A detailed study of a population of dogs with kennel cough was undertaken. Twenty-seven (77 per cent) of a total of 35 dogs had pathological evidence of respiratory disease in the form of tracheobronchitis with, in some animals, exudative pneumonia. A variety of viral and bacterial agents were isolated from the respiratory tract of diseased dogs but Bordetella bronchiseptica and canine parainfluenza virus SV-5 appeared to be the most significant organisms recovered.

A prospective, randomized, single-blind comparison of parenteral cefamandole and ampicillin was conducted in 27 hospitalized adult patients with pneumonia or purulent tracheobronchitis due to Haemophilus spp. Patients received either parenteral cefamandole or ampicillin in a dose of 1 g every 6 h. Cefamandole was as effective and safe as ampicillin. Of the 14 patients treated with cefamandole, 13 were considered cured, as were 12 of the 13 treated with ampicillin. One patient in each treatment group improved clinically but did not clear his sputum of Haemophilus spp. One patient treated with cefamandole had a recurrence of Haemophilus spp. bronchitis 9 days after cure. Adverse effects were more common in the cefamandole-treated group (50% versus 15%), but were mild and did not require discontinuation of therapy in any patient. The in vitro susceptibilities of 64 clinical isolates of Haemophilus spp. to 10 antibiotics were determined. Cefamandole was the most active of the cephalosporin-cephamycin antibiotics tested, inhibiting 98% of 61 non-beta-lactamase-producing isolates at 2 mug/ml and 100% at 4 mug/ml. Cefamandole inhibited the three ampicillin-resistant isolates at 2 mug/ml or less. Cephapirin, cefoxitin, and cephalothin were the next most active, whereas cefazolin and cephradine were the least active.

Serial tracheal cultures for aerobic and anaerobic bacteria were obtained from 27 pediatric patients during one year of follow-up. The patients had required tracheostomy and prolonged intubation for periods ranging from 3 to 12 months (average, 7 1/2 months). Cultures of tracheal aspirates yielded 1,508 isolates of pathogenic aerobic (969 isolates) and anaerobic (539 isolates) bacteria. The most frequent aerobic isolates were Streptococcus pneumoniae and Staphylococcus aureus. The predominant anaerobes were anaerobic gram-positive cocci, Fusobacterium nucleatum, and Bacteroides fragilis. Replacement of one pathogen by another occurred frequently. Tracheobronchitis occurred in 24 patients, all of whom had episodes of pneumonia. The data suggest that anaerobic bacteria are a part of the bacterial flora in colonization, tracheobronchitis, and pneumonia in patients with tracheostomy and prolonged intubation.

Six collie dogs, eight weeks old, were inoculated intramuscularly with an aluminium hydroxide adjuvanted preparation of killed Bordetella bronchiseptica; the inoculation was repeated after two weeks. Two weeks after the second inoculation, the vaccinated dogs and a comparable group of six unvaccinated animals were challenged by exposure to an aerosol of pathogenic B bronchiseptica. All six unvaccinated control dogs developed respiratory disease characterised by persistent coughing. In contrast, four of the vaccinated dogs remained free from clinical respiratory disease while, in the other two dogs, disease was less severe and of shorter duration than in controls. At necropsy, there were only slight changes in the lungs of vaccinated dogs but in controls there was a severe tracheobronchitis with areas of exudative pneumonia. Bacteriological examination showed a marked reduction in the numbers of B bronchiseptica isolated from the respiratory tract of vaccinated animals compared with controls. An aluminium hydroxide adjuvant vaccine may be of value in controlling naturally occurring respiratory disease in dogs in which B bronchiseptica is involved.

In a retrospective review of 53 patients, 58 episodes of infection due to Acinetobacter calcoaceticus var. anitratus (Herellea vaginicola) were studied. Although the organism is widely distributed in nature, it is of relatively low virulence since colonization is more frequently noted than infection and since most infections occur in patients subjected to the epidemiologic pressures common to nosocomial, gram-negative bacillary infection: prior antibiotic therapy; instrumentation and manipulation (e.g., endotracheal intubation, urinary bladder catheterization, arterial and venous cannulation); surgery; hospitalization, especially with residence in an intensive care unit; severe underlying disease, either systemic (e.g., chronic obstructive pulmonary disease, malignancy) or localized to the infected area (e.g., prior bacterial or aspirational pneumonia, trauma). Pneumonia was the most common infection due to A. calcoaceticus, and occurred only in patients with a tracheostomy or endotracheal tube in place. In over half the 25 patients, more than one lobe was involved and bronchopneumonia was the usual roentgenographic appearance. Cavitation (2 patients) and empyema formation (3 patients) were uncommon. The severity of acinetobacter pneumonia is reflected in the high mortality rate (44% overall, with a 36% mortality rate due primarily to infection). Tracheobronchitis due to A. calcoaceticus was less severe than pneumonia since no patients died primarily as a result of the infection. Urinary tract infections occurred in five patients, none of whom were ill and none of whom died. Urinary bladder catheterization was thought to be responsible for infection in three patients, and in at least four of the five patients infection was restricted to the lower tract. Wound infections were noted in six patients who had undergone surgery and were related to the presence of foreign bodies in the operative site in five of the patients. Surgical debridement and/or drainage of the infected area was the primary therapeutic measure employed in most cases. Only one patient died and this was a result of noninfectious causes. Skin infection due to A. calcoaceticus was seen in two patients, one of whom exhibited fulminant, fatal cellulitis and septicemia in the setting of pancytopenia. All nine patients with acinetobacter septicemia had received antecedent antibiotic therapy, and in all cases intravenous catheters were in place at the time bacteremia occurred. Clinically, seven of the nine patients were in shock. The mortality rate was 44% overall, with a 22% mortality rate due to infection. Although septicemia was thought to be "line-related" in five of the nine patients, serious post-bacteremic complications developed in three patients: prosthetic valve endocarditis, suppurative thrombophlebitis and subhepatic abscess.

