During the winter of 1982 concurrent outbreaks of influenza A and influenza B occurred. The epidemiology and clinical features of 151 cases referred during this time are described, and patients are discussed according to age and presenting clinical syndrome: croup was the commonest presentation in young children, a typical influenza syndrome predominated in young adults, while older patients were more likely to have lower respiratory tract infection. There was no significant difference between the clinical features of influenza A and influenza B. Unusual clinical features include rash, exudative tonsillitis and the need for myringotomy during the course of influenza.

The significance of BKV infections relative to infections by generally tested respiratory agents was investigated in children with acute respiratory disease. Paired sera from 177 children admitted to a hospital for acute respiratory disease were tested for significant rises in antibodies. Sera from seven patients showed a seroconversion to BKV and clinical signs of acute upper respiratory tract infection were exhibited by each of these patients. BKV infections were present in 8% of the patients with upper respiratory tract disease while seroconversions to adenovirus (2%), influenza A virus (1%), parainfluenza virus (5%), RS virus (6%) and mycoplasma pneumoniae (1%) were observed in 15% of the patients with upper respiratory tract disease. BKV was isolated from the urine of one child with tonsillitis with a concomitant seroconversion to BKV. Tonsils from children with recurrent attacks of acute respiratory disease were tested for the presence of BKV DNA by hybridization with a cloned genomic 32P-labeled DNA of prototype BKV. Five of twelve tonsil DNAs showed hybridization with BKV DNA. Each tonsil showing hybridization with BKV DNA contained multiple nonintegrated copies of the BKV genome per diploid amount of host cell DNA. Attempts to recover infective BKV by transfection of primary human embryonic cells with tonsil DNAs or by co-cultivation of tonsillar cells with primary human embryonic cells were unsuccessful.

Infectious psychoses were studied clinically in 60 children divided into three age groups: 3-7 years, 7-12 years and 12-17 years. Acute respiratory diseases, influenza, tonsillitis, otitis were the etiologic factors of psychoses. Short-term delirium states characterized by visual and tactile hallucinations prevailed in the first group. Affective-asthenic syndromes prevailed in the second group children. Their psychoses were characterized by both visual and acoustic illusions. Protracted or periodical disorders of the consciousness including psychosensory, senestopathic and depersonalization ones prevailed in the third group.

Infections of the upper airways reduce life quality of adults on average twice, that of children even 4 to 8 times a year. Usually these are viral infections subsiding spontaneously without antibiotics. Only if concomittant circumstances arise and in specific clinical situations (otitis, sinusitis, tonsillitis, epiglottitis) as well as in acute exacerbation of chronic bronchitis are antibiotics already indicated at first consultation. The physician who is usually consulted by a patient with increasing discomfort under self-medication and who is expecting an antibiotic treatment has next to the clinical findings and prior laboratory test results to consider before all the individual (home, workplace, school) and epidemiologic (influenza) situation, the possible pathogens, their susceptibility to antibiotics as well as the side effects and costs for the decision to prescribe antibiotics. Today because of an increasing pressure of selection for pathogens resistant to antibiotics a greater reluctance to prescribe antibiotics is recommended. This requires close follow-up of the patient with symptomatic treatment or only short "probatory" antibiotic therapy and a good patient-physician relationship.

Antibodies (Abs) hydrolyzing proteins, DNA, and RNA are detected in the blood of patients with various autoimmune diseases. In the present work, homogeneous preparations of IgG Abs from the blood of the healthy donors as well as patients with A, B, C, and delta types of viral hepatitis, influenza, pneumonia, tuberculosis, tonsillitis, duodenal ulcer, and some types of cancer were purified. For the first time, the fraction of IgG and its Fab fragments of patients with viral hepatitis were shown to have high DNA- and RNA-hydrolyzing activity. In case of Abs from the healthy donors and patients with other diseases, high activity of Abs was not detected. The data obtained by various methods indicate that the activity of hepatitis Abs is an intrinsic property of the immunoglobulins. The relative rates of hydrolysis of cCMP, poly(U), poly(A), poly(C), and tRNA(Phe) by hepatitis Abs were compared with those of RNase A and other RNases from human blood. Significant differences in activities of Abs and nucleases in hydrolysis of model substrates were demonstrated. Thus, catalytically active Abs can appear in the blood of patients not only with autoimmune disorders, but with viral diseases as well.

