A retrospective review of charts for 650 children who had lumbar puncture for suspected meningitis was undertaken to determine the characteristics of patients with and without meningitis, identify other conditions suggesting meningitis, and evaluate the predictive value of signs and symptoms of meningitis. The incidence of positive lumbar punctures increased with patient age. Younger infants did not present with classical features of meningitis. Bulging fontanel, lethargy, and irritability were nonspecific symptoms. Vomiting and headache, although not specific, proved to be more sensitive indicators of meningeal infection. Most patients with meningitis (75%) had at least one sign of meningeal irritation, but so did 25% of patients without meningitis. Brudzinski's sign was not specific. In contrast, nuchal rigidity and Kernig's sign had high predictive value. Up to age five, the diseases most often suggesting meningitis were right-sided pneumonia, gastroenteritis, otitis, tonsillitis, exanthema subitum, and urinary tract infections. Of 171 patients with febrile convulsion, one (0.5%) had bacterial meningitis and four had aseptic meningitis.

Portsmouth is a nine-bedded mixed adult and paediatric general intensive care unit (ICU) There is great controversy about nursing children in adult ICUs but it occurs. The aim in this care study is to show how this problem was approached in one instance and how care could have been improved. Samuel was a 13-month-old boy who presented to his general practitioner (GP) with a 2-day history of feeling unwell, miserable and lethargic. The GP provisionally diagnosed epiglottis and referred him to the paediatric unit at St Mary's Hospital. The diagnosis was later changed to severe tonsillitis. His condition deteriorated over 24 h and he was referred to the intensive care unit (ICU) team, intubated and admitted to the ICU. Betty Neuman's systems model (Aggleton & Chalmers 1989) was used as a basis for Samuel's care. This incorporates the physiological, psychological, socio-cultural, developmental and spiritual aspects of an individual's needs. In Samuel's case this involved care of his family as well as himself. All names have been changed to protect the individuals involved.

Geotrichum candidum was cultured from the tonsils of a free-ranging weaner pig that was presented with lethargy and inappetence. Histopathology indicated pyogranulomatous tonsillitis with intralesional fungal hyphae and arthrospores. Geotrichosis is a rare, opportunistic mycosis of immunocompromised hosts, usually human beings.