The AA. present the results obtained determining by the F.C. test the antibodies for respiraotry viruses and for Mycoplasma pneumoniae over 1112 patients, of which 742 affected with pathological respiratory processes, during the period 1971-1975. 104 patient have shown a significant correlation between respiratory disease and antibody movement for M. pneumoniae. They have been classified according to their clinical-radiological properties as follows: 38 affected with pneumonia; 37 affected with pleuritis; 29 affected with tracheobronchitis. Particularly interesting is the range of occurrence of the disease: we have noticed a high incidence in the period 1971-1972, followed by an almost complete disappearance of pathological instances in the period 1973-first semester of 1975, and signs of a new recrudescence in the second semester of 1975. These data have been confirmed also by investigations carried out among healthy people. It has also to be pointed out the possibility of family's epidemicity, and the AA. report two epidemic cases under personal observation.

Progressive pulmonary insufficiency appears to be a universal response to the lung to a variety of injuries which damage the pulmonary-capillary emdothelium. Persistent hyperventilation, unresponsive to the administration of oxygen, is the earliest clinical sign of this complication of trauma and should prompt close monitoring of pulmonary function (measurement of arterial blood gas and pH levels, Vd/Vt A-aDo2, minute ventilation, vital capacity and inspiratory force) to assess the severity of the disease, the need for mechanical ventilatory support and the effectiveness of treatment. Other pulmonary complications of burn injury range from carbon monoxide poisoning and narcotics overdosage in the immediate postburn period through marked hyperventilation directly related to burn size occurring in the absence of significant parenchymal change to later occurring hematogenous and airborne pneumonia. Inhalation injury, a chemical tracheobronchitis which significantly increases the mortality of a given-sized burn, may be present immediately postburn but clinically inapparent for 48-72 hours. 133Xenon lung scans permit early diagnosis of this pulmonary injury and the timely institution of a graduated therapeutic response keyed to the severity of pulmonary disability. Knowledge of the pathogenesis of each of these complications is requisite for the physician caring for burn patients and permits the employment of rational preventive and therapeutic measures.

The efficacy of respiratory support devices has been compromised by respiratory infection possibly related to the support mechanism itself. Differentiation between airway contamination (tracheobronchitis) and parenchymal infection (pneumonitis) is clinically significant, as is the differentiation of respiratory infection from other foci of sepsis in the complicated surgical patient. Serial quantitative tracheal cultures provide excellent objective measures of the presence, progression, and/or resolution of respiratory infection with few false positive or negative observations. Indeed, such observations often allow earlier definitive diagnosis of infection than can be achieved with conventional clinical, chemical, or roentgenographic studies. The method represents a useful supplement to the care of the patient requiring respiratory support when infection is a realistic possibility.

Oesophageal perforations associated with cervical fractures occur from a variety of injuries. Fractures of the cervical spine, blunt trauma and penetrating injuries such as gunshot wounds, knives and missiles, perforate the cervical oesophagus. This retrospective study consists of 24 patients with an oesophageal perforation and cervical fracture. Motor vehicle accidents were responsible for 54% of the oesophageal perforations. The other oesophageal injuries were related to anterior spine surgery, gunshot wounds and sports-related activities. The clinical features related to these injuries included the obvious signs of an oesophageal perforation as well as fever of unknown origin, leukocytosis and unexplained persistent tachycardia. A variety of techniques was used to establish the diagnosis. All the patients had treatment for the cervical fracture and 20 patients required surgical repair of the oesophagus. The most common oesophageal complications were stricture of the oesophagus (54%) and oesophageal diverticulum (10%). The other complications were atelectasis, pneumonia, tracheobronchitis, pulmonary embolism, cervical osteomyelitis, cervical abscess, mediastinitis, septicemia and cervical fistulae. These patients have a serious life-threatening illness that may be difficult to diagnose and treat.

Respiration was tested in 66 patients with middle (15) and lower (51) esophageal cancer. In 32 patients damages in respiration biomechanics before surgery have been observed. In 26 patients respiratory failure was associated with concomitant diseases (pneumosclerosis, lung emphysema, cardiovascular diseases, etc.). 28 patients developed postoperative complications, in 22 of them lungs were affected (pneumonia, tracheobronchitis, pleuritis, pleural emphysema). Those complications were more frequently encountered in patients with signs of respiratory failure before surgery (72.7% of cases). It has been shown that with PEF less than 65%, FVC less than 85%, MEF 75% less than 65%, MEF50% less than 70%, MMF less than 70% and MVV less than 65% of due values, the likelihood of pulmonary complications in the postoperative period is enhanced.
