One hundred patients suffering from recurrent acute tonsillitis were studied. One hundred tonsils were cultured. Surface bacteria were compared to deep bacteria. In 48 per cent of the cases a different pathogen was cultured from the interior of the tonsil as compared to the surface culture. This fact may explain recurrence of
tonsillitis
and makes the reliability of the conventional tonsillar culture questionable. Surgical extirpation of the tonsils seems to be the only treatment since deep bacteria remain unidentified and resist the antibiotic therapy that may affect only the surface microflora.

Forty-eight cases of otorhinolaryngological infections were treated with amoxicillin (AMPC) at a daily dose of 750mg. The clinical and microbiological effects were studied, and the results were summarized as follows. 1. The subjects comprised 20 cases of otitis media, 10 of

, 4 of sinusitis, 4 of chroditis, 2 bronchitis, 5 of furuncle of the ear and 3 of furuncle of the nose. The clinical effective rate of AMPC was 82.9%, and the microbiological effective rate was 80.6%. 2. The effect of AMPC against strains isolated from the above diseases was also studied. The effective rate against Streptococcus was 91.6% and against Staphylococcus 83.3%. 3. Side effects were observed in 4 cases (one of diarrhea, two of abdominal discomfort and one of lingual pain), but none of them was so severe as the use of AMPC should have been discontinued.

To study the effect of tonsillectomy as a possible part of the treatment of infection-released, relapsing and recalcitrant (to topical therapy and courses of penicillin) psoriasis vulgaris in children and adolescents and to test a possible correlation between
tonsillitis
and exacerbations of psoriasis vulgaris, a retrospective study (charts and questionnaires) of the course of psoriasis after tonsillectomy was undertaken in 74 patients with such psoriasis. Each patient served as his own control. At tonsillectomy the average age of patients and the duration of psoriasis were 14-2 years and 4-5 years, respectively, while the average follow-up period was 4-5 years. The clearing of psoriasis vulgaris was stastically significant, p less than 0-01, as 1/3 of the patients obtained clearing of psoriasis throughout the entire follow-up period, while an additional 1/3 noticed considerable improvement of their psoriasis. After having tried both topical therapy of various sorts and courses of penicillin, tonsillectomy might be taken into consideration in relapsing, recalcitrant psoriasis vulgaris in children and adolescents.

The differential diagnosis must include deep neck-space abscesses, congenital cysts, and benign as well as malignant neoplasms. A diagnosis can be derived from a consideration of the history and physical findings, the age of the patient, and the location, size, and consistency of the mass. A careful otolaryngologic exam--including the nasopharynx, a chest radiograph, and appropriate blood studies--should be routine. When malignancy is suspected, a biopsy of the mass is indicated.

The results are discussed of a retrospective study of 126 patients by whom an excess of the fusospirochaetal complex was found by direct microscopic examination of tonsillar material. These bacteria are not only found in unilateral ulcero-necrotic

, but also in different other tonsillar and general diseases. Vincent's angina appears almost exclusively in young persons, aged from 16 to 25 years; in the adolescence (16-20 years) it forms about 40% of all cases of acute tonsillitis. The fusospirochaetal complex is also often found in chronic tonsillitis and even in morfologically normal tonsils, especially in patients with general diseases like rheumatic arthritis. In one third of the patients with infectious mononucleosis, the complex is present in the tonsillar lesions. The occurrence of Vincent's angina is double as high in women than in men. From our assessments we can conclude that the fusospirochaetal complex has a very low pathogenity, and that it only can grow in patients with a reduced local or general resistance, even without causing local lesions.

Amoxicillin granule (100 mg AMPC potency per 1.0 g granule) was administered to pediatric patients with the following results. 1) AAMPC serum concentration and the urinary recovery of two preparations (Clamoxyl 'Beecham' and Sawacillin 'Fujisawa') granule were compared in a cross over trial in 5 healthy adult volunteers. No significant difference in the results was observed between the two granules. 2) The peak serum concentration after oral administration of 10 mg/kg AMPC to 5 children gave an average of 5.06 mug/ml 1 hour after administration. The 6-hour urinary excretion was distributed 51.4 approximatley 78.9%. 3) AMPC was administered at a dose of about 20 mg/kg/day to 35 children with acute pediatric infections that is scarlet fever, acute tonsillitis,

lacunalis, acute bronchitis, bronchial asthma +bronchitis and infectious impetigo. An effective therapeutic result was obtained in all cases. 4) The granule was well accepted by young children, and at a dose level of about 20 mg/kg/day for a week no disorder on hepatorenal function was observed in any og the patients.

The usual treatment for peritonsillar abscess in the United States is incision, drainage and antibiotic therapy followed by tonsillectomy several weeks later. Why this treatment began is not clear but it probably originated from fear of complications which might arise from operating during the acute stage. This therapy requires two hospitalizations and tonsillectomy after previous abscess can be difficult.
Tonsillitis
or peritonsillar abscess can recur any time after the initial abscess is treated. A series of 50 cases of peri-tonsillar abscesses treated by immediate tonsillectomy is presented and evaluated. The results show this operation has a low morbidity and no significantly greater complication rate than elective tonsellectomy.
