A prospective aetiologic analysis made of 70 children and adults with the clinical and haematologic features of infectious mononucleosis (IM) in Sao Paulo, Brazil, revealed 65.7% due to EBV (31 heterophile antibody positive (HA+), 15 HA negative (HA-) cases), 8.6% due to T. gondii and 4.3% due to CMV. One case was related to viral hepatitis and one to drug hypersensitivity. The other 18.6% were of unknown cause. In the 46 IM cases positive tests were found for EBV/IgM in 93.5%, for heterophile by the absorbed horse Rbc test in 64.2% and by sheep Rbc in 37%. The immune-adherence haemagglutination test was slightly more sensitive than the horse Rbc test in 39 IM sera tested. In 41 EBV/IgM positive sera, EBV/IgA was present in 17/25 containing heterophile antibody and in 2/16 lacking heterophile antibody; anti-EA was present in 85%. The average age of HA+ IM cases was 13.2 years and of HA- cases 4.7 years. Three HA+ and 5 HA- IM cases occurred in the 0-2 year old age group. Few clinical features differentiated between aetiological agents. Exudative tonsillitis was the most helpful one and occurred in 67.7% of HA+ IM cases, 26.7% of HA- cases, in 1 of 3 CMV cases, and not at all in 6 T. gondii infections.

Measurements were made of serum and urine myoglobin in 48 patients with leptospiral jaundice (LJ) and 56 patients with various acute infections. At the height of LJ blood myoglobin level reached 28.96 +/- 4.3 micrograms/l (normal concentration 0.315 +/- 0.002 microgram/l). Compared to acute pneumonia, acute viral hepatitis, tonsillitis, erysipelas, diphtheria, health values, the ratio of serum myoglobin to urine myoglobin in leptospirosis made up 45.25 against 5.4, 4.8, 6.8, 3.7, 1.8 and 1.3, respectively. A relationship was found between concentrations of myoglobin, bilirubin, creatinine in the blood and leptospirosis severity. Elevation of serum myoglobin as a manifestation of specific myositis is pathognomic for leptospirosis and contributes to the onset of acute renal failure and disturbance of bilirubin metabolism. Quantitation of blood myoglobin may be helpful as an additional test for leptospirosis severity.

Antibodies (Abs) hydrolyzing proteins, DNA, and RNA are detected in the blood of patients with various autoimmune diseases. In the present work, homogeneous preparations of IgG Abs from the blood of the healthy donors as well as patients with A, B, C, and delta types of viral hepatitis, influenza, pneumonia, tuberculosis, tonsillitis, duodenal ulcer, and some types of cancer were purified. For the first time, the fraction of IgG and its Fab fragments of patients with viral hepatitis were shown to have high DNA- and RNA-hydrolyzing activity. In case of Abs from the healthy donors and patients with other diseases, high activity of Abs was not detected. The data obtained by various methods indicate that the activity of hepatitis Abs is an intrinsic property of the immunoglobulins. The relative rates of hydrolysis of cCMP, poly(U), poly(A), poly(C), and tRNA(Phe) by hepatitis Abs were compared with those of RNase A and other RNases from human blood. Significant differences in activities of Abs and nucleases in hydrolysis of model substrates were demonstrated. Thus, catalytically active Abs can appear in the blood of patients not only with autoimmune disorders, but with viral diseases as well.

The purpose of the study was to assess the impact of alcohol abuse on the composition of serum lipids in patients with tonsillitis. The values of the serum lipid spectrum were studied in 100 alcohol abusers with Streptococcus-induced lacunar tonsillitis, 100 alcoholics without markers of viral hepatitis, and 50 healthy individuals. The effect of alcohol was shown in elevated triglyceride levels in patients with alcoholism and tonsillitis. Exposed to bacterial infection, the tonsillitis patients who abused alcohol had higher levels of phospholipids and free cholesterol and lower concentrations of free fatty acids and cholesterol esters than the alcoholics. The tonsillitis patients who abused alcohol were observed to have more considerable changes in the spectrum of phospholipids as a low relative content of lysophospholipids and higher levels of phosphatidylcholine and phosphatidylethanolamine than the alcoholics, which may be due to the predominant impact of bacterial infection.
