablets, 5 and 7.5 mg, have been compared for their capacity to produce subjectively determined tremor in a randomised, double-blind, cross-over study in 40 healthy volunteers. The duration of each treatment was one week, with an intervening washout period of at least 2 weeks. On the second and the last treatment days in each period, tremor was subjectively assessed on six occasions. In addition, an overall assessment of night and day tremor was made each morning and evening. Analyses were made for the second and last treatment days and during the treatment week. Paired comparisons for tremor during daytime showed a significant difference between the treatments, with the exception of bambuterol 20 mg and terbutaline 5 mg. Bambuterol 10 mg caused less tremor and terbutaline 7.5 mg caused more tremor than the other treatments. At the end of the week, the differences were smaller and were only significant when comparing bambuterol 10 mg with the 20 mg dose and with high dose terbutaline. Terbutaline 7.5 mg caused more tremor at night than the other treatments. The tremor experienced was generally mild, and after bambuterol 10 mg it was almost negligible (mean 0.13 after the first dose). Two subjects recorded a score of 3 after terbutaline 7.5 mg. The results indicate that, in doses equipotent with regard to bronchodilation, tremor is less pronounced after bambuterol as compared to controlled release tablets of terbutaline.

1 The effects of oral doses of the bronchodilator drugs, salbutamol (4 and 8 mg) and terbutaline (5 and 10 mg), on physiological tremor, bronchial tone, heart rate and blood pressure were studied in six normal subjects. 2 Both drugs produced a significant increase in physiological tremor, compared with placebo. Terbutaline, but not salbutamol, produced a dose related response. No significant differences were observed between drugs. 3 Terbutaline (5 and 10 mg) and salbutamol (8 mg) produced a significant bronchodilatation compared with placebo. Dose related responses were obtained with both drugs, although this reached significance only with terbutaline. 4 Both doses of each drug produced a tachycardia, although this was only statistically significant with the high doses. 5 No significant change in blood pressure was observed with either drug. 6 Peak effects of salbutamol occurred between 1 and 3 h and terbutaline between 1 and 4 hours. Both drugs were still active at 6 hours.