An outbreak of upper respiratory infection (influenza-like syndrome) took place among the staff members of a large office building in Beijing, June, 1997. To understand the cause of the outbreak, with a case-control study with serological antibodies detection against serogroup 1 to 14 of Legionnaires' disease bacteria (LDB) and bacteriological test and culture on samples collected from the air-conditioning system of the office building was performed. The findings showed that the attack rates of tonsillitis and upper respiratory infection were 61.86% and 34.62% respectively. The relation was statistically significant between the symptoms of fever and tonsillitis in the cases (chi 2 = 77.88, P < 0.01). The positive rates of LDB antibodies on serogroups 9, 10, 12, 14 were 45.94% in the cases of upper respiratory infection and 3/15, 6/17, 0/15, 1/16 respectively in the four controls groups. Condensied water samples from an air-conditioning system showed that Legionella Pneumophila was positive. Evidence showed that it was an outbreak resulting from LDB serogroup 9 and 12, accompanied by upper respiratory infection related to air-conditioning system contaminated by LDB. This was the first report Lp infection associated with an air-conditioning system in China.

Antibiotic resistance is associated with prior receipt of antibiotics. An analysis of linked computerized databases for physician visits and antibiotic prescriptions was used to examine antibiotic prescribing for different respiratory infections in preschool children in Canada. In 1995, 64% of 61,165 children aged <5 years made 140,892 visits (mean, 3.6 visits per child) for respiratory infections; 74% of children who made visits received antibiotic prescriptions. Antibiotics were prescribed to 49% of children with upper respiratory tract infection, 18% with nasopharyngitis, 78% with pharyngitis or tonsillitis, 32% with serous otitis media, 80% with acute otitis media, 61% with sinusitis, 44% with acute laryngitis or tracheitis, and 24% with influenza. Acute otitis media accounted for 33% of all visits and 39% of all antibiotic prescriptions. The estimated Canadian-dollar cost of overprescribing was $423,693, or 49% of the total cost of antibiotics ($859,893) used in this group. This population-based study confirms antibiotic overprescribing in Canada.

The content of medium-weight molecules (MWM) in the plasma was evaluated by the screening method in 280 patients with prevalent infectious diseases (viral hepatitides A, B, and C, influenza, acute dysentery, alimentary toxicoinfections, and tonsillitis). Control group consisted of 70 donors. The maximum levels of MWM were observed at the peak of disease. In viral infections the increase in the level of MWM was longer. The values of MWM depended on the disease severity and presence of the intoxication syndrome. Plasma concentration of MWM correlated with the presence of the leading clinical syndromes in infectious diseases. Measurement of MWM level in the plasma of infectious patients can be used as a criterion of recovery and a prognostic and additional diagnostic test.

As the main target of influenza viral aggression, the respiratory tract is subject to easier bacterial infection superimposition. The researchers from Les Laboratoires Servier--France, managed to isolate a substance--fusafungine--from the microspore of the fungus Fusarium lateritium, which demonstrates unique anti-inflammatory and antibiotic action, and is the active ingredient of Bioparox Spray, an inhalant. The principal indications of Bioparox Spray for treatment of respiratory tract infections fall within the range from the sinuses to the finest alveolar duct, namely: rhinitis, sinusitis, tonsillitis, pharyngitis, laryngitis, tracheitis and bronchitis. Air pollution in all its forms, including sulfur dioxide, ozone, fine particles, carbon monoxide and nitrogen oxides, has resulted in human deaths and diseases worldwide. This article reports on the human suffering caused by air pollution in terms of mortality and morbidity. Based on interviews with scientists, health experts and victims, it is noted that the cities of Thailand, Mexico, Japan, Poland, the Czech Republic, Romania, and the US have the highest levels of air pollution. In these areas people suffer from respiratory illnesses such as pharyngitis, sinusitis, laryngitis, tonsillitis, bronchitis, asthma, flu, and loss of lung function. A most alarming finding indicates that residents of Los Angeles exposed to ozone pollution have double the risk of cancer compared to residents of cleaner cities. Aggravating this situation is the fact that governments often opt to sacrifice human health and lives when forced to choose between protecting the public and shielding industry from pollution regulations.
