1.2 mega U Penicillin V potassium was given to 20 patients 2 hours (group 1) and another 19 patients 3 hours (group 2) before tonsillectomy. After the operation biological determinations of the levels of active principle were made from tonsillar tissue and serum with statistical evaluation of the results (Spearman's rank correlation). There was a significant connection between the serum and tonsillar tissue concentrations (in children and adults) in both groups. The level of the active principle in group 2 was only slightly lower than in group 1. Many of the micro-organisms responsible for tonsillitis can be influenced by the concentrations found here, so that application of Penicillin V can often be expected to have a good therapeutic effect.

In the otolaryngology literature, little attention is given to pharyngeal gonococcal infections. In the past, gonorrheal stomatitis was emphasized; recent publications dealt especially with tonsillitis. A gonococcal focus in the tonsil is not easily eradicated and often leads to diseminated gonorrhea with arthritis. Homosexual contacts are of importance. Among 196 patients with suspected venereal disease (93 men and 103 women), 74 had genital or rectal gonorrhea, or both; two female patients had an isolated gonococcal pharyngeal infection. A positive gonococcal pharyngeal culture was obtained from one homosexual man who had tonsillitis. In all cases of tonsillitis in "young" adults, the otolaryngologist should be aware of the possibility of a gonorrheal infection. Even if there is the slightest suspicion, a tonsillar culture for gonococci is required, for which suction of the crypts is recommended.

Treatment of inflammations of the oral and pharyngeal cavities (glossitis, pharyngitis, pharyngolaryngitis, tonsillitis) in 60 patients with Larypront Throat Tablets alone, without administration of systemic antibiotics, gave very good results in 49 cases (81.6%) with complete regression of the complaints in 3-4 days. In 11 patients with acute pharyngitis and tonsillitis without demonstrable effect, systemic antibiotic therapy had to be added because of radiologically demonstrable acute sinusitis or increasing antibacterial infection. Side effects were not seen with the use of Larypront in any case.

44 children with acute tonsillitis were treated without the usual penicillin therapy. 22 children received lozenges containing Dequalinium chloride, 22 children received void lozenges under conditions of a double blind study. The results show that the treatment with Dequalinium chloride did not influence conditions caused by haemolytic streptococci -- verified by bacteriological examinations of pharyngeal smears -- inspite of its efficiency in vitro. Also general symptoms like fever, leukocytosis and rapid erythrocyte sedimentation rate were not influenced by Pequalinium chloride. Local symptoms like sore throat, tonsillar redness and exsudation were influenced beneficially. It can therefore be assumed that in cases of acute tonsillitis penicillin therapy is not necessary except in cases of streptococci infections. These should be excluded by pharyngeal smear. The medium of Liebermeister and Braveny is a very simple indicator to differentiate between tonsillitis caused by streptococci and tonsillitis of other origin.

We have undertaken some basic and clinical studies on midecamycin granules with following results: 1) After ingesting of 4 g of midecamycin granules, peak blood levels (1.51 microgram/ml on an average) appeared at one hour in infants, detectable amount lasting for 6 hours. 2) Urinary excretion within 6 hours ranged from 1.1 to 2.7% of the drug dosed. 3) In the treatment of a total of 19 acute cases, consisting of 9 cases of tonsillitis, 7 cases of lacunar tonsillitis and 3 cases of bronchitis, midecamycin was found effective in 79% of the cases. 4) In all the 3 cases of pneumonia due to Mycoplasma, response to midecamycin was assessed as excellent. 5) Hepatic and renal functions tests performed in cases treated with the drug for a prolonged period (40 approximately 50 mg/kg for 13 approximately 18 days) revealed no undesirable effect, indicating that midecamycin can be administered continuously to younger infants with infections.

Clinical and laboratory studies on S-6437 were made, and the following results were obtained. 1. Thirty pediatric patients with various types of infections such as tonsillitis, bronchitis, pneumonia, cystitis, pyelitis, lymphadenitis colli and pyodermia were treated with S-6437 at the daily dosage of 50 mg/kg orally, the clinical effectiveness was 89.3%. 2. The peak blood level of cephalexin after a single oral administration (25 mg/kg) was observed after 4 hours and the average peak blood level of 10 cases was 7.4 microgram/ml. 3. The average urinary excretion rate of 5 cases was 71.2%. 4. Mild side effects were noticed in a few cases.

In order to clinically evaluate S-6437, the following study was carried out in pediatric patients. This clinical study was performed in 30 patients ranging from 2 years and one month to 10 years and one month of age. Seven patients had scarlet fever, 3 acute pharyngitis, 4 acute suppurative tonsillitis, 6 acute bronchitis, 2 acute pneumonia, 3 acute pyelonephritis, 1 chronic pyelonephritis, 2 vaginitis, 1 acute gastro-enteritis, and 1 impetigo. The degree of these diseases were all mild or moderate. These patients were orally administered 35 approximately 50 mg/kg/day in two divided doses for 3 approximately 10 days. As a result, effectiveness of this preparation in these patients was 80% and no side effects were observed.
