The author describes a 47-year-old patient in whom lingual hemiatrophy developed one month after angina. The development of hemiatrophy was preceded by occipital headaches and pain behind the ear on the side of hemiatrophy. In the differential diagnosis the author excluded inflammatory processes neoplasms and developmental anomalies and thinks that the cause of this short-lasting hemiatrophy might have been
tonsillitis
with compression of the nerve by the oedematous inflamed tissues with action of bacterial toxins on the nerve.

The aim of this study was to characterize glycosaminoglycan components in human palatal tonsils and to determine whether the proportions of individual glycosaminoglycans change with bacterial infection of the palatal tonsils and after recovery. The changes in glycosaminoglycans' content in the course of
tonsillitis
were mainly related to heparin and chondroitin-4-sulphate (increase) and hyaluronic acid (decrease). After recovery the polydispersity of glycosaminoglycans was found to be comparable to that in normal palatal tonsils.

Renal biopsies of 43 patients who developed renal complications after treatment with antibiotics were studied. The treatment with antibiotics in these cases was used for many different reasons such as: bronchitis, bronchopneumonia, cystitis,
tonsillitis
, sepsis, peritonitis, gangrene of the foot and tuberculosis. The renal function of these patients, before the treatment with antibiotics was normal. The biopsies were studied by light, electron and immunofluorescence microscopy. In 43 cases treated with antibiotics renal changes were shown. Three types of morphologic changes were found: acute tubular necrosis (ATN) (13 cases), acute tubulo-interstitial diseases (ATID) (21 cases), focal glomerulonephritis with crescents (FGN) (9 cases). The renal pathologic changes were most commonly seen in patients treated with 2 groups of antibiotics: aminoglycosides (21 cases) and antibiotics of the penicillin group (15 cases). The most characteristic feature of aminoglycosides is their direct toxic effect leading to ATN. Antibiotics of the penicillin type more commonly caused an allergic reaction leading to ATID (secondary to cellular mechanisms) or FGN (secondary to a predominantly humoral mechanism). Renal changes in the use of other antibiotics were much less manifest and were usually due to a hypersensitivity reaction. Cephalosporins, if used in combination with other antibiotics can increase their nephrotoxicity.

The only treatment method in chronic palatine
tonsillitis
is the removal of tonsils (tonsillectomy). The study aimed at a clinical evaluation of postoperative fusafungine use in patients who underwent tonsillectomy. The study included 200 patients who underwent tonsillectomy for chronic palatine pyotonsillitis in the Otolaryngology Clinic of the Military Institute for the Health Services in Warsaw between 2000 and 2002. The patients were divided into two groups, each group including 100 patients: group I--patients who underwent tonsillectomy, including 67 females and 33 males, aged 16 to 52 (mean age 34.8), in whom fusafungine was used; group II--patients who underwent tonsillectomy, including 58 females and 42 males, aged 17 to 56 (mean age 36.1), who did not receive fusafungine. The subjects were observed during the first 14 days following the operation. The treatment was administered from the first day after the operation, by spraying 4 doses into the throat, 4 times a day, at 8.00 hrs, 12.00 hrs, 16.00 hrs and 20.00 hrs. The two groups were then compared in terms of: symptoms of sore throat, body temperature, swallowing difficulties, the course of healing, appearance of the pharyngeal mucosa, quality of life and general affection. Post-tonsillectomy fusafungine use was found to improve the patients' quality of life, appeases the sore throat and swallowing difficulties, decreases the need for general antibiotic therapy; the above results are statistically significant. Fusafungine was not found to influence body temperature, the appearance of the pharyngeal mucosa or the healing process.

Deep neck infection is relatively rare but potentially life threatening complication of common oropharyngeal infections. This retrospective study was aimed at analyzing the occurrence of complications, diagnostic methods and proper management of deep neck infection. A review was conducted in 32 cases who were diagnosed as having deep neck infection from 1995 to 2005. The causes of deep neck infections were

(16 cases), tooth diseases (6 cases), paratonsillar abscess (4 cases), parotitis (1 case), pussy lymphonodes after tonsillectomy (2 cases), pussy congenital neck cyst (1 case), chronic otitis media (1 case), parotitis (1 case), foreign body of the esophagus (1 case). All the puss bacterial cultivation were positive. All the patients were treated by different ways of chirurgical drainage and use of large dosage of antibiotics. Deep neck infection should be suspected in patients with long lasting fever and painful swelling of the neck and treatment should begin quick as possible.

We present a chronically hemodialysed patient who developed an acute throat hemorrhage after standard tonsillectomy, planned as a standard surgery for purulent
tonsillitis
being contraindication to kidney transplantation in this case. Hemorrhage diagnosed firstly as a local post tonsillectomy complication proved to be caused by in the acute stomach ulcer. In the discussion we present the differentiate diagnosis as well as procedures which should be undertaken in hemodialysed patients who develop acute hemorrhage episodes from upper respiratory tract after tonsillectomy.

Antihistamines are very popular cold treatments. 9640 prescriptions (4069 men and 5391 women) were analysed during the January -March 2006 period. The informations of polish doctors opinions were gathered from questionnaires. The patients studied suffered from respiratory infections (common cold - 32%, laryngitis - 21%, sinusitis - 19%, bronchitis - 18%,

- 10%). In 1621 (16.8%) persons antibiotics were applied. Among them the most common amoxycilline (41%), macrolides (26,4%) and cephalosporines (22.8%) were prescribed. The coexistence of repiratory infection with allergic disease was confirmed in 6181 (65%) patients. The combination of antihistamine - cetirizine (Alermed, Lekam, Poland) with other medications were applied in every patient treatment. Anti-inflammatory properties in 36%, allergy in anamnesis in 14% and allergic symptoms in 28% were pointed out by polish doctors as the main reasons for prescriptions of cetirizine. Only 7% of doctors admitted in survey that they always prescribed antihistmines for respiratory infections. Duration of illness was shorter than seven days in most patients. In polish doctors opinions cetirizine may be applied in respiratory infections, because of its effects in alleviating clinical symptoms and in shortening of illness duration. Especially this treatment is indicated when coexistence of respiratory infection with allergic disease is confirmed. Additionally cetirizine is safe and good tolerated drug. However, further controlled studies are necessary to confirm these data.

Tularemia is a rare zoonosis occuring in many clinical forms, including ulceral, glandular, oropharyngeal, pneumonic, and septic form. ENT specialists seeing their patients in ambulatory and emergency settings are most likely to encounter oropharyngeal and glandular form. Tularemia became widely discussed clinical entity in recent years due to its potential to being used as a biological weapon in acts of terrorism. Authors present a case of a 75 yrs old woman treated for atypical
tonsillitis
with suppurative cervical lymphadenitis. As infection with typical pathogens was ruled out and no improvement with antibiotics was observed, further evaluation was initiated. Malignancies and tuberculosis were excluded. Final diagnosis was established based on a serological test. Epidemiology, clinical manifestations, diagnostic procedures and treatment of tularemia, as well as diagnostic pitfalls are briefly discussed.
